The role of the silastic prosthesis in the management of gastroschisis.

We reviewed data on 43 patients with gastroschisis treated at The Hospital for Sick Children, London, between 1959 and 1975. In the past five years, the overall mortality has been reduced from 76% to 33%. The introduction of the Silastic silo has been the most important single factor in this improvement. Mortality may be further reduced by achieving all-layer closure following thorough bowel decompression and stretching of the abdominal wall.

Ileal conduit urinary diversion in children.

In a review of 113 paediatric patients followed for an average of 5 years, the ileal conduit has been very effective in preventing infection and loss of renal function. The incidence of early and late complications can be minimised by proper patient selection, attention to surgical details and diligent postoperative follow-up.

Metastatic orchioblastoma: alpha1-fetoprotein in diagnosis and combination chemotherapy in treatment: a case report.

A case in which orchioblastoma metastasised to a lymph node at the left renal hilum is described. The metastasis was not detected by bipedal lymphography, but was associated with a rise in the serum level of the alpha1-fetoprotein. The metastasis responded to combination chemotherapy, including adriamycin, and residual tumour was excised at laparotomy. Tumour regression was associated with a fall in serum alpha1-fetoprotein which may be of value as a tumour marker substance in the diagnosis and monitoring of therapy in this tumour.

Simultaneous bilateral pyeloplasties.

Eleven patients with bilateral pelviureteric junction obstruction are presented. The operative technique for simultaneous bilateral pyeloplasty is briefly described and the results discussed. The importance of adequate extraperitoneal loin drainage is stressed and the incidence of subsequent adhesion obstruction is recorded.

Ureteral metastasis from Wilms tumor.

Urothelial spread of Wilms tumor is uncommon and only 5 instances of ureteral metastases have been reported previously. A case of Wilms tumor with mid ureteral metastasis is presented and the literature is reviewed. Clinical implications and suggestions for surgical management are discussed.

Simultaneous bilateral pyeloplasties.

A technique is described which gives excellent results in the treatment of unilateral hydronephorsis and allows for simultaneous bilateral pyeloplasties. The advantages for the patients with bilateral hydronephrosis are a single admission to hospital and a single anaesthetic and operative procedure. The exposure is superior to that achieved by a loin incision, allowing the surgeon to perform the pyeloplasties more rapidly and with less traction on the renal vessels. Over a follow-up period of 1-7 years no subsequent pyeloplasty or nephrectomy has been required and all 11 patients are well and symptom free.

Endovesical electrotherapy in treatment of urinary incontinence in spina-bifida patients.

A total of 20 children with incontinence due to myelomeningocele and 1 due to occult neuropathic bladder underwent a course of endovesical electrotherapy. There was temporary improvement in 1 and more lasting in another, but this was the result of catheterisation and bladder training rather than the electrotherapy. It is concluded that the electrotherapy has helped none of the patients, and no significant change in their bladder function was detected.

[An assessment of the functional results of various forms of surgery for epispadias and exstrophy (author's transl)].

Urinary continence can be achieved by total reconstruction in perhaps 20 percent of patients born with exstrophy of the bladder. Uretero-rectal anastomosis offers a reasonalbe alternative but carries a high morbidity. Cutaneous diversion is the safest form of long-term management, but this involves the life-long penalty of a stoma and the need for appliances. Conservative reconstructive surgery is advocated in the first instance, but repeated surgical procedures may be necessary for a successful result.
