Tissue and tumor mosaicism of the myotonin protein kinase gene trinucleotide repeat in a patient with multiple basal cell carcinomas associated with myotonic dystrophy.

We describe the third case (to our knowledge) of multiple basal cell carcinoma associated with myotonic dystrophy and carry out a genetic study of the tumor comparing it with healthy skin. We consider that our results show that this association might be not a purely random phenomenon and that the particular genetic characteristics of this disorder might have a role in the pathogenesis of the tumor.

Presentation of telangiectasia macularis eruptiva perstans as a long-standing solitary plaque associated with renal carcinoma.

BACKGROUND: Mastocytosis is a rare disease characterized by a primary pathological increase in mast cells in different tissues. The skin is the most frequently affected organ. Cutaneous mastocytosis, including urticaria pigmentosa, solitary mastocytoma, diffuse cutaneous mastocytosis, and telangiectasia macularis eruptiva perstans (TMEP), is usually distinguished from systemic mastocytosis. TMEP is characterized mainly by telangiectatic macules. OBJECTIVE AND METHODS: We report a case of TMEP with an unusual clinical presentation as a solitary plaque of telangiectatic macules. A renal clear cell carcinoma was detected in a workup for systemic mastocytosis. We discuss the clinical and histological findings and treatment of TMEP. CONCLUSIONS: TMEP is a rare form of mastocytosis, which occurs mainly in adults, generally has a good prognosis, and little tendency to urticate or show constitutional symptoms. Clinicians should consider this disorder when confronted with a progressive atypical telangiectatic lesion. However, the malignant neoplasm also found in this patient is of uncertain significance.

Localization of Sweet's syndrome in radiation-induced locus minoris resistentae.

Sweet's syndrome is a reactive dermatosis clinically characterized by fever, leukocytosis, and multiple, erythematous, painful plaques. Histopathologically, the lesions show edema of the papillary dermis and a dermal infiltrate mainly composed of neutrophils, without vasculitis. We describe a patient with breast carcinoma and Sweet's syndrome mostly involving previously irradiated skin.

Subungal abcesses secondary to paclitaxel.

Various cutaneous side effects, including nail changes, have been associated with taxane chemotherapeutic agents, but usually docetaxel has been implicated. We report a patient with acute paronychia due to paclitaxel administered for treatment of breast cancer.

Porokeratosis of the lower lip.

The porokeratoses are defined as specific disorders of keratinization that are histologically characterised by the presence of a dense, parakeratotic column, well-circumscribed from the rest of the corneocytes, known as the cornoid lamella. In this article, we report two cases of porokeratosis of the lower lip, drawing attention to the unusual site. We also emphasize the clinical appearance of these lesions, which may easily be misdiagnosed if not examined correctly. As a consequence of this misdiagnosis, the prevalence of these lesions is probably greater than at present estimated. Moreover, a good response to cryotherapy is shown.
