Quality management in the doctor's consulting room.

Clinical research and the measurement of quality in healthcare have similar properties. Clinical research is generally restricted to university and teaching hospitals because of it's time consuming nature while quality management and subsequent quality measurement are of interest to all medical doctors. To serve both purposes a Kaplan Meier satellite module was developed as an add-on to a medical workstation. Kaplan Meier survival statistics has interesting possibilities as a clinical tool for the measurement and graphic visualisation of quality of care. It does express the clinical behaviour of a patient population in terms of cumulative survival, patency or any other binary phenomenon. A vascular surgeon collected cumulative patency data on 340 vascular reconstructions with this Kaplan Meier module within 6 months without any perceptable influence on the normal outpatient clinic routines. The module is easy to operate, easy to maintain and very adaptable to any clinical question.

Comparative study of strain gauge plethysmography and Doppler ultrasound in patients with occlusive arterial disease of the lower extremities.

A series of 305 normal subjects and 77 patients with angiographically proven stenotic or occlusive arterial disease of the lower limbs was studied with Doppler ultrasound pressure measurements and strain gauge plethysmographic arterial flow measurements. The 2 techniques were compared singly and in combination at rest and after exercise or reactive hyperemia. Pressure-flow patterns were found to correlate well with the segment(s) and extent of involvement with arterial disease, especially in patients with superficial femoral artery disease. However, it was found that calf blood flow measurements were of little assistance in predicting the level and severity of involvement over and above the information obtained by pressure measurements alone.

Interobserver variability in single-plane aortography.

Arteriography is regarded as the objective method for evaluation of arterial disease and is commonly used as the reference for the assessment of the accuracy of noninvasive diagnostic methods. Because no data are available supporting its reliability, a study regarding the interobserver variability of single-plane arteriography was done. Twenty-one translumbar aortograms were read by 11 surgeons and radiologists, and the results were analyzed with a statistical method which takes into account agreement by chance. It was found that interobserver agreement concerning the profunda bifurcation was only marginally more than that provided by chance. Occlusions were well recognized by the 11 observers, but the interobserver agreement on the degree of arterial stenosis was poor.

A critical review of the literature on long-term treatment of peripheral arteriosclerosis with oral anticoagulants.

Protracted administration of oral anticoagulant agents to patients presenting symptoms of peripheral arteriosclerosis is a treatment in widespread use in the Netherlands a Germany. However, a critical perusal of the available literature reveals a scarcity of objective data which might confirm the value of this treatment. It may be concluded from the data in the literature that protracted administration of anticoagulants may be useful only in case of stenosis of the superficial femoral artery. It is pointed out that a statistically correct study of this subject, with use of objective parameters, is urgently necessary.
