[Socioeconomic context and perceived oral health in an adult population in Rio de Janeiro, Brazil: A multilevel analysis].

One of the main limitations of the ecological design is aggregation bias. The problem can be addressed by designs that use small contextual hierarchical units with internal homogeneity. This study aimed to compare different geographic data structures in order to explore contextual effects of socioeconomic variables on the odds of perceived poor oral health. The analyses were based on a mixed design that included data from civil servants participating in the "Pró-Saúde" censuses I and II and residing in the city of Rio de Janeiro, Brazil, in 1999 (n = 2,426); contextual data were obtained from the National Demographic Census conducted in 1991. We used the variance partition coefficient to compare different geographic hierarchical structures. In this study, the smallest geographic partition (census tract) was the best unit for contextual analysis for studying variability in perceived poor oral health. Individual socioeconomic attributes explained more of the variance in perceived oral health than did the socioeconomic contexts of residential areas.

Predicting smear negative pulmonary tuberculosis with classification trees and logistic regression: a cross-sectional study.

BACKGROUND: Smear negative pulmonary tuberculosis (SNPT) accounts for 30% of pulmonary tuberculosis cases reported yearly in Brazil. This study aimed to develop a prediction model for SNPT for outpatients in areas with scarce resources. METHODS: The study enrolled 551 patients with clinical-radiological suspicion of SNPT, in Rio de Janeiro, Brazil. The original data was divided into two equivalent samples for generation and validation of the prediction models. Symptoms, physical signs and chest X-rays were used for constructing logistic regression and classification and regression tree models. From the logistic regression, we generated a clinical and radiological prediction score. The area under the receiver operator characteristic curve, sensitivity, and specificity were used to evaluate the model's performance in both generation and validation samples. RESULTS: It was possible to generate predictive models for SNPT with sensitivity ranging from 64% to 71% and specificity ranging from 58% to 76%. CONCLUSION: The results suggest that those models might be useful as screening tools for estimating the risk of SNPT, optimizing the utilization of more expensive tests, and avoiding costs of unnecessary anti-tuberculosis treatment. Those models might be cost-effective tools in a health care network with hierarchical distribution of scarce resources.

Spatial analysis of the distribution of leprosy in the State of Ceará, Northeast Brazil.

The aim of this study was to describe spatial patterns of the distribution of leprosy and to investigate spatial clustering of incidence rates in the state of Ceará, Northeast Brazil. The average incidence rate of leprosy for the period of 1991 to 1999 was calculated for each municipality of Ceará. Maps were used to describe the spatial distribution of the disease, and spatial statistics were applied to explore large- and small-scale variations of incidence rates. Three regions were identified in which the incidence of leprosy was particularly high. A spatial gradient in the incidence rates was identified, with a tendency of high rates to be concentrated on the North-South axis in the middle region of the state. Moran's I statistic indicated that a significant spatial autocorrelation also existed. The spatial distribution of leprosy in Ceará is heterogeneous. The reasons for spatial clustering of disease rates are not known, but might be related to an heterogeneous distribution of other factors such as crowding, social inequality, and environmental characteristics which by themselves determine the transmission of Mycobacterium leprae.

[Short version of the "job stress scale": a Portuguese-language adaptation].

OBJECTIVE: To describe the adaptation to Portuguese of the short version of the "job stress scale", originally in English. METHODS: We evaluate six aspects of equivalence between the original scale and the Portuguese version: conceptual, semantic, operational, item, measurement, and functional equivalences. A reliability test-retest study was conducted with 94 selected subjects. RESULTS: Reproducibility (interclass correlation coefficients) for the 'demand', 'control', and 'social support' dimensions of the scale was estimated at 0.88, 0.87, and 0.85, respectively. Internal consistency (Cronbach's alpha) estimates for these same dimensions were 0.79, 0.67, and 0.85, respectively. CONCLUSIONS: Our results suggest that the adaptation of the scale was successful, and indicate that its use in the sociocultural context of the studied population (Pró-Saúde survey) is appropriate.

[Gender and leisure-time physical activity].

This article discusses the relationship between gender and the social construction of the body, specifically focusing on physical exercise during leisure time. The Pró-Saúde Project is a prospective study consisting of 4,030 employees of a university in Rio de Janeiro, in which we analyzed the answers on leisure-time physical activity (LTPA) by 3,740 participants. The results show a prevalence of males in LTPA. Age, schooling, and per capita family income were directly associated with exercise among women, as compared to per capita family among men. Men were more frequently involved in group sports and physical activities that required more strength than women, including football, tennis, volleyball, martial arts, jogging, and weight lifting. Women performed more individual physical activities and those demanding less strength, like walking, dance, gymnastics, and hydrogym. The results suggest that LTPA is a domain of daily life that is organized according to certain conventions, amongst which gender-related conceptions concerning the ideal body, where men and women display distinct behaviors in relation to physical exercise.

Inequality and leprosy in Northeast Brazil: an ecological study.

BACKGROUND: Leprosy is an important public health problem in many developing countries and many features of its determinants are still obscure. METHODS: To investigate whether the incidence of leprosy is related to certain environmental and socioeconomic determinants, an ecological study was undertaken in 165 municipalities of the state of Ceará, Brazil. Social, economic, education, sanitation, demography, meteorology, and health data were collected. The dependent variable was the average incidence rate of leprosy from 1991 to 1999. Simple and multiple linear regressions were performed to assess the relationship between the dependent and the independent variables. RESULTS: The average incidence rate for all the municipalities for the 1991-1999 period, varied from 0.06 to 14.68 per 10000 persons per year. The level of inequality (beta = 1.67, P = 0.011), the mean years of study among the population >or=25 years old (beta = 1.35, P < 0.001), the population growth from 1991 to 1996 (beta = 0.02, P = 0.007), the percentage of children 7-14 years old that did not go to the school (beta = 0.02, P = 0.028), and the presence of a railroad in the municipality (beta = 0.45, P = 0.038) were found to be predictors of the incidence rate of leprosy in Ceará. CONCLUSION: Our findings fit the assumption that, in Ceará, leprosy is associated with a high level of poverty and uncontrolled urbanization. We put forward the hypothesis that urbanization increases not only social inequality eventually leading to strong polarization, but also excludes people from social and material opportunities. Apparently, such deprivations render them susceptible for leprosy.

[The resurgence of pertussis in developed countries: a problem for Brazil as well?].

Pertussis is currently considered an important public health problem in developed countries. In most of these countries, mass immunization for pertussis was initiated in the 1950s and was followed by a marked decrease in disease incidence. In the 1970s, pertussis was apparently under control in countries were vaccine coverage was maintained high. However, in the last two decades of the 20th century, the number of reported cases increased in all age groups, including adolescents and adults, indicating resurgence of the disease. This brief note aims to present the possible reasons for resurgence of this disease and to discuss the prospects of its future dynamics in Brazil. There has been no evidence to date for the resurgence of pertussis in Brazil. However, since mass immunization in Brazil began only in the 1980s, one cannot rule out the possibility that pertussis will resurge in the near future. Therefore, it is important that public health services closely monitor the epidemiological situation of pertussis in order, if necessary, to rapidly update the current immunization strategy.

[Portuguese-language cultural adaptation and translation of "The Bowel Disease Questionnaire" used to assess functional bowel disorders].

BACKGROUND: Functional bowel disorders are considered a public health problem, but there are a few prevalence studies in Brazil. In 1999, researchers from the Mayo Clinic developed The Bowel Disease Questionnaire with the purpose to evaluate functional bowel disorders. Although this has been used in different studies and population, it is often necessary to perform a cultural adaptation of a questionnaire developed for use in another culture, in order to improve the quality of the information obtained. OBJECTIVE: To translate and adapt the Bowel Disease Questionnaire to Portuguese for its use in adolescents. METHODS: Assessment of conceptual and item equivalence involved evaluation of the pertinence of the concepts in the Brazilian context; translation was evaluated through semantic equivalence between the original instrument and the Portuguese version; measurement equivalence between the original and the Portuguese version was assessed by the intra-observer reliability and internal consistency. RESULTS: The Bowel Disease Questionnaire's concepts were considered pertinent in the Brazilian context; the semantic equivalence between the original and the Portuguese version were high; reliability ranged from moderate to almost perfect; internal consistency of the scales ranged from 0,66 to 0,74. CONCLUSION: The Portuguese version of the Bowel Disease Questionnaire seems to be semantically and culturally equivalent to the original version and might be useful for measuring functional bowel disorders among Brazilian adolescents.

[Hepatitis A and B seroprevalence in Macaé, Rio de Janeiro State, Brazil].

Total HAV and HBc seroprevalence rates in two socioeconomic groups in Macaé, Rio de Janeiro State, Brazil, were estimated in 1,100 surplus serum samples from routine laboratory tests identified by sex, age, neighborhood, and category of medical care, i.e., the public health system or National Unified Health System (SUS) as compared to private health services (NSUS). Seroprevalence rates by age, 95% confidence intervals, and statistical significance tests for differences between SUS and NSUS are presented. Distribution of seroprevalence rates (P) for total HAV (P = 88.8%; 95% CI = 86.8-90.6) and total HBc (P = 15.3%; 13.2-17.6) by age showed an ascending curve. Prevalence rates in the SUS group were significantly higher that in the NSUS group, for both HAV (chi2 = 31.15; p < 0.0001) and HBV (chi2 = 15.41; p < 0.0001). The high prevalence rates reflect the epidemiological pattern of HAV infection in developing countries and the relevance of the social and environmental context. The proportion of susceptible individuals in the < 5 and > 20 year groups highlights the need to vaccinate for hepatitis A and the potential increase in severe cases. High HVB prevalence among adolescents underscores the importance of vaccinating this group. The results serve as a reminder to health professionals concerning biosafety norms.
