Development of quartz-crystal-microbalance-based immunosensor array for clinical immunophenotyping of acute leukemias.

An integrated piezoelectric immunosensor array has been developed to immunophenotype acute leukemias in clinic. Each quartz crystal microbalance (QCM) was fabricated with plasma-polymerized film of n-butylamine, nanogold particles, and protein A (PA) to be used to immobilize antibodies in orientation. Leukemic lineage-associated monoclonal antibodies were separately immobilized onto the nanogold-PA-modified surface of the crystals, which were constructed by a 2 x 2 type of probes forming a QCM-based immunosensor array. The main detection conditions were investigated, including the immobilization amount of antibodies, pH, immunoreaction time, sample dilution ratio, etc. The immunophenotyping feasibility of the new technique was investigated through simultaneously analyzing Jurkat cells by the immunosensor array method, immunohistochemistry, and flow cytometry. It was found that the developed technique could readily identify leukemia samples in 5 min and might monitor dynamically the immunoreaction processes. Moreover, comparison studies were carried out for CD antigens expressed on the nucleated cells isolated from 96 acute leukemic patients and 24 normal subjects using the QCM-based immunosensor method and the fluoroimmunoassay. Results obtained by immunophenotyping patients' samples with the immunosensor-based method achieved the rate of 88.93% in 768 groups of numerical data, where no significant statistical difference was observed between the two methods when checked by chi2 analysis (chi2 = 3.4, p > 0.05). This new immunosensor array showed the merits of high sensitivity, high specificity, good reproducibility, easy operation, and low cost. The results of specimen evaluation indicated that it might be clinically suitable for quantifying human differentiated leukocytes and immunophenotyping of acute leukemias.

[The diagnosis and treatment of special airway foreign body].

OBJECTIVE: The aim of this study was to discuss the diagnosis and treatment of special airway foreign body. METHOD: Clinical data of 69 patients with airway foreign body were analyzed,of which 45 patients underwent rigid bronchoscopy while another 24 patients underwent tracheotomy. RESULT: The operations succeeded in 43 of the 45 patients who underwent rigid bronchoscopy, while the other 2 patients died of respiratory failure because of the foreign body slipping off at the level of the glottis. All of the 24 other patients were cured, in which continuous saturation of blood oxygen below 90% occurred in 8 patients and turned normal after positive airway pressure in intensive care unit (ICU). The total cure rate reached 97.1%. CONCLUSION: Most of special foreign body in the airway can be taken out through tracheotomy under general anesthesia. If the foreign body is too big and irregularly, or the patients is too young or have the pulmonary atelectasis, it is much safer to remove foreign body by tracheotomy.

[Imaging diagnosis of tumor in parapharyngeal space].

OBJECTIVE: To investigate the diagnostic value of CT and MRI in tumors of parapharyngeal space (PPS). METHOD: The data of one hundred and thirty-eight patients with PPS tumor treated in our department during the period from January, 1988 to January, 2005 were collected and reviewed. Eighty-three men and 55 women were enrolled, not including patients with metastasis tumors in PPS or nasopharyngeal carcinomas invading PPS. The age varies from 7 to 73 yrs(average 44.7 yrs). Among the patients, 12 were operated for the second time because of recurrence. All the patients were examined by CT or/and MRI. RESULT: CT and MRI could provide the position, size, confine of the tumor and its relationship with cervical blood vessels well, which were important to operation schemes. CONCLUSION: CT and MRI before operation are valuable to the treatment of PPS tumors.

Relationship between the expression of CD44v6 and development, progress, invasion and metastasis of laryngeal carcinoma.

The expression of CD44v6 and its relationship with the development, progress, invasion and metastasis of laryngeal carcinoma was investigated. The expression and content of CD44v6 mRNA in tissuess were detected by both RT-PCR and FCM which were respectively extracted from normal laryngeal mucosa, leukoplakia of larynx, laryngeal papilloma, polyp of vocal cord, tissues of laryngeal carcinoma, metastatic and nonmetastatic lymph nodes of neck, and tissues close to carcinoma. The outcome of RT-PCR indicated that the expression rate of CD44v6 mRNA involved in tissues of laryngeal carcinoma and metastatic lymph nodes of neck was the highest (90%-100%) compared with that of leukoplakia of larynx, laryngeal papilloma, tissues close to carcinoma by 0.5 cm (55.56%-60.00%) and that of normal laryngeal mucosa, polyp of vocal cord, nonmetastatic lymph nodes and tissues close to carcinoma by 1.0 cm was the lowest (13.33%-20%). The result from FCM was highly consistent with that from RT-PCR. It was suggested that CD44v6 was closely related with the development, progress, invasion and metastasis of laryngeal carcinoma. The outcome from the tissues close to carcinoma by different distance could do help to the determination of incisal edge in surgery abstractly.

[Factors affecting on the surgery of polyp of vocal cord].

OBJECTIVE: To analyze the factors affecting on the surgery of polyp of vocal cord so as to enhance the success rate of surgery and improve the treatment effect. METHOD: Two thousands seven hundreds and ninety-two cases with polyp of vocal cord were summarized retrospectively according to the types of surgery they received. The factors affecting on the three kinds of surgery were analyzed which including the differences of patients, the types of polyp, the skill of surgeons, the instrument and the anaesthesia. RESULT: The unsuccessful rates in group A, group B and group C were 6.46%, 4.12% and 1.53% respectively. The occurrence rates of complications in three groups were 0.19%, 0 and 1.16% respectively. The proportion of the factors affecting on the surgeries in group A, B, C were the differences of patients 61.77%, 6.67%, 85.71%; the types of polyp 27.94%, 60.00%, 0; the skill of surgeons 30.88%, 46.67%, 0%; the instrument 8.82%, 13.33%, 14.29% and the anaesthesia 16.18%, 33.33%, 0%. CONCLUSION: The main factor affecting the surgery of polyp of vocal cord in group A, C is the differences of patients. The main factor inducing to the complications of surgery is the anaesthesia. All factors affecting on the surgeries should be considered carefully before operation and the necessary measures to avoid the complications must be took.

[Clinical therapic procedure of obstinate epistaxis].

OBJECTIVE: To investigate the clinical treatment consideration and the therapic procedure. METHOD: We analyze the clinical data of 171 cases of obstinate epistaxis in recent years, mainly the pathogeny, treatment consideration and therapic procedure. RESULT: Were two steps were selected to the therapeutics according to different causations: step one, using the methods including nasal septum rectification, nasal cavity microwave cauterization, the curative ratio were 87.20%, 91.25% respectively; step two, using the methods including DSA (digital subtraction angiography), ligation of the external carotid artery, arteria ethmoidalis clipping, the curative ratio were 90%, 100%, 100% respectively. CONCLUSION: The thought of taking two steps methods to treat the obstinate epistaxis is very clear, and the curative effect is satisfactory as well.
