Periodontal changes following coronal/root perforation and formocresol pulpotomy.

A clinical case report is presented which describes the sequelae of an iatrogenic lateral crown/root perforation and a formocresol pulpotomy. The post-operative course of the initial therapy included significant hard and soft tissue destruction and eventual tooth loss. The possible implication of the role of formocresol is discussed.

Clinical and microbiological effects of a sanguinaria-containing mouthrinse and dentifrice with and without fluoride during 6 months of use.

The purpose of this study was to test the efficacy and safety of sanguinaria-containing regimens with and without fluoride using the American Dental Association guidelines for evaluating chemotherapeutic agents. The study was a 6-month, double-blind, 4-cell, placebo-controlled, parallel investigation involving 120 subjects. Following screening procedures, subjects were randomly assigned to 4 groups. Group 1 received a dentifrice containing 0.075% sanguinaria extract (SaE) and 2.0% zinc chloride (ZnCl2) in a dicalcium phosphate base, plus an oral rinse containing 0.03% SaE and 0.2% ZnCl2. Group 2 received identical products without SaE or ZnCl2. Group 3 received a dentifrice containing 0.8% sodium monofluorophosphate, 0.075% SaE, and 0.05% ZnCl2 in a silica base, plus an oral rinse containing 0.03% SaE and 0.2% ZnCl2. Group 4 products were identical to those of Group 3 but without SaE and ZnCl2. Supragingival plaque and gingival inflammation were scored at 0, 1, 2, 1.5, 3, 4.5, and 6 months; bleeding upon probing was measured at 1, 1.5, 3, and 6 months. Microbiological samples were taken from plaque, tongue, and cheek areas. The active products produced statistically significantly lower scores than the placebo agents for all indices (P less than .0001). Six-month plaque scores were 13.1% lower for Group 1 and 17.4% lower for Group 3 compared to placebo products. When the Plaque Severity Index was applied, the percentage reductions were 33% for Group 1 and 41% for Group 3 compared to placebos. Gingival inflammation scores were 16.7% lower for Group 1 and 18.1% lower for Group 3 at 6 months compared to placebo scores.(ABSTRACT TRUNCATED AT 250 WORDS)

Human immunodeficiency virus integration in a cell-free system.

Integration of the viral genome into the nuclear DNA of a host cell plays a pivotal role in the replication of retroviruses. We have developed an in vitro method for studying the biochemistry of human immunodeficiency virus (HIV) integration by using extracts from HIV-infected cells. Analysis of the reaction products showed that HIV integration in vitro accurately reproduces the in vivo process. Integration occurred without apparent specificity for the target sequence, and the integrated provirus was directly flanked by a 5-base-pair duplication of DNA from the target site. HIV integration did not require a high-energy cofactor, and the enzymatic activities required for integration were recovered with the viral DNA when cell extracts were fractionated by gel exclusion chromatography.

Effect of a hydraulic jet prophylaxis system on composites.

Because air-powered devices that remove surface stain from teeth are commercially available, recent literature concerning the effect of the hydraulic jet prophylaxis system on oral tissues and restorative materials was reviewed. A study designed to compare the volume loss by different types of composites abraded by this system revealed a relatively high rate of abrasion for the small-particle composite compared with composites containing larger filler particles.

The relationship of inflammatory periodontal disease to diabetic status in insulin-dependent diabetes mellitus patients.

This study was designed to evaluate the relationship of inflammatory periodontal disease to the diabetic status of the insulin-dependent diabetes mellitus (IDDM) patient. 52 IDDM patients, ages 11-22 years, were evaluated. These patients were closely monitored at regular intervals in the University of Kentucky pediatric diabetic clinic. A periodontal examination was carried out for each patient. The patients were then assigned to a periodontitis or non-periodontitis group. Moderate to advanced periodontitis was found in 5.8% of the subjects. The gingival index and sulcular bleeding index were significantly higher in the periodontitis group (P less than 0.05). There was no significant difference between groups for plaque index, age of diabetic onset, duration of diabetes, present age, insulin dosage/weight, or serum glucose (P greater than 0.05). There was a greater % of ketoacidosis, retinopathy and neuropathy in the periodontitis group. IDDM patients with neurological complications or a history of chronic infections had a significantly higher gingival index score than those without the complication (P less than 0.05).

Incidence of anterior ridge deformities in partially edentulous patients.

A random sample of 416 diagnostic casts was evaluated to determine the incidence and type of ridge deformities present in anterior ridges of partially edentulous patients. Loss of teeth resulted in 91% (31 of 34) of ridge deformities. The most prevalent defect was the class III or combination labiolingual and apicocoronal type of deformity.

A comparison of postoperative pain experience following periodontal surgery using two local anesthetic agents.

This controlled, double-blind, split-mouth study was designed to evaluate postoperative pain experience following periodontal surgery on 20 patients. Two commercially available local anesthetic agents, bupivacaine HCl and lidocaine HCl, were used. Periodontal surgeries were standardized to minimize differences in difficulty, extent and time. A patient questionnaire was used to collect data for the 24-hour observation period following periodontal surgery. During this period, pain perception was assessed by visual analogue scales. The results indicated that when bupivacaine was used, there was less postoperative pain, fewer postoperative analgesics taken and a longer period of "numbness" (anesthesia) as compared to lidocaine. The patients expressed a strong preference for bupivacaine over lidocaine.

Technique for an immediate temporary fixed tooth replacement.

A clinical technique for the immediate replacement of an extracted anterior tooth was presented. The clinical crown of the patient's extracted tooth was used as a pontic by attaching it to the adjacent teeth with acid-etch bonded resin. This technique provides a practical and esthetic immediate fixed tooth replacement that requires minimum materials and time.

Aphthous ulcer--a treatment complication. Report of a case.

Multiple theories of the cause and treatment of aphthous ulcers are described in the medical and dental literature. A case is reported in which there were posttreatment sequelae from the application of a sillver-nitrate stick (pencil) to an aphthous ulcer on the tongue. Compications subsequent to the use of silver nitrate may contraindicate the use of this agent in its highly concentrated form on the mucous membranes of the mouth.
