Breast reconstruction using the inferior gluteus free flap.

Microsurgical tissue transplantation initiated a new dimension in breast reconstruction and is the preferred method for carefully selected patients. The inferior gluteus maximus musculocutaneous flap provides abundant tissue for a permanently soft and well-contoured breast with a donor site that is often enhanced by tissue sculpting. As a knowledgeable and skilled member of a multi-disciplinary team, the professional nurse is in a position strategic to the success of breast reconstruction utilizing microsurgical tissue transfer.

Epidural analgesia in the postoperative patient: a nursing perspective.

The epidural administration of narcotics and local anesthetics is an effective method of postoperative pain management. An improved quality of pain control with less sedation promotes better pulmonary function, early ambulation, and probably fewer postoperative complications. Nurses caring for these patients must have a strong knowledge base and specialized skills to deliver safe and effective nursing care.

Nipple and areola reconstruction.

Nipple-areola reconstruction is an integral part of breast reconstruction. Optimum results are usually obtained when nipple-areola reconstruction is staged after the breast mound has attained its final shape and is well vascularized. The use of intradermal tattoo allows the use of a variety of nonpigmented donor sites. Women report that reconstruction of the nipple-areola enhances their overall satisfaction with breast reconstruction. The knowledgeable and skilled nurse is a valuable member of the professional team during this final phase of breast reconstruction.
