Effects of nandrolone decanoate or testosterone decanoate on murine lupus: further evidence for a dissociation of autoimmunosuppressive and endocrine effects.

Nandrolone decanoate injections (20, 40 or 80 mg/kg/3 weeks) in female or castrated male New Zealand black/white mice, starting at various ages (4, 9, 17 or 26 weeks) prolong survival, reduce proteinuria and affect the weights of various endocrine and non-endocrine organs. Nandrolone decanoate is at least equally potent as testosterone decanoate with respect to its beneficial effects on lupus-associated symptoms; in contrast, its effects on some other androgen-sensitive endocrine parameters are significantly less. These observations show that the autoimmunosuppressive effects of steroids are not quantitatively correlated to endocrine properties.

Effects of sex, gonadectomy and several steroids on the development of insulin-dependent diabetes mellitus in the BB rat.

The spontaneous development of the putative autoimmune insulin-dependent diabetes mellitus (IDDM) in BB rats was not affected by the following factors: (a) sex, (b) gonadectomy, (c) administration of testosterone decanoate, 11 alpha-hydroxynandrolone decanoate, ethinyloestradiol or Org OD14 (tibolone). Treatment with nandrolone decanoate lead to a statistically significant decrease in the incidence of diabetes in male rats in one experiment, but failed to do so in the second. These results and various data from the literature suggest that the influence of sex and steroids on the development of diabetes mellitus in the BB rat is different to that of autoimmune disorders in other animal models (New Zealand Black/White, New Zealand Black mice, Obese Strain chickens) and in man.

Age Factors↗

Therapeutic effects of nandrolone decanoate, tibolone, lynestrenol and ethylestrenol on Sjögren's syndrome-like disorder in NZB/W mice.

Growth of mononuclear cell infiltration in submandibular glands is significantly inhibited by Org OD14 (tibolone), lynestrenol and ethylestrenol given orally to New Zealand Black/White (NZB/W) mice from 26 weeks of age onwards. In addition, the extent of already established mononuclear cell infiltrations is significantly inhibited and reduced by nandrolone decanoate injected from 43 weeks of age onwards. Tibolone and nandrolone decanoate are the most potent of the four drugs. The therapeutic effect of these four steroids on the Sjögren's syndrome-like disorder in NZB/W mice is not related to their endocrine activities.

Animals↗

Effects of nandrolone decanoate in NZB/W mice treated concomitantly with maintenance doses of dexamethasone sodium phosphate.

Treatment of female or castrated male NZB/W mice with nandrolone decanoate (80 mg/kg s.c.), once every three weeks, delays the onset of murine lupus and associated symptoms, even when treatment was started at the 17th week of age. Continuous low dose glucocorticoid therapy did not affect the therapeutic effects of the nandrolone decanoate injections. These results indicate that SLE patients receiving maintenance low dose glucocorticoid therapy may also benefit from treatment with nandrolone-decanoate.

Experimental work with anabolics in autoimmunity models.

19-nortestosterone (nandrolone) or its decanoate was tested in six animal models for autoimmune disease. The compound was found capable of inhibiting the development of disease symptoms or the deterioration of certain immune parameters in three out of five models of spontaneous autoimmune disease: NZB/NZW F1 (NZB/W) mice (lupus glomerulonephritis, Sjögren's disease-like syndrome), NZB (lupus glomerulonephritis, autoimmune haemolytic anaemia) and OS chickens (thyroiditis). No effect of the compound was seen in MRL lpr/lpr mice (lupus glomerulonephritis, lymphoproliferative disease) and in BB rats (insulin-dependent diabetes mellitus; IDDM). Nandrolone decanoate also seems to be active in myasthenia gravis evoked in rats immunised with acetylcholine receptor but further experiments are necessary to confirm this. In a first experiment also ethylestrenol was found to give favourable effects in the model.

Effect of nandrolone decanoate on Sjögren's syndrome like disorders in NZB/NZW mice.

The effects of 2.5-40 mg/kg nandrolone decanoate (ND) on the development and growth of mononuclear cell infiltrations in the submandibular glands of NZB/W (B/W) mice have been studied using a quantitative histological method. Injections with ND for 9 months, once every 3 weeks, starting at 4 weeks of age, reduced the number and total area of infiltrations in submandibular glands of female and castrated male B/W mice. Since ND has relatively weak virilizing properties, this substance may be useful for the treatment of Sjögren's syndrome in humans.

The effects of nandrolone, testosterone and their decanoate esters on murine lupus.

Treatment of NZB/NZW F1 (B/W) female and castrated male mice with testosterone or 19-nortestosterone (nandrolone), either by implantation in silastic tubing or by subcutaneous injections of their decanoate esters, reduced in a dose-dependent manner symptoms associated with murine lupus (proteinuria, IgG antibodies to DNA) and prolonged survival. These phenomena were observed under both prophylactic (start at 3-4 weeks) and therapeutic treatments (start 27-29 weeks). Nandrolone and its decanoate ester were at least as potent as testosterone and testosterone decanoate. As the unwanted androgenic properties of nandrolone and its ester are significantly less pronounced than those of testosterone and its ester, also in these NZB/NZW mice, the beneficial effect on murine lupus does not seem to be associated with these properties.

Long-term reproducibility of conventional Doppler analysis in patients with prosthetic valves.

Long-term reproducibility of Doppler recordings made by the same investigator using the same ultrasound equipment was determined in 50 clinically stable patients. The mean interval between the first and second examination was 16 +/- 7 months. In 90% of the 33 patients with aortic prostheses, the relative difference between the first and second examination was less than 16% (mean value 9.1%) for the maximum instantaneous gradient and less than 17% (mean value 7.4%) for the mean gradient; the relative difference was less than 20% (mean value 8.5%) for the maximum flow velocity in the left ventricular outflow tract and less than 24% (mean value 10.8) for the maximum flow velocity ratio. In 90% of the 25 patients with mitral prostheses, the absolute difference between the first and second examination was less than 3 mmHg for the maximum instantaneous gradient, less than 2.5 mmHg for the mean gradient, and less than 20 msec for the pressure half-time. We conclude that long-term reproducibility of Doppler echocardiographic characteristics of prosthetic valve function is good as far as transprosthetic gradients or pressure half-time are concerned but is less so for maximum flow velocity in the left ventricular outflow tract and the maximum flow velocity ratio. Changes beyond the aforementioned values may represent a real change in prosthetic valve function.

The value of Doppler echocardiography in the management of patients with valvular heart disease: analysis of one year of clinical practice.

Management recommendations based on Doppler echocardiographic examination and cardiac catheterization were compared in a prospective study in 100 consecutive patients who were admitted for evaluation and treatment of suspected valvular heart disease during 1988. Management recommendations were provided independently after both Doppler echocardiography and cardiac catheterization by different and blinded investigators. Criteria for severe (clinically significant) and moderate to mild (insignificant) valvular lesions and management recommendations were agreed on in advance. There was disagreement on the severity of aortic stenosis based on the aortic valve area and maximum instantaneous pressure gradient in 1 of 54 patients, which resulted in differing management recommendations. Mitral stenosis was severe (valve area less than or equal to 1 cm2) at Doppler echocardiography but not at cardiac catheterization in 5 of 14 patients. Because pulmonary artery pressure increase during exercise at cardiac catheterization also suggested severe obstruction, management recommendations were similar. There was a potentially significant disagreement on the severity of aortic regurgitation in 9 of 76 patients and of mitral regurgitation in 14 of 90 patients; however, this did not produce differing management recommendations because with most patients coexistent valvular lesions or an impaired ventricular function mainly determined the ultimate management decision. Although of good quality, Doppler echocardiographic examination was nonconclusive for clinical decision-making in 15% of the study population because of uncertainty about the severity of mitral regurgitation or aortic regurgitation or because of problems in assessing the degree of left ventricular dysfunction in patients with severe regurgitation.(ABSTRACT TRUNCATED AT 250 WORDS)
