In vitro activity of beta-lactam and non-beta-lactam antibiotics in extended-spectrum beta-lactamase-producing clinical isolates of Escherichia coli.

The activity of different beta-lactam and non-beta-lactam antibiotics was assessed against extended-spectrum beta-lactamase (ESBL)-producing and non-ESBL-producing clinical isolates of Escherichia coli. A phenotypic study to discover the presence of ESBLs in 399 clinical isolates of E. coli was made by the disk diffusion method following the Clinical and Laboratory Standards Institute (formely NCCLS, 2004) guidelines. The activity of different antibiotics was subsequently studied using the automated VITEK 2 system (bioMérieux, Marcy l'Etoile, France). One hundred fifteen isolates proved to be ESBL-producing and 284 non-ESBL-producing. Among the former, percentage susceptibilities to the antibiotics assayed were meropenem and amikacin, 100%; piperacillin/tazobactam, 97.4%; cefepime, 94.8%; amoxicillin/clavulanic acid, 84.3%; tobramycin, 84.3%; gentamicin, 83.5%; cefoxitin, 83.5%; nitrofurantoin, 71.3%; cotrimoxazole, 46.1%; norfloxacin, 29.6%; ciprofloxacin, 27%; and ofloxacin, 26.1%.

[Human herpesvirus 6 and multiple sclerosis].

In this work the possible etiologic relationship among the multiple sclerosis and the herpesvirus 6 (VHH-6) has been studied, subject debated in the last years. They have been carried out two studies using blood and LCR of sick of multiple sclerosis, and control in healthy fellows and with other neurological illnesses. The followed techniques are described to isolate the virus, their DNA, and the antibodies IgG and IgA in front of the VHH-6. The presente of the VHH-6 has not been demonstrated neither of its DNA in any case, but if it has been proved, with statistical significance that in the sick persons with buds of multiple sclerosis, the antibodies of the type IgG, as much in plasm as in LCR, was more present and in higher levels that in the controls.

Introduction of HIV type 1 non-B subtypes into Eastern Andalusia through immigration.

A study of the distribution of HIV-1 subtypes in the native and immigrant populations of Eastern Andalusia (Southern Spain) was conducted to determine any changes between 1983 and 2001 and to identify antiretroviral resistance mutations in non-B subtype strains among the immigrant population. The study included 111 native patients from Eastern Andalusia: 94 infected with HIV before 1996 and 17 infected since 1996. A parallel study was conducted on 26 HIV-positive immigrants from Africa. Subtyping was done with the heteroduplex mobility assay. Resistance mutations were determined by line probe assay. A total of 137 patients were studied: 9.2% had subtype A (n = 12), 80.8% subtype B (n = 105), and 1.5% subtype C (n = 2). Among the Eastern Andalusia population infected before 1996, 10.9% had non-B subtypes, compared with 23.5% of those infected after that year. The greatest percentage of non-B subtypes (52.4%) was found among the immigrant population. Resistance mutation K70R was detected in one of the six immigrants with non-B subtype and M41L in another. There has been a slight increase in the diversity of HIV-1 subtypes in Eastern Andalusia over the past few years, possibly influenced by non-B subtypes introduced by immigrants from sub-Saharan Africa.

[Bacterial adherence in pathogenesis of urinary tract infectious].

The possibility of a colonization and later urinary infection is due to a first contact among a series of structures of the bacterium, denominated adhesins (fimbrica or no-fimbrica) and some receivers or ligands of the surface of the urinary epitelium. The bacterial fimbriae of Escherichia coli, of those that have been studied up to seven different types, are protean structures coded by the chromosomal DNA, being the most important those of type 1, in connection with the colonization of the low roads, and the type P, with the cystitis and pyelonephritis. They are studied with detail their different protean components and the very complex genetic regulation of their production, made of great interest in the pathogeny of these infections and in the possibility of their prevention. The receivers of each fimbriae type are also chemically different, and their knowledge would explain important clinical data.

[Microbiology and cystic fibrosis].

The respiratory infections are the rule in sick persons of cystic fibrosis. After the colonization in one's early childhood by Staphylococcus aureus and Haemophilus influenzae, it is established the colonization produced by the mucous strains of Pseudomonas aeruginosa, the principal etiogenic of this illness, that can become to isolate in more than 95% of sick persons at three years old. That's why the etiogenic factors of this bacterium are studied, as they are toxins, pigments, enzymes, and principally the alginate, hydrocarbonated polymer responsible of the mucous substance that covers these characteristic strains of cystic fibrosis sick persons. All these components are regulated by a complex genome, the biggest until noe sequenced in bacterium, and that explains the importance of the microorganism and its natural resistance or acquired to environmental alterations and to antibiotics and disinfectants. In the evolution of the clinic chart, it can appear others gramnegative bacteriums (among it emphasizes Burkholderia cepacia and Stenotrophomonas maltophilia), mycobacterium and even fungus; all that would explain the chronification of lung charts and the difficulty of these sick persons treatment.
