[Disseminated juvenile xanthogranuloma].

Juvenile xanthogranuloma (JXG) is the most common type of non-Langerhans cell histiocytosis occurring predominantly in infants and children. Typical lesions are asymptomatic red-yellow papules and nodules on the scalp or in the axillae or groins. Multiple lesions are more common in children than in adults. A 2-year old girl presented with approximately 20 red-brown lesions, leading to the diagnosis of disseminated JXG. Internal involvement was excluded. Spontaneous regression occurred over 6 months of follow-up. A wait-and-see strategy is recommended for cutaneous JXG.

Diagnostic telepathology: long-term experience of a single institution.

OBJECTIVES: The paper reviews the development of the application of telepathology in a department of surgical pathology between 1991 and 2003. The goal of the efforts during this time was to give up the concept of programming a single application, available only between two fixed workstations with sophisticated devices and special software, and to find the virtual "largest common denominator" for implementing as many different applications as possible with the same basic system. METHODS: A new telepathology system was designed as a client-server system with a relational database at its centre. The clients interact together by transferring the questions (texts and images) to a record (case) in the database on the server and by transferring the answers to the same record on the database. RESULTS: The new "open" telepathology system iPath (http://telepath.patho.unibas.ch) has been very well accepted by many groups around the world. The main application fields are: consultations between pathologists and medical institutions without a pathologist (e.g. for frozen section diagnoses or for surgical diagnoses in hospitals in South Asia or Africa), tumour boards, field studies and distance education (http://teleteach.patho.unibas.ch). CONCLUSIONS: Having observed that with iPath we have succeeded in satisfying all our telepathology needs, we are inclined to put the emphasis on the nature of the tasks being performed, as opposed to the methods or technical means for performing a given task. The three organisation models proposed by Weinstein et al. (2001) can be reduced to only two models: the model of discussion groups and the model of expert groups (virtual institutes).

FDG-PET in the pretherapeutic evaluation of primary squamous cell carcinoma of the oral cavity and the involvement of cervical lymph nodes.

PURPOSE: The diagnostic role of positron emission tomography (PET) with 2-deoxy-2-[18F]fluoro-D-glucose (FDG) in squamous cell carcinoma of the oral cavity is evaluated. PROCEDURES: In 38 patients, the results of FDG-PET, magnetic resonance imaging (MRI), computed tomography (CT), and ultrasound were compared. The standard uptake values (SUV) of FDG-PET were correlated to histopathological grading and DNA-image cytometry. RESULTS: In the case of lymph node metastases, the sensitivity of FDG-PET (93%) was higher than the sensitivity for the compared methods. The specificity was best for CT. SUVs of diploid tumor cell lines seemed to be lower than in non-diploid tumor cell lines. CONCLUSIONS: The high sensitivity and the high negative predictive value of PET may lead to more restrictive therapeutic regimens regarding lymph node metastases. Studies are necessary regarding possible relationships between glucose metabolism and tumor grading.

COX-2 expression in rectal carcinoma: immunohistochemical pattern and clinical outcome.

BACKGROUND: Increasing evidence suggests that COX-2 promotes carcinogenesis but it is controversial as to whether COX-2 expression is a prognostic factor for rectal carcinoma. MATERIALS AND METHODS: Data of 62 consecutive patients with rectal carcinoma in UICC-stages I-III, which were curatively resected between 1995 and 1996, were analyzed. The paraffin-embedded tumor samples were stained for COX-2 expression. A labeling index of stained cells was calculated and the median COX- 2 labeling index was used as the cut-offpoint for statistical analysis. RESULTS: The median labeling index was 0.58 (SD +/- 0.25). The Chi-square test revealed no correlation between COX-2 overexpression and the established prognostic factors. In the univariate analysis, COX-2 did not show significance according to the endpoints, local recurrence (p=0.41), disease-specific survival (p=0.28) or overall-survival (p=0.69). In contrast increased COX-2 level was a significant prognostic factor for pulmonary metastasis (p=0.04). DISCUSSION: COX-2 expression lacked significance as a prognostic factor for local control and survivaL However, there is evidence that COX-2 might be linked to an increased risk of hematogenous metastatic spread.
