The methamphetamine crisis. What EMS providers need to know to stay safe and treat patients.

Methamphetamine is a highly addictive drug that is an increasingly recognized problem for emergency responders in all areas of the country. Any clandestine drug lab is a hazardous materials incident and crime scene. Enter a clandestine drug lab only if you have proper training and equipment--do not become a victim. Methamphetamine users are generally paranoid, agitated and anxious, and may become violent during assessment and treatment. Children living at a meth lab are at greater risk for physical and/or sexual abuse and neglect.

Pediatric fluid resuscitation and airway management.

In summary, if the problem is volume shock, stop the loss and replace the fluid without overloading. IV fluids may be necessary to maintain perfusion pressure and administer medications. Pediatric airway anatomy requires different techniques and equipment for proper management, but all airway management starts with basic life support skills.

Emergency stabilization of unstable pelvic fractures.

Pelvic ring fractures resulting from high-energy and/or mass mechanisms can cause life-threatening severe bleeding, however, devices can be applied by EMS at the emergency scene to help slow or reduce that possibility. Circumferential compression is an effective and safe method to stabilize open book pelvic fractures.

Fetal trauma from motor vehicle collisions.

To summarize: The best fetal protection is proper maternal use of seat belt restraints. All pregnant occupants in a motor vehicle crash require physician evaluation. Focus on maternal assessment. Maternal stability is the best indicator of fetal stability. Key treatments are high-flow oxygen, i.v. fluid loading and immobilizing in left lateral position. Evaluate the fetus after maternal stabilization.

The nose: bleeds, breaks and obstructions.

Nosebleeds from a break and/or obstructon can result in life-threatening problems. The nose is the airway opening that warms, humidifies and filters air. Control epistaxis with well-aimed direct pressure for 10-15 minutes. Nasal fractures, which are the most common facial fracture, can be the outward sign of skull fracture, brain injury or c-spine impairment, and may pose a threat to the airway and breathing. The greatest concern of a nose obstruction is aspiration.
