[Hearing evaluation in chronic renal failure children].

UNLABELLED: Chronic renal failure (CRF) causes a lot of systemic side-effects, among them: neurological and otological complications. In present study hearing evaluation was made in young patients, using distortion product otoacoustic emissions (DPOAEs) and auditory brainstem responses (ABR). THE AIM OF STUDY: (1) objective assessment of hearing organ activity in CRF children, (2) localization of hearing organ part involved in CRF, (3) establishing best parameters of ipsilateral stimulation in DPOAEs for clinical use. MATERIALS AND METHODS: Hearing acuity was assessed in 22 CRF children ranging in age from 9 to 17 years and 18 healthy children ranging in age from 10 to 18 years. At first pure tone audiometry and tympanometry were evaluate. Only patients with normal middle ear condition and hearing threshold better than 30 dB HL were put forward further investigations. Objective assessment of hearing condition included: (1) DPOAEs using DP-gram format. Two simultaneous pure-tone signals (primaries) were presented to the ear at two different frequencies (f1 and f2, where f2 > f1) and the 2f1-f2 were analyzed. Five pairs of non equal level and L2 were used L1 and L2: panel A1: L1 = 65 and L2 = 60 dB SPL, panel A2: L1 = 63 and L2 = 55 dB SPL, panel A3: L1 = 59 and L2 = 45 dB SPL, panel A4: L1 = 55 and L2 = 35 dB SPL, panel A5: L1 = 51 and L2 = 25 dB SPL; (2) ABR investigation using click stimulus, at 90 dB nHL level. Wave I, III, V latencies and intervals I-III, III-V, I-V were evaluated. Our results reviled significantly lower mean DPOAEs amplitudes in CRF children when compare to healthy children, at all frequencies measured. No differences in ABR evaluation were observed. Most optimal DPOAEs parameters in clinical use seem to be panel A1, A2 and A3 of level L1 and L2.

[Rare case of primary sinonasal tuberculosis].

INTRODUCTION: Tuberculosis, considering entry of infection, most frequent concerns the lungs. Rarely we observed extrapulmonary cases of tuberculosis within the bones, joints, urogenital organs and lymph nodes as well as within the head and neck region--larynx, middle ear, pharynx, tonsils, tongue, parotid glands and central nervous system. Sinonasal tuberculosis, either primary or secondary to pulmonary infection, is very rare, but should be considered in the differential diagnosis of sinonasal granulomas. The diagnosis of the head and neck tuberculosis is based on the medical patientis examination as well as on the bacteriological and histological study of the biopsy specimen. AIM: The aim of the study was to show own, very rare case of primary sinonasal tuberculosis. MATERIAL AND METHODS: We described a case of primary sinonasal tuberculosis in an adult female. She was suspected of the neoplastic disease. The diagnosis was based on the histopathology. The treatment was successful and consisted with antituberculous drug therapy and surgery (Functional Endoscopic Sinus Surgery). The patient was followed up for 12 months and during this time we did not notice any symptoms of recurrence. CONCLUSIONS: Tuberculosis of the head and neck region should be take in to consideration during the differential diagnostic of the neoplastic diseases.

[Interleukin-10 in laryngeal cancer patients treated by surgery or radiotherapy].

INTRODUCTION: The early detection and right monitoring of the laryngeal cancer make a better chance of recovery. Therefore factors that might be showing the presence of neoplastic process are constantly being looked for. The detection of correlation between incidence of cancer and immunity system disorders makes us look for and try to answer an important question: are there any factors that might be cancer markers in immunity system? Cytokines are molecules that regulate the process of differentiation and proliferation. They are also the mediators of immunological and inflammatory reactions. Interleukina-10 (IL-10) is a cytokine that has various important functions stimulating immunity processes. IL-10 is a factor stimulating killer cells/cytoxic lymphocytes/ and is able to kill cancer cells growth spontaneously. It has been detected that some cancer cells can produce some IL-10, too. The aim of the study was to evaluate the serum level concentration of IL-10 in the laryngeal cancer patients before any medical treatment and its denotation after surgery or radiotherapy. MATERIAL AND METHODS: A group of 43 male patients with laryngeal cancer was considered for the purpose of this study. 22 patients were treated by surgery and 21 by radiotherapy. In all the cases the serum concentration of IL-10 before the treatment, in the first and 30th days after therapy was evaluated. The results were compared with the group of 20 healthy volunteers. RESULTS: The mean value of IL-10 concentration in laryngeal cancer patients treated by surgery was 14.0 pg/ml before treatment, 12.8 pg/ml in the 1 day and 14.4 pg/ml 30 days after the therapy. IL-10 concentrations in patients treated by radiotherapy were respectively: 15.4, 14.4 and 15.9 pg/ml. The mean value of IL-10 in the control group was 4.9 pg/ml. The serum concentrations of IL-10 in the laryngeal cancer patients were significantly higher than in the control group. These IL-10 concentrations didn't change significantly after the surgery or radiotherapy. CONCLUSION: The serum concentration of IL-10 in the laryngeal cancer patients is significantly higher than in the healthy control group. The changes of serum concentrations after the surgery or radiotherapy of laryngeal cancer patients are not significant and characteristic.

[Interleukin 2 concentration changes in the laryngeal cancer patients during the surgical treatment].

INTRODUCTION: Cytokines are molecules that regulate the process of differentiation and proliferation. They are also the mediators of immunological and inflammatory reactions. One of the cytokines that participates in immunological processes is interleukine-2 (IL-2), whose participation in neoplastic processes has been proved in many researches. It's anticancer effect is an indirect one and is connected with stimulating processes of immunologically competent cells, which are cytotoxic for cancer cells. IL-2 is produced by T-helper lymphocytes (THL), and on the basis of feedback it stimulates those lymphocytes to new cells and other cytokines, which stimulate next immunity cells or kill the cancer cells, production. The aim of the study was to evaluate the serum level concentration of IL-2 in the laryngeal cancer patients before any medical treatment and its denotation after the surgical procedure. MATERIAL AND METHODS: A group of 22 male patients with laryngeal cancer was considered for the purpose of this study. All of them were treated by surgery. In all the cases the serum concentration of IL-2 before the treatment, in the first and 30th day after therapy was evaluated. The results were compared with the group of 20 healthy volunteers. RESULTS: The mean value of IL-2 concentration in laryngeal cancer patients treated by surgery was 23.4 pg/ml before treatment, increased to 37.8 pg/ml in the 1 day and to 54.3 pg/ml 30 days after the therapy. The serum concentrations of IL-2 in the laryngeal cancer patients were lower than in the control group before the surgical treatment. These IL-2 concentrations increased significantly after the surgery and 1 month after the treatment were higher than in the healthy population. CONCLUSION: The serum concentration of IL-2 in the laryngeal cancer patients treated by surgery is lower than in the healthy control group. IL-2 increased significantly after the surgery to higher levels than in the control healthy group.

[Otoacoustic emissions measurements in children during the chemotherapy because of the acute lymphoblastic leukemia].

INTRODUCTION: Chemotherapy is associated with an increased risk of ototoxic changes. The predictive value of conventional pure-tone audiometry on early detection of ototoxicity has been questioned. Otoacoustic emissions (OAEs) appear to be more sensitive to cochlear insult than the conventional pure-audiometry. The purpose of our study was (a) investigation the clinical usefulness of Distortion Product Otoacoustic Emissions (DPOAEs) as early indicator of chemotherapy-induced ototoxicity, (b) determination which of the protocols of chemotherapy is most ototoxic as measured by DPOAEs, (c) comparison of the short-term and long-term effects of chemotherapy on DPOAEs. MATERIAL AND METHODS: Tonal audiometry (0,25-8 kHz), immitance audiometry and DPOAEs were measured in 10 children with acute lymphoblastic leukemia (ALL). Measurements were performed before and after each protocol of ALL IC-BFM 2002 chemotherapy: protocol I: vincristine (VCR), L-asparaginase (L-ASP), daunorubicin (DNR), cyclophosphamide (CPM), cytarabine (ARA-C), 6-mercaptopurine (6-MP), methotrexate (MTX); protocol mM: 6-MP, MTX; protocol II: VCR, doxorubicin (DOX), L-ASP, CMP, ARA-C, thioguasine (6-TG), MTX; protocol III: VCR, DOX, L-ASP, CMP, ARA-C, 6-TG, MTX. DPOAEs were measured using ILO2 92 Otodynamics Analyser. Cochlear activity was evaluated by recording 2f1-f2 DPOAEs with L1 = 65 and L2 = 60 dB SPL. Comparisons of the DP-grams amplitudes were performed between baseline measurements and those recorded before and after each chemotherapy course. RESULTS: Our results indicate that: a)DPOAE is a more sensitive technique for the assess of chemotherapy-induced ototoxicity than conventional audiometry, b) with DPOAE monitoring very subtle hearing changes can be detected, c) DPOAE amplitude was significantly decreased at all frequencies studied in 50% children with leukemia, d) depression of DPOAE amplitude was evident only during and after first protocol, e) long-term DPOAE monitoring reviled reversibility of ototoxicity in all children, f) a large individual variability in the DPOAE response following the chemotherapy was observed, g) in a few cases a transient increase in DPOAE amplitude had been observed before it was reduced. CONCLUSIONS: Distortion product otoacoustic emissions measurements are very sensitive on early detection of the changes in cochlear function and are recommended for monitor hearing in patients during chemotherapy.

[An unusual parotid gland foreign body].

A foreign body in the parotid gland is extremely uncommon. It can penetrate to the parotid gland through the skin or from the oral cavity via the Stenonís duct. A case of 23-years-old man with the several pieces of glass within the superficial lobe of the right parotid gland was described. The surgical removal was performed and the diagnosis of the foreign body was quite easy.

[Perceptual method and videostroboscopy in patients with vocal fold hypertrophy underwent microsurgery].

INTRODUCTION: Chronic hypertrophic laryngitis concerns many people in our population. Their etiopathogenesis is complicated and usually connected with exogenous harmful factors like occupational or environmental exposure. Organic voice disorders of the larynx, particularly concerning vocal folds are the cause of dysphonia. Both surgical and conservative methods in the treatment of patients with laryngeal hypertrophy are applied. Objective assessment of the voice improvement after microsurgery is the subject of discussion of many authors. The problem of voice improvement is rarely approached to in the comprehensive way, that is with reference to activity and anatomy of the larynx. The basic aim of this research was to asses phonation function of the larynx after microsurgery on the basis of subjective and videostroboscopic voice analysis. The second aim was to estimate the surgical treatment effectiveness depending on vocal fold hypertrophy extensiveness. MATERIAL AND METHOD: The research was conducted on 50 subjects chosen from the population of patients with vocal fold hypertrophy treated at the Department of Laryngology Silesian Medical Academy in Zabrze from January to December 2003. All the examined patients underwent classical microsurgery. Voice quality examinations were done just before microsurgery and 3 months after the operation. On the basis of histopathological results, following diagnosis were established: vocal fold polyps (oedematosus, teleangiectaticus and inflammatorius), oedema Reincke, laryngeal papilloma, laryngeal granuloma and cyst. The research methodology included: perceptual voice examination based on GRBAS scale (G--grade; R--roughness; B--breathiness; A--asthenicity; S--strain) and assessment of vibrations of vocal folds in videostroboscopy. Examination results in two groups with different extensiveness of vocal fold hypertrophy were compared. The control group presented 30 subjects without any voice disorders who did not use their voice for professional purposes. RESULTS: In both groups (with minor and extensive vocal fold lesions) significant improvement in perceptual and videostroboscopic voice parameters was observed. The results in the group with minor vocal fold lesions were better after microsurgery. CONCLUSIONS: The methodology based on the application of the two methods of voice examination could be an objective rate in the estimation of the treatment progress.

[Histopathologic changes of the soft palate in snoring and obstructive sleep apnea syndrome patients].

Histology of the soft palate and uvula in snorers and patients with OSA syndrome has been a subject of investigation of many authors. In majority of specimens the hypertrophy of the salivary glands as well as congestion and dilation of the thin-walled vessels were observed. Some of the samples presented atrophy of the muscle bundles. Also, the inflammatory changes as lymphocytic infiltrations were proven. In each case adults served as a control group. In this study the histologic analysis of the soft palate and uvula samples from patients suffering from snoring and OSA syndrome was performed. The uvula samples from the newborn who died on the first day of their lives were chosen as a control group. The choice of such a group excluded the influence of vibration force on the soft palate structure. Comparisons among the groups were made by Fisher exact two-tailed test. The muscular atrophy was observed only in patients with airway disturbances. No case of neonatal tissue sample with this pathology was found. Less dilation and congestion of the blood vessels were observed in the newborn group. Significantly more frequent superficial salivary glands localized between the muscle bundles and epithelium were found in the OSA and snoring patients. Our results showed distinct differences between the tissues of the patients with airway disturbances and the control group. These differences may be caused by the influence of the vibration on the soft palate and uvula, but on the other hand they may be the reason for excessive flaccidity of these structures and disturbances occurring during sleep.

[The evolution of epidemiology and clinical characteristics of laryngeal and hypopharyngeal carcinoma in Poland from 1991 to 2001].

On the base of retrospective analysis of 12,888 cases of carcinoma of larynx and hypopharynx, diagnosed in 19 ENT Departments in Poland from 1991 to 2001, the assessment of basic epidemiological data, including the localization of tumor and stage of local and clinical advancement of the disease at the time of diagnosis has been conducted. In analyzed period of 11 years the trends to change of the mentioned above parameters has been examined. The significant increase of female patients in this period was observed, with average proportion M:F = 8:1. The glottis localization of carcinoma dominated (47.6%), followed by supraglottis (40.8%) and pyriform fossa (7.8%), with significant increase of pyriform fossa tumors in the analyzed period of 11 years. In the majority of cases the carcinoma of larynx and hypopharynx was diagnosed in the advanced stage (T3 + T4) of local disease, with the highest percentage in localization within the pyriform fossa (81.0%), and the lowest percentage in glottis tumors (45.6%). The regional lymph nodes metastases has been diagnosed in 46.7% of the analyzed group, with the highest percentage in tumors localized in pyriform fossa (82.9%), and the lowest percentage in tumors of glottis localization (33.1%). In the 11 years time the significant drop down of N0 cases and tendency to increase of N2 and N3 in the supraglottis localization of tumor. The distant metastases in the analyzed group at the time of diagnosis has been registered in 2.0%, with the highest percentage in posterior pharyngeal wall (7.6%) and pyriform fossa (7.4%). The authors postulate the renewal of prospective study on epidemiology, clinical characteristics and treatment results of larynx and hypopharynx carcinoma in Poland.

[Usefulness of ventilation tests in the diagnosis and evaluation of treatment results in select laryngeal diseases].

The obstruction of upper airways is a common symptom of some laryngological diseases. The patients need exact diagnosis and proper treatment. The ventilation tests seem to be one of the most important tool in process of diagnosis and treatment, however they are more pulmonological than laryngological procedures. The aim of the study was to evaluate the efficacy of ventilation tests and analysis of their interpretation in patients with laryngeal and/or tracheal obstruction. Also, we intended to show a method of spirometric examination in patients with tracheostoma. Based on our common clinical experience (more than 3000 tests in patients with upper airways obstruction in ENT Departments in Zabrze and Szczecin) we presented our observations. In majority, the tests were used in patients after laryngeal surgery due to the cancer or bilateral vocal cord paralysis and in patients with tracheostomy, before decannulation. The attention was paid on usefulness of some coefficients, especially in diagnosis of upper airway obstruction. For better results interpretation the authors introduced the term of mixed obstruction in contrast to fixed and dynamically variable narrowing. Also the FIV1/N FEV1 parameter was purposed. In conclusion the great role of ventilation tests was emphasized, especially in diagnosis of the narrowing on the larynx and trachea level and also in evaluation of the results after surgical procedures on upper airways.

[Evaluation of the cochlear efferent system in patients with cerebello-pontine angle tumor].

The aim of the study was to estimate a cochlear efferent system in the group of patents with cerebello-pontine angle tumor (CPA). The size of an efferent effect was defined as the difference of the amplitudes in click evoked otoacoustic emissions (CEOAEs) during without contralateral acoustic stimulation (CAS) and with broad-bond noise (BBN) was estimated. The research was also conducted on the influence of tumor dimensions, age, sex, and the hearing level in tonal audiometry on the size of the efferent effect. The research was carried out on a group of 14 patients with cerebello-pontine angle tumor and on an appropriate fourteen-person test group. Having our results analyzed we were able to state that, compared with the test group, in the group of patients with CPA tumor significant reduce suppressions of otoacoustic emission response during CAS occurred both in an ear on the tumor side and in an ear on the healthy side. However, the tumor dimensions, sex, and the hearing level in tonal audiometry seemed not to affect significantly the size of the efferent effect.

[Evaluation of cochlear function in children with chronic renal failure].

UNLABELLED: In present study cochlear function was evaluate in young chronic renal failure (CRF) patients using distortion product otoacoustic emissions (DPOAEs). THE AIM OF STUDY: (a) objective assessment of cochlear function in CRF children, (b) establishing best parameters of ipsilateral stimulation in DPOAEs (DP-gram format) for clinical use. Hearing acuity was assessed in 19 CRF children and 18 healthy children. At first pure tone audiometry and tympanometry were evaluate. Only patients with normal middle ear condition and hearing threshold better than 30 dB HL were put forward further investigations. Objective assessment of cochlear function included DPOAEs using DP-gram format. Our results reviled significantly lower mean DPOAE amplitudes in CRF children when compare to healthy children, at all frequencies measured. Most optimal DP-gram parameters in clinical use seems to be level L1 and L2 equal respectively: 65 and 60 dB SPL; 63 and 55 dB SPL; 59 and 45 dB SPL.

[Assessment of cochlea activity in a group of newborns with central nervous system impairment as an effect of perinatal asphyxia using click-evoked otoacoustic emissions (CEOAEs)].

During adaptation of newborn to extra-uterine environment dramatic changes in functioning in the inner organs and the entire human body is observed. Complicated or premature birth as well as complicated course of a perinatal period may cause reversible or irreversible damage of various tissues, organs or systems, and, consequently, their dysfunction. The most often changes in the central nervous system (CNS) in newborns occurring as an effect of perinatal asphyxia are found to be ischemic-hypoxic encephalopathy (IHE), periventricular leucomalacia (PVL) and intraventricular hemorrhages (IVH). Chronic hypoxia is considered to affect the hearing organ in newborns. Reversible or irreversible changes within the cochlea, brainstem or cortex may result in perceptive hearing losses. The aim of the study was to carry out the objective assessment of the cochlea activity using CEOAEs in neonates with CNS impairment occurring following perinatal asphyxia. To the investigation 16 newborns with IHE, PVL or IVH were included. The control group encompassed the health newborns matched as to the age. Perinatal anamnesis, general pediatric status, results of trans-fontanel ultrasonography and biochemical test results were taken into account in statistical analyses. In all newborns otoscopic examination and CEOAEs after birth and 3 months later were performed. CEOAEs in session 1 were significantly reduced in the investigated group comparing to control babies but 3 months later no differences between groups were observed. The outer hair cells activity in first days of life is reduced in newborns with asphyxia in anamnesis but with time no differences in cochlea development are observed.

[Treatment of otosclerosis in children].

Between 1994 and 2004 in the ENT Department Silesian Medical University Zabrze 330 stapedotomy were performed in patients suffered from otosclerosis. In this group of patient's 16 stapedotomies in 13 children aged from 9 to 17 years were performed. 4.85% of all the patients with otosclerosis treated in our Department were children. The purpose of this study was to analyze the audiologic results of stapedotomies in children. The postoperative evaluations were performed in the period from 6 months to 8 years after the surgical treatment. In the hearing state evaluation we analyzed the change of the air-bone gap, an average air conduction hearing improvement, the rate of percentage hearing improvement and an average hearing loss by Bell Telephone Laboratories. In all the operated children we observed the decrease of the air conduction hearing level. Stapedotomy was successful (postoperative air bone gap within 10 dB) in 62.5% of the cases. In 5 cases the air bone gap was lower than 20 dB (31.25%). In one case it was greater then 20 dB. The air bone gap closure was at the average of 4.39 dB level. The results were comparable with the other authors. Stapedotomy can be an effective procedure for correcting conducting hearing losses due to the otosclerosis in the pediatric patients.
