Late onset of severe graft-versus-host disease following liver transplantation.

Graft versus host disease (GVHD) is an uncommon but lethal complication following liver transplantation that results from the engraftment of T lymphocytes associated with the liver graft. It usually occurs 2 to 6 weeks after the procedure. We herein report a case of late onset of severe GVHD 4 months after cadaveric liver transplantation for hepatocellular carcinoma in a 54-year-old woman, which was characterized by refractory diarrhea and abdominal pain. Moreover we discuss risk factors of GVHD including the recipient age and cytomegalovirus (CMV) infection.

Desulfurization of dibenzothiophene by Bacillus subtilis recombinants carrying dszABC and dszD genes.

The desulfurization (dsz) genes from Rhodococcus erythropolis DS-3 were successfully integrated into the chromosomes of Bacillus subtilis ATCC 21332 and UV1 using an integration vector pDGSDN, yielding two recombinant strains, B. subtilis M29 and M28 in which the integrated dsz genes were expressed efficiently under the promoter, Pspac. The dibenzothiophene (DBT) desulfurization efficiency of M29 was 16.2 mg DBT l(-1) h(-1) at 36 h, significantly higher than that of R. erythropolis DS-3 and B. subtilis M28 and also showed no product inhibition. The interfacial tension of the supernatant fermented by M29 varied from 48 mN m(-1) to 4.2 mN m(-1), lower than that of the recombinant strain, M28, reveals that the biosurfactant secreted from M29 may have an important function in the DBT desulfurization process.

Switchable electro-optic diffractive lens with high efficiency for ophthalmic applications.

Presbyopia is an age-related loss of accommodation of the human eye that manifests itself as inability to shift focus from distant to near objects. Assuming no refractive error, presbyopes have clear vision of distant objects; they require reading glasses for viewing near objects. Area-divided bifocal lenses are one example of a treatment for this problem. However, the field of view is limited in such eyeglasses, requiring the user to gaze down to accomplish near-vision tasks and in some cases causing dizziness and discomfort. Here, we report on previously undescribed switchable, flat, liquid-crystal diffractive lenses that can adaptively change their focusing power. The operation of these spectacle lenses is based on electrical control of the refractive index of a 5-mum-thick layer of nematic liquid crystal using a circular array of photolithographically defined transparent electrodes. It operates with high transmission, low voltage (<2 Vrms), fast response (<1 sec), diffraction efficiency > 90%, small aberrations, and a power-failure-safe configuration. These results represent significant advance in state-of-the-art liquid-crystal diffractive lenses for vision care and other applications. They have the potential of revolutionizing the field of presbyopia correction when combined with automatic adjustable focusing power.

Cytotoxic cembranoid diterpenes from a soft coral Sinularia gibberosa.

Five new alpha-methylene-gamma-lactone-bearing cembranoid diterpenes, sinularolides A-E (1-5), along with 10 known cembranoids, were isolated from the soft coral Sinularia gibberosa. Their structures were determined by extensive spectroscopic (IR, MS, 2D NMR) data analysis. Compounds 2-5 showed moderate activity against selected tumor cell lines.

Steroids from the soft coral Dendronephthya sp.

Fifteen steroids were isolated from the soft coral Dendronephthya sp., of which five are determined as new compounds, namely (22E)-3-O-beta-formylcholest-5,22-diene (1), (22E)-3-O-beta-formyl-24-methyl-cholest-5,22-diene (2), 2-ethoxycarbonyl-2-beta-hydroxy-A-nor-cholest-5-ene-4-one (3), (22E)-2-ethoxycarbonyl-2-beta-hydroxy-A-nor-cholest-5,22-diene-4-one (4), and (22E)-2-ethoxycarbonyl-2-beta-hydroxy-24-mthyl-A-nor-cholest-5,22-diene-4- one (5). 1 and 2 belonged to 3-O-formylated cholesterol analogues, and 3 to 5 are unique ring A-contracted steroids. Their structures were elucidated by extensive 2D NMR in association with IR, MS analysis.

[A clinical report of 12 case-times of living related liver transplantation].

OBJECTIVE: To evaluate and summarize the clinical application of living related liver transplantation (LRLT). METHODS: A retrospective analysis was made in altogether 12 LRLT operations of living related liver transplantation performed in our department. The indication and timing, surgical complications, and nonsurgical issues including infection, rejection, advantages of LRLT in our series were reviewed. RESULT: All the 11 donors are uneventfully after operation; the first receptor was dead, and 10 receptors with Wilson's disease achieve long-term survival. The postoperative survival time till now is 58 w, 51 w, 48 w, 38 w, 37 w, 36 w, 22 w, 18 w, 10 w and 7 w, respectively. No rejection was detected in the recipients with Wilson's disease, whose liver function and cuprum oxidase level had returned to normal. In this 10 cases, 6 of them has returned or gone to school, 2 of them were discharged from hospital, 2 of them has been recovered smoothly in the hospital due to short time postoperatively. The primary complications after operation including blood vessel, biliary tract, lung and the infection of microbe or virus, were mainly involved in our series. CONCLUSION: The process of operation was complicated; the operation technology was exigent and difficult with respect to the safety of the donors and receptors. LRLT has capacious clinical application for there are many unsurpassable advantages, such as plentiful resource and fine quality of graft liver, lower cost of LRLT.

Three-dimensional optoelectronic stacked processor by use of free-space optical interconnection and three-dimensional VLSI chip stacks.

We present a demonstration system under the three-dimensional (3D) optoelectronic stacked processor consortium. The processor combines the advantages of optics in global, high-density, high-speed parallel interconnections with the density and computational power of 3D chip stacks. In particular, a compact and scalable optoelectronic switching system with a high bandwidth is designed. The system consists of three silicon chip stacks, each integrated with a single vertical-cavity-surface-emitting-laser-metal-semiconductor-metal detector array and an optical interconnection module. Any input signal at one end stack can be switched through the central crossbar stack to any output channel on the opposite end stack. The crossbar bandwidth is designed to be 256 Gb/s. For the free-space optical interconnection, a novel folded hybrid micro-macro optical system with a concave reflection mirror has been designed. The optics module can provide a high resolution, a large field of view, a high link efficiency, and low optical cross talk. It is also symmetric and modular. Off-the-shelf macro-optical components are used. The concave reflection mirror can significantly improve the image quality and tolerate a large misalignment of the optical components, and it can also compensate for the lateral shift of the chip stacks. Scaling of the macrolens can be used to adjust the interconnection length between the chip stacks or make the system more compact. The components are easy to align, and only passive alignment is required. Optics and electronics are separated until the final assembly step, and the optomechanic module can be removed and replaced. By use of 3D chip stacks, commercially available optical components, and simple passive packaging techniques, it is possible to achieve a high-performance optoelectronic switching system.

[Application of microsurgical technique to hepatic artery reconstruction in liver transplantation].

OBJECTIVE: To explore the value of the microsurgical technique in the reconstruction of hepatic artery. METHODS: From September 2000 to June 2001, we performed liver transplantation for 11 patients including living related liver transplantation (4) and 7 orthotopic liver transplantation (7). Arterial reconstruction was performed under an operating microscope. RESULTS: No patients developed hepatic arterial thrombosis and serious complication, nor death for multiple organ failure. CONCLUSION: Microsurgical technique in reconstruction of the hepatic artery can improve surgical outcome, not only in orthotopic liver transplantation but also in living related liver transplantation.

[Multimodal approach to clinical liver transplantation].

OBJECTIVE: To sum up the clinical experience of liver transplantation. METHOD: A retrospective study was made in 11 patients receiving living donor liver transplantation (LDLT)/and 14 patients having orthotopic liver transplantation (OLT), including one time operation of reduced size liver retransplantation and one time operation of cadaveric liver retransplantation. RESULTS: The voluntary donors were a sister and 10 mothers of recipients. The location of graft included 3 patients of segment II, III, part of IV (not including intermediate hepatic veins), 6 patients of segment II, III, IV (including intermediate hepatic veins), and 2 patients of V, VI, VII, VIII (not including intermediate hepatic veins). The weight range of graft was 270 - 620 g. Twenty-four recipients achieved a long-term survival and retained normal liver function during the follow-up. Only 1 patient died from serious rejection on the 72nd day postoperatively. Ten patients with hepatitis B cirrhosis were treated with lamivudine and anti-HBVIg, and HBV-DNA in serum was negative during the follow-up for 4 approximately 21 months. Copperoxidase, ceruloplasmin and main indexes of liver function became normal in all patients with Wilson's Disease. Postoperative complications included abdominal hemorrhage (2 patients), acute respiratory distress syndrome (5), acute rejection (4), and acute renal function failure (2). CONCLUSIONS: The wise solution to improve the result of liver transplantation and optimize liver resources is the "multimodal approach", by which all kinds of techniques for liver transplantation including CLT, LDLT and RSLT should well developed.
