A spark counter as a control unit of a radio frequency surgery device.

A new approach in radio frequency (rf) electrosurgery, used for tissue treatment, is achieved by using a new process control method. An external control unit allows a commonly available rf-generator to automatically supply the appropriate power for differing tissue types, thus ensuring best cutting quality. The sparks, generated during the scalpel electrode interaction with the tissue, appear statistically distributed. The spark rate depends on various factors and is monotonic with the supplied electrical power. This allows it to be used as the controlled variable in the cutting process. The ac current passing through the tissue is evaluated by an external control unit using an analyzation algorithm to determine the number of sparks. The external unit is comprised of a system, which measures the spark rate, and a subsequent proportional integral (P.I.) controller. The functionality of the control method as well as the electrical circuitry is verified through cuts with different degrees of carbonization and cuts through tissue heterojunctions.

Molecular analysis of the immunoglobulin VH gene rearrangement in a primary cutaneous immunoblastic B-cell lymphoma by micromanipulation and single-cell PCR.

The immunoglobulin VH gene rearrangement in a primary cutaneous, large-cell (centroblastic and immunoblastic) B-cell lymphoma was analyzed using a micromanipulation/single-cell polymerase chain reaction technique. In all single B cells obtained from CD20-stained skin sections that gave a polymerase chain reaction product (eight of 27 in biopsy I), the same VHDJH rearrangement, consisting of DP-54-DIR1-JH3a genes, was detected, with no intraclonal nucleotide diversity. Comparison with the most closely related germline counterpart showed significantly altered complementarity determining gene regions as a result of somatic mutations, suggesting an antigen-driven selection and expansion ofthis particular B-cell clone. Interestingly, in a biopsy obtained from the patient 9 mo later, during disease progression (deep muscle infiltration), the lymphoma cells again contained the same VHDJH gene rearrangement (six of 18 in biopsy II) without any further somatic mutations. Therefore, it is suggested that the cutaneous lymphoma characterized throughout this study descended from postgerminal center B-cells.

Search for Kaposi's sarcoma-associated virus DNA in hemangioproliferative disorders and cutaneous malignant lymphoma.

Recently, a Kaposi's sarcoma-associated herpesvirus (KSHV) was discovered. We evaluated by PCR 14 paraffin-embedded specimens with the histological diagnosis of endemic, classic and HIV-associated Kaposi's sarcoma (KS) for the presence of the KSHV DNA sequence. In addition, biopsies of adjacent, histologically unaffected skin, peripheral-blood mononuclear cells (PBMCs) of HIV-infected KS patients, PBMCs of one classic KS patient, and specimens of patients with hemangioproliferative disorders other than KS as well as samples of cutaneous T- and B-cell lymphoma were analyzed for KSHV. In all cases of KS, independent of the KS subtype, KSHV was detected in lesional skin. No KSHV was found in biopsies of the adjacent unaffected skin or PBMCs of HIV-infected KS patients. We found KSHV in the PBMCs of a patient with classical KS. All specimens of cutaneous T- and B-cell lymphomas or lymphomatoid papulosis were negative for KSHV. In addition, the samples with hemangioproliferative disorders other than KS were negative for KSHV. There was one borderline case of KS or acroangiodermatitis that was positive for KSHV. Additional histological sections and clinical evaluation confirmed the diagnosis of classic KS. In summary, the data indicate that PCR for KSHV should be a useful diagnostic tool in cases of hemangioproliferative disorders.

[Scleredema diabeticorum. Report of 4 cases].

We report on four patients who developed thickened skin on the neck and back after a long history of insulin-dependent diabetes mellitus. Pathogenesis, histology and methods of treatment are discussed.

Comparison of the effects of calcipotriol, prednicarbate and clobetasol 17-propionate on normal skin assessed by ultrasound measurement of skin thickness.

In this study, we investigated the effect of calcipotriol, prednicarbate and clobetasol 17-propionate on skin thickness over a treatment period of 6 weeks. The study was conducted as a controlled, randomized, double-blind comparison. The influence of these drugs on normal skin under occlusive conditions was assessed visually and by measuring skin thickness using 20 MHz B mode ultrasound. Both topically applied glucocorticosteroids lead to a significant decrease in skin thickness. In contrast to the glucocorticosteroid-induced atrophy, calcipotriol application on normal skin leads to an increase in skin thickness in all volunteers. The effect remains constant for the duration of treatment. The cause of this increase seems to be an irritative reaction of the skin which was histologically investigated in one volunteer. The histological features of this reaction are characteristic for a subacute dermatitis. The implications of these findings for the therapeutic mechanism of calcipotriol are discussed.

Regional lymph node morphology in malignant melanomas and their prognostic value.

The occurrence of granulomatous epithelioid cell reaction, hyperplasia of lymphatic paracortex and existence of histiocytosis of the intermediary-medullary sinuses observed on sections of lymph nodes of 582 patients suffering from high-risk melanomas, examined during a ten-year period, have been established as reliable indicators of survival. On the other hand, lymphocytic infiltrative processes of the lymph node capsule, medullar plasmocytosis, and hyperplasia of germinal centres have been seen as indicators of a bad prognosis. Lymphocytic paracortical depletion and a predominance of postcapillary venules within the T zone were found to be less important in patients with a low chance of survival. We were able to establish a connection between lymphonodular parameters and the survival rate of the patients. In a multivariate test we found that there were four factors for statistical ascertainment of the prognosis of melanoma patients with lymphadenectomy: mitotic rate of primary tumour, lymphocytic infiltration of the lymph node capsule, existence of paracortical hyperplasia, and sex of the patient.

[UV-A-1 therapy of subacute cutaneous lupus erythematosus].

Because of relative contraindications to corticosteroid or immunosuppressive treatment, a 71-year-old female patient with subacute cutaneous lupus erythematosus underwent serial whole-body irradiation with extremely long-wave ultraviolet light (UVA-1). The cumulative dosage was 186.1 J/cm2 within 9 weeks. Impressive improvement was achieved with some delay.

[Imaging subcutaneous atrophy in circumscribed scleroderma with 20 MHz B-scan ultrasound].

The technique of 20-MHz B-scan sonography is non-invasive and allows quantification of the different compartments of the skin. Seven patients with atrophic linear circumscribed scleroderma and one patient with guttate circumscribed scleroderma were examined by ultrasound. All patients were found to have a total or subtotal loss of subcutaneous fatty tissue, whereas the thickness of the dermis remained almost unchanged. These findings should lead to further investigation of the pathophysiological mechanism involved in subcutaneous atrophy in circumscribed scleroderma.

Laser surgery in superficial penile tumours.

The first results of laser surgery in superficial penile tumours are reported. In 2 patients with condylomata acuminata, 2 cases of giant condylomata Buschke-Löwenstein and in 2 patients suffering from early stage penile cancer diagnostic and/or therapeutic treatments using carbon dioxide or neodymium-YAG laser have been performed. In one patient with giant condyloma a recurrent tumour was found 5 weeks after the initial treatment and a second one using CO2 laser was done. All other patients are tumour-free with a follow up of 6-24 months and erectile function is normal. Therefore, in superficial penile tumours laser treatment is an excellent alternative therapeutic approach.

[Histologic changes after laser treatment of superficial penis tumors].

In the last years the excision of superficial benign and malignant penile tumors by laser surgery has been well established. Our first experience confirm the excellent cosmetic and functional results after laser surgery. Furthermore a decrease of recurrence rate was found due to laser vaporization of the adjacent tumor area. In spite of tissue damage by histological examination we were able to classify benign and malignant findings.

[Leukoplakia-like epidermal nevus of the mouth mucosa. Case report].

We report on a nevus of the oral mucosa, which became present in the age of 30 of a male patient. By microscopic examination we did not find any vacuolation of the spinous layer of the epithelium. So this nevus has been considered quite different from the typical white sponge nevus, and we prefer to name it leucoplakial nevus of the oral mucosa.

[Transitory acantholytic dermatosis (Grover) in an HIV infected patient].

We report on a patient with HIV infection and persistent generalized lymphadenopathy (stage III of HIV infection), who developed transitory acantholytic dermatosis (Grover's disease). Both the clinical picture and the histological features were typical for Grover's disease; the skin eruptions subsided within 6 months.

[Erythema elevatum diutinum--a rare variant of skin changes in systemic lupus erythematosus].

It is reported on 5 patients with erythema elevatum diutinum, in 2 of these cases exists a systemic lupus erythematosus and another patient suffers from cutaneous lupus erythematosus. This information can be considered as a possibility that cutaneous alterations similar to erythema elevatum diutinum could be the first symptoms of systemic lupus erythematosus when the usual treatment is inefficient.

[Disseminated superficial actinic porokeratosis. A case report].

It is reported on a 72-year-old pensionary who developed one year ago a disseminated superficial actinic porokeratosis on the lower legs after chronic sun exposure during twenty summers. The diagnostic features and therapeutic schedules are summarized and the role as praecancerosis is emphasized.

[Multicenter reticulohistiocytosis. A contribution to clinical aspects and therapy].

It is reported on a 48-year-old woman suffering from a multicentric reticulohistiocytosis. Most trouble in this case is caused by severe arthritis, which could not be stopped by various therapy schedules. The actual treatment is the combination of Prednisolone, Chloroquine, Ibuprofen and roentgenotherapy of the hands.
