The correlation between root canal patterns and interorificial distance in mandibular first molars.

OBJECTIVES: The aim of this study was to investigate the relationship of the distance between mesiobuccal (MB) and mesiolingual (ML) orifices (interorificial distances) in the pulp chamber floor and the degree of canal curvature in the mandibular first molars. STUDY DESIGN: In this in vitro study, 102 extracted teeth were radiographed in both buccolingual (CV) and mesiodistal (PV) directions and then grouped according to the Vertucci classification. The interorificial distance was measured by the stereomicroscope method. RESULTS: The interorificial distance was significantly higher in type IV compared to type II (P= .016). In the logistic regression analysis, the increase in the degree of primary canal curvature of the ML in CV was a predictive factor for increase in the primary curvature of ML in PV (P = .01). CONCLUSION: The interorificial distance in the pulp chamber floor and primary curvatures in CV were found to be key factors for evaluation of root canal patterns in PV.

Comparative analysis of autoantibodies against a-fodrin in serum, tear fluid, and saliva from patients with Sjögren's syndrome.

OBJECTIVE: To evaluate levels of IgA and IgG antibodies against a-fodrin in serum, tear fluid, and saliva and compare them with anti-Ro and anti-La antibody levels in the same samples of patients with Sjögren's syndrome (SS). METHODS: Samples from 25 patients with SS (17 primary and 8 secondary), 8 patients with systemic lupus erythematosus (SLE), and 7 patients with rheumatoid arthritis (RA) as well as 20 healthy blood donor controls were collected. Antibodies were measured using ELISA. RESULTS: Although 40% of patients with primary SS had IgG anti-a-fodrin in their sera, it was also found in 36% and 32% of samples of their tear fluid and saliva, respectively. IgA a-fodrin antibodies were detected in 32% of SS sera, 20% of tear fluid samples, and 32% of saliva samples. Although the level of IgG anti-a-fodrin was significantly greater in serum, tear fluid, and saliva of SS patients compared to controls (p < 0.001), a significant difference was observed only in serum and saliva. While anti-Ro was detected in 48%, 56%, and 24% of serum, tear fluid, and saliva samples, respectively, anti-La was found in 40%, 44%, and 28%. Significant association was observed between serum IgG antibodies against a-fodrin and dry eye symptom score and rose bengal staining score. A negative association was also noted between tear IgA antibodies against a-fodrin and Schirmer I test. CONCLUSION: Correlation of IgG and IgA antibodies against a-fodrin with the severity of eye involvement suggests that these autoantibodies may be considered activation markers of SS.

Serum mannose-binding lectin levels are decreased in behcet's disease and associated with disease severity.

OBJECTIVE: To investigate serum levels of mannose-binding lectin (MBL), a complement-like protein of collectin family, in patients with Behcet's disease (BD). METHODS: MBL levels were measured in sera of 130 patients with BD, 64 patients with recurrent oral ulcerations (ROU), and 105 healthy controls (HC) with ELISA. RESULTS: Patients with BD had significantly lower median serum MBL levels compared to HC (1857 vs 3136 ng/ml, p = 0.001). No significant difference was observed in median serum MBL levels between BD and ROU (2309 ng/ml, p = 0.252). Low MBL levels (</= 500 ng/ml) were present in a higher proportion of BD patients compared to HC (29% vs 16%, p = 0.021). A severe disease course (total clinical severity score >/= 4) was more frequently observed in BD patients with very low serum MBL levels (</= 100 ng/ml) (19% vs 6%, p = 0.046). When serum MBL levels were analyzed separately according to gender, the frequency of vascular disease was higher in men with very low serum MBL levels (80% vs 42%, p = 0.042). CONCLUSIONS: MBL deficiency might contribute to the pathogenesis of BD and affect its clinical course.

Financing of dental services in Turkey: opinions and expectations of dentists, residents, and patients.

OBJECTIVES: This study investigates the attitudes of dentists, residents, and patients on the financing of dental services in Turkey. METHODS: In this descriptive study, randomly selected groups of 860 dentists, 641 patients, and 866 residents from 9 provinces of Turkey were interviewed. Their agreement on various statements on financing issues of dental services were recorded and analyzed. RESULTS: Eighty-two percent of the dentists, 83.8 percent of the patients, and 76.4 percent of the residents agreed that "governments should finance all dental services." The majority of the dentists (83.6%), patients (74.7%), and residents (84.4%) stated that patients should pay a copayment for their dental treatment. CONCLUSION: Survey results indicate that the current financing system of dental services in Turkey is not satisfactory either for the providers or for the users.

Patient satisfaction in dental outpatient clinics in Turkey.

AIM: To measure the level of patient satisfaction in the dental outpatient clinics from a group of state hospitals in Turkey and explore the factors that may influence satisfaction. METHODS: In a cross-sectional study, patients attending the outpatient clinics state hospitals from 9 provinces were interviewed. Data regarding socio-demographic characteristics of the respondents were collected and a patient satisfaction questionnaire, which consisted of 29 statements regarding various aspects of satisfaction, were completed during face-to-face interviews (n=641, response rate 89%). RESULTS: There was no difference between the mean satisfaction scores of the study group with regard to socio-demographic characteristics except for the type of health insurance held by the patient. Patients covered by state pension funds had significantly higher satisfaction scores than the self-employed group with public insurance. Patients were most satisfied with privacy (82.1% of the respondents). Waiting time was the least satisfactory issue, with only 41.8% of patients reporting satisfaction with waiting time. CONCLUSION: The type of health insurance of the patients seems to be a significant predictor of dental patient satisfaction in Turkish state hospitals. Privacy was the most satisfactory service attribute whereas the waiting time had the lowest satisfactory rating.
