Emerging invasive liver abscess caused by K1 serotype Klebsiella pneumoniae in Korea.

OBJECTIVES: The high incidence of invasive liver abscess caused by Klebsiella pneumoniae in Taiwan, contrasted with the rareness of this disease in Western countries, has aroused special interest. There have been few detailed reports from other Asian countries. To investigate a current epidemiology of K. pneumoniae liver abscess in Korea and to determine K serotype distribution in K. pneumoniae strains causing liver abscess, we performed a nationwide prospective study. METHODS: Community-acquired, culture-proven liver abscess cases were enrolled between 2004 and 2005. Etiologies and clinical features were analyzed. K. pneumoniae isolates were serotyped according to K antigen. Meta-analysis was done to determine the time trend of the etiologies of liver abscess in Korea. RESULTS: Out of 371 cases collected prospectively, 290 (78.2%) were caused by K. pneumoniae. Most K. pneumoniae liver abscesses were monomicrobial. Diabetes mellitus was the most common underlying disease (39.9%). Distant metastatic infections were frequently observed (8.7%). magA PCR revealed that 95 (59.4%) out of 160 K. pneumoniae isolates belonged to the K1 serotype. CONCLUSIONS: Our study indicates that K. pneumoniae has emerged as a major etiologic agent of liver abscess in Korea, and these emerging infections seem to be attributable to invasive K. pneumoniae strains with capsular K1 serotype.

Pulmonary tuberculosis in young Korean soldiers: incidence, drug resistance and treatment outcomes.

OBJECTIVE: To determine the incidence and treatment outcomes of pulmonary tuberculosis (PTB) in young soldiers of South Korea. DESIGN: From 2000 to 2004, all soldiers with a new diagnosis of tuberculosis (TB) were enrolled in the study, based on the official records of the Armed Forces Medical Command. The demographic and clinical data of the cases were evaluated retrospectively. RESULTS: A total of 3115 TB cases were reported during the study period, of whom 2071 (66.5%) were reported as PTB. The annual incidence rates of PTB were 96.4 per 100,000 population in 2000, 89.3 in 2001, 67.6 in 2002, 60.2 in 2003, and 63.1 in 2004. A total of 270 patients diagnosed and treated at the Armed Forces Capital Hospital were analysed. Of the Mycobacterium tuberculosis isolates, 87.4% were susceptible to all available anti-tuberculosis drugs; 253 (93.7%) patients eventually completed initial anti-tuberculosis treatment. Among the patients with smear-positive PTB, the cure rate was 89.3% (100/112). CONCLUSION: Our results demonstrate that the incidence of PTB in Korean soldiers, although still high, was declining steadily. With good case management, the overall success rate of initial treatment was approximately 90%.

Systemic-to-pulmonary venous shunt in superior vena cava obstruction: depiction on computed tomography venography.

PURPOSE: To describe computed tomography (CT) venographic appearances of systemic-to-pulmonary venous shunts with CT venography and three-dimensional reconstruction images from patients with superior vena cava obstruction. MATERIAL AND METHODS: From January 1994 to April 2002, CT venography was performed in 45 patients with superior vena cava obstruction using a single-detector helical CT scanner (n=38) and four-detector row CT scanner (n=7). Analysis of CT scan data included the cause and degree of venous obstruction, the presence of pleural thickening and enhancement, and the attenuation of pulmonary veins. The causative factor for systemic-to-pulmonary venous shunt was evaluated using the Fisher exact test. RESULTS: Systemic-to-pulmonary venous shunts were observed in four patients (9%) who had high-attenuated pulmonary veins and pleural enhancement on CT venography. Pleural thickening (P=0.01) and a history of pulmonary tuberculosis (P=0.034) are statistically significant risk factors. CONCLUSION: CT venography showed strong pleural enhancement and high-attenuated pulmonary veins indicating systemic-to-pulmonary venous shunts. Radiologists should study the earlier enhancement of pulmonary veins in patients with superior vena cava obstruction.

Modulation of immune responses by treadmill exercise in Sprague-Dawley rats.

AIM: The duration-dependence of the effect of forced treadmill exercise on the immune system is a subject of ongoing research. In this study, the effect of forced treadmill exercise on immune responses was investigated by evaluating the lymphocyte subset fractions in the peripheral blood and spleen of Sprague-Dawley rats. METHODS EXPERIMENTAL DESIGN: Comparative investigation over 8 weeks. SETTING: Experimental animal laboratory. PARTICIPANTS: Male Sprague-Dawley rats 5 weeks of age, weighing 150+/-10 g. INTERVENTIONS: Animals were randomly assigned to one of the 4 following groups: the control group, the 1-week-exercise group, the 4-week-exercise group, and the 8-week-exercise group. MEASURES: Lymphocyte subset fractions, including those for T, B, CD4+, and CD8+ cells and the T/B and CD4+/CD8+ ratios in the peripheral blood and spleen were measured via flow cytometric analysis after treadmill exercise. RESULTS: The T cell and CD4+ cell fractions in both the peripheral blood and spleen were increased significantly after 8 weeks of treadmill exercise, but the B cell and CD8+ cell fractions did not change significantly. CONCLUSION: From the results of the present study, it is suggested that a period of one week is insufficient to eliminate the effects of exercise-induced stress, that 4 weeks are needed to return to the control state, and that at least 8 weeks are needed in order for exercise of moderate intensity to have a positive effect on the immune system.

Pleckstrin homology domain interacts with Rkp1/Cpc2, a RACK1 homolog, to modulate Pck2-mediated signaling process in Schizosaccharomyces pombe.

Rkp1/Cpc2, a fission yeast RACK1 homolog, interacts with Pck2, a PKC homolog, and is involved in the regulation of pck2-mediated signaling process. The N-terminal region of split pleckstrin homology domain (nPH) in human PLC-gamma1 bound to Rkp1/Cpc2 concomitantly with Pck2. nPH inhibited kinase activity of GST-Pck2 purified from Schizosaccharomyces pombe in vitro. The lethality induced by pck2(+) overexpression was suppressed by coexpression of either rkp1(+) or nPH domain. This result suggests that Rkp1/Cpc2 interacts with PH domain-containing protein and regulates the Pck2-mediated signaling process in S. pombe.

Octa(mu3-selenido)hexarhenium(III) complexes containing axial monodentate diphosphine or diphosphine-monoxide ligands.

A series of the octahedral hexarhenium(III) complexes containing a variable number of diphosphine (diphos) or diphosphine-monoxide (diphosO) ligands have been prepared by the substitution of the diphosphine Ph2P(CH2)nPPh2 (n = 1 to 5) for the iodide ions in the parent octahedral hexarhenium cluster compound [Re6Se8I6]3-. The diphosphine Ph2P-(CH2)nPPh2 ligands adopt an eta1-bonding mode with the Re6(mu3-Se)8 core, and the P donor atom in the pendant arm is noncoordinated and oxygenated in most cases. The series of new hexarhenium(III) complexes have been well-defined by 1H, 13C, and 31P NMR spectroscopic and FAB-MS data. Four compounds among the series were characterized by X-ray structural determination. Geometrical isomers were identified by NMR spectroscopy as well as by the structural determinations. The apical ligand substitution induces significant change in the redox potentials and the photophysical properties of the Re6(mu3-Se), core. The E1/2 value of the reversible process ReIII6/ReIII5ReIV becomes more positive with the increasing number of the coordinated P donors. The phosphine-substituted hexarhenium(III) derivatives are highly luminescent, with microsecond scale emissive lifetime at ambient temperature, and the fully substituted derivatives with the formula [Re6Se8-(eta1-diphosO)6]2+ display the strongest luminescence with the longest emission lifetimes.

Characterization of the CDC10 product and the timing of events of the budding site of Saccharomyces cerevisiae.

Budding cells of the yeast Saccharomyces cerevisiae possess a ring of septin filaments of unknown biochemical nature that lies under the inner surface of the plasma membrane in the neck that connects the mother cell to its bud. Mutants, defective in any of the four genes (CDC3, CDC10, CDC11, CDC12), lack these septin filaments and display a pleiotropic phenotype that involves abnormal bud growth and an inability to complete cytokinesis. The cloned CDC10 was fused to bacterial genes to generate antibodies specific for the CDC10 product was a constituent of the septin filaments. Cdc10p-specific antibodies for septin staining and actin-specific rhodamine-phalloidine were used to investigate the timing of the localization of septin and actin at the budding site using the immunofluorescence microscopic technique. In wild-type cells, the timing of the appearance and disappearance of these proteins was indistinguishable. In addition, the cdc10 mutant did not prevent actin localization at the budding site. The mutant that was blocked in the actin function also did not prevent the septin localization of the Cdc10p. This result may suggest an organizational independence between these proteins in the bud formation. Finally, the localization of septin and actin in the cdc24 mutant cell was examined. It was found that the CDC24 function was necessary for the organization of septin and actin at the budding site.

Molecular cloning and functional expression of bovine brain GABA transaminase.

We isolated a cDNA that encodes the bovine brain gamma-aminobutyrate transaminase (GABA-T; EC 2.6.1.19) from the lambda gt 11 cDNA library, which showed a high degree of sequence similarity to the corresponding enzymes from various sources. Northern blot analysis revealed two differentially expressed GABA-T transcripts of approximately 2.0 and 6.0 kb in the bovine tissues. Southern blot analysis indicates that the two GABA-T transcripts are encoded in a greater-than 10-kb, single-copy gene. Bovine GABA-T cDNA was expressed in E. coli using the pGEX bacterial- expression vector system. The overexpressed GABA-T was enzymatically active after purification, and it had very similar kinetic parameters when compared with those of other mammalian GABA-Ts.

Caffeine inhibits exercise-induced increase in tryptophan hydroxylase expression in dorsal and median raphe of Sprague-Dawley rats.

Effect of caffeine on the expression of tryptophan hydroxylase (TPH), rate limiting enzyme of serotonin synthesis, in dorsal and median raphe was investigated via immunohistochemistry. In exercise groups, Sprague-Dawley rats were put on treadmill running for 30 min per day for 6 consecutive days. On the seventh day, animals of control-with-caffeine group were injected subcutaneously with 4 mg/kg caffeine, while control-without-caffeine group were injected with 0.9% NaCl, sacrificed 2 h later. Exercise-with-caffeine group and exercise-without-caffeine group were injected with caffeine and NaCl, respectively; all-out time was determined 1 h after injection, and then sacrificed. Caffeine increased all-out time in exercised rats, and inhibited the exercise-induced elevation in TPH expression. The suppressive effect of caffeine on TPH expression in exercised rats can be suggested as one possible ergogenic mechanism of caffeine.

Time-resolved total internal reflection fluorometry study on polarity at a liquid/liquid interface.

The polarity of a water/oil (oil: cyclohexane, carbon tetrachloride, toluene, chlorobenzene, o-dichlorobenzene, or 1,2-dichloroethane) interface was investigated by means of time-resolved total-internal-reflection (TIR) fluorescence spectroscopy of a polarity-sensitive probe: sulforhodamine B (SRB). In bulk solutions, the nonradiative decay rate constant of SRB increased with an increase in a solvent polarity parameter [ET(30)], and this relationship was used to estimate the polarities of water/oil interfaces. For the oil having a relatively low solvent polarity [ET(30) < 35 kcal/mol], the polarity of the water/ oil interface agreed with that of the arithmetic average of the polarities of the two phases [ET(30)calc]. For water/odichlorobenzene and water/1,2-dichloroethane interfaces [ET(30) of the oil > 35 kcal/mol], on the other hand, the interfacial polarity determined by TIR spectroscopy was lower than the ET(30)calc. The results are discussed in terms of thickness/roughness of the water/oil interface.

Rkp1/Cpc2, a fission yeast RACK1 homolog, is involved in actin cytoskeleton organization through protein kinase C, Pck2, signaling.

The Rkp1/Cpc2, a fission yeast RACK1 homolog, interacted with Pck2, one of the known PKC homologs, in vivo and in vitro. The rkp1-deletion mutants (Deltarkp1) are elongated and the pck2-deletion mutant (Deltapck2) showed abnormal morphology. The double-deletion mutant (Deltarkp1Deltapck2) showed more aberrant cell shapes and was sensitive to high salt concentration. Both Deltarkp1 and Deltapck2 cells were sensitive to latrunculin B (Lat B) which inhibits actin polymerization. The cells expressing the human RACK1 homolog complemented the latrunculin B sensitivity of Deltarkp1 indicating that human RACK1 is a functional homolog of Rkp1/Cpc2. We propose that Rkp1/Cpc2 may function as a receptor for Pck2 in the regulation of actin cytoskeleton organization during cell wall synthesis and morphogenesis of Schizosaccharomyces pombe.

Water structures in ion-exchange resin particles: solvation dynamics of Nile Blue A.

The structures of water, partitioned in cation-exchange resin particles, were studied on the basis of fluorescence dynamics of Nile Blue A (NB). The fluorescence lifetime of NB in the resin was longer than that in water and increased with increasing cross-linking density of the resin (rho). The results demonstrated that the water structures in the resin were significantly different from those in water and dependent on rho. A study on solvation dynamics of NB in the resin, reflecting structured water around the ion-exchange group, revealed the roles of "bound water" molecules in the water structures, since the solvent relaxation time (tauS) in the resin was much longer than that in water and depended on p; tauS increased from 34 to 55 ps with increasing rho from 2 to 8%. The origin of the rho dependence of tauS was discussed in terms of the separation distance between the ion-exchange groups, and the effects of the counterion of the ion-exchange group on the solvation processes were also discussed.

Short-term effect of mitomycin-C augmented trabeculectomy on axial length and corneal astigmatism.

PURPOSE: To evaluate the short-term effect of trabeculectomy with adjunctive mitomycin-C (MMC) on corneal astigmatism and axial length. SETTING: Asan Medical Center, University of Ulsan, Department of Ophthalmology, Seoul, Korea. METHODS: Eighteen consecutive eyes of 16 patients having trabeculectomy with adjunctive MMC were prospectively analyzed. Intraocular pressure (IOP) and axial length were evaluated preoperatively and 1, 3, 6, and 12 months postoperatively. Postoperative changes in corneal astigmatism were evaluated using vector analysis. RESULTS: The overall mean induced astigmatism showed with-the-rule change up to 3 months postoperatively followed by an against-the-rule shift. The mean axial length was significantly less postoperatively and changed throughout the 12 month follow-up. There was a positive correlation between postoperative axial length and IOP. Eyes with higher preoperative IOP had a greater decrease in axial length after trabeculectomy with MMC. CONCLUSION: The induced corneal astigmatism after trabeculectomy with MMC was long lasting, although less than that in previous studies. The change in axial length after surgery was significant, especially in eyes with a high preoperative IOP. The decrease in axial length persisted throughout the follow-up.

Molecular cloning and high-level expression of G2 protein of hantaan (HTN) virus 76-118 strain in the yeast Pichia pastoris KM71.

Hantaan viral G2 envelope gene, which is known to be one of major antigens and induce neutralizing antibodies, was cloned into expression vector pHIL-S1 which consists of AOX1 promoter, PHO1 signal sequence, HIS4 gene and other components. The recombined plasmid was transformed into methylotropic yeast, Pichia pastoris of KM71 and recombinant strains harboring multi-copy of G2 gene were selected. Expression of the cloned G2 gene was confirmed with Western blot analysis using anti-sera of guinea pig immunized with the carboxyl terminal region of G2 protein expressed in Escherichia coli. The expression of G2 gene from the recombinant strain was tightly repressed by dextrose and effectively induced by methanol, an inducer of AOX1 promoter. The highest expression level was observed from 1 day after induction and maintained at the same level for up to 4 days.
