[Acute pericardial tamponade in cardiac echinococcosis].

An echinococcal cyst of the heart is a rare cause of acute cardiac tamponade. We report on a 24 year old male from the Kosovo who was brought in an emergency state from a provincial hospital complaining of severe dyspnea, thoracic pain, dizziness, and a short period of unconsciousness. Surgical decompression had to be performed urgently, because the pericardium could not be punctuated due to the position of the hydatid cyst. The differential diagnosis was cardiac tumor or echinococcal cyst. Because of a negative result of a test for anti-echinococcal antibodies (indirect haemagglutination) and no eosinophilia (5%), the diagnosis of hydatid cyst was at first discarded. Later on, the test for anti-echinococcal antibodies became positive and a marked eosinophilia (59%) was manifest. In combination with a typical appearance in the echocardiograph and NMR, the diagnosis of a cardiac hydatid cyst was made. After preoperative treatment with albendazole, the cyst was sterilized with a 20% NaCl solution and the contents evacuated. The therapy with albendazole was continued. When last seen eight months after the first incidence, the patient was well except some degree of dyspnea on exertion. As a differential diagnosis of a cardiac tumor, a hydatid cyst should be taken into account in patients from an area where Echinococcus granulosus is endemic. A negative test on antiechinococcal antibodies and the absence of eosinophilia do not rule out echinococcosis.

Isolation of antigens recognized by coeliac disease autoantibodies and their use in enzyme immunoassay of endomysium and reticulin antibody-positive human sera.

Components were isolated from rat liver, sheep lung, rhesus and orang-utan intestine. In enzyme immunoassay, these components detected 57%, 72%, 84% and 88% of human sera containing endomysium and reticulin antibodies (EmA and ARA). At most, 7% of EmA/ARA-negative sera reacted with the antigens. The spectrum of EmA/ARA-positive sera reactive with the various components was different but overlapping. When the antigens of sheep lung and orang-utan intestine were used as a cocktail, 98% of EmA/ARA-positive sera (45/46), but only 2% of control sera (1/46) were detected. The isolated antigens from sheep lung and monkey intestines were able to absorb EmA and ARA from human sera and thus should be suspected to contain the epitopes recognized by EmA and ARA, whereas the rat liver component did not bind. Therefore, the spectrum of autoantibodies in coeliac disease might comprise more than that covered by the terms EmA and ARA. The N-terminal amino acid sequence of some of the antigens was homologous with casein, gliadin, fibrinogen, and collagen.

[Significance of blood Ca(++) determination for assessment of the current status of calcium homeostasis].

Out of the blood calcium fractions know only the ionized calcium is biologically active. From the height of the total calcium level despite significant dependence no secure conclusion to the Ca++ concentration can be drawn. The level of the total calcium spreads further than that one of the ionized calcium. The up to now used mathematical approaches for the estimation of the blood Ca++ concentration also proved unsuitable; none of them provides a really better statement than the total calcium level. Thus even they represent no acceptable alternative to the Ca++ determination by means of ion-selective electrodes. The investigation by means of ion-selective electrodes. The investigation of various paraclinical sizes (total calcium, total protein, albumin, creatinine, alkaline phosphatase) in a group of patients and a control group in quality and quantity resulted in a different influence on the Ca++ level by the influential factors tested. Only by measurement of the ionized calcium the real actual condition of the calcium homoeostasis can be reflected. Therefore it is no more justified to draw diagnostically and therapeutically relevant consequences from the exclusive determination of the total calcium level.

[Digitalis therapy in ambulatory practice].

When an unselected number of patients with internal diseases was evaluated, a digitalization rate of 16.6% was found. In the majority of cases the glycoside treatment did not correspond to the strong standards demanded nowadays. The side effect rate of 19.6% emphasizes the necessity of a scientifically based mode of prescription.

[Behavior of copper and manganese in serum in hemodialysis].

The extent to which chronic dialysis alters the serum copper and manganese content was examined. The copper content was not significantly altered, whereas the level of manganese in the blood rose highly significantly. This manganese originates from the dialysis fluid. The behaviour of the manganese level during dialysis treatment suggests that the element is deposited in the body. Its transport is clearly connected with protein, since the manganese is not found in the ultrafiltrate. The is no fear of manganese intoxication.

[Crohn disease of the oral and pharyngeal cavities].

Crohn's disease is very rarely found in the oral and pharyngeal cavity. The article reports on a female patient with Crohn's disease in the oral and pharyngeal cavity. The differences in the macroscopic findings due to the disease being localized in the oral and pharyngeal cavity are described. The clinical oral and pharyngeal lesions which suggest the presence of Crohn's disease.

[Acute postsurgical noninflammatory renal failure-a survey (author's transl)].

A review on the most important aspects of acute postsurgical noninflammatory renal failure is given. New aspects have arisen especially in regard to the pathogenesis and treatment of the syndrome. In spite of the efforts to present a complete survey special attention has been attached to clinical aspects and treatment of acute postsurgical noninflammatory renal failure in order to inform those surgeons who, in acute cases cannot call for the assistance of a nephrologist with modern facilities.

[Hemodynamic parameters and plasma-renin activity following intravenous dihydralazine administration--a contribution to the regulation of renin].

On account of the complex effective mechanism the control of dihydralazin is performed particularly with regard to the changes of the plasma-renin-activity. The examinations were carried out in 7 patients each with renal and essential hypertension. The plasma-renin-activity, the behaviour of blood pressure, the stroke volume and the minute volume as well as the peripheral arterial resistance should be examined concerning the influence of two doses of dihydralazin and with regard to the influence of the basic disease to the above mentioned parameters. The plasma-renin-activity is decreased as well as increased by dihydralazin. Baroreceptors are made responsible for the renin regulation. Already with 4.125 mg dihydralazin (1/8 ampoule Depressan) nearly the same permanent effect reducing the blood pressure is to be achieved as with a double dosage. Stroke volume and minute volume are reduced by dihydralazin. The peripheral arterial resistance decreases in no case, but in the renal hypertension it shows a rising tendency. With the help of these examinations it shall particularly referred to the fact that the therapeutic mechanism of the slowly beginning and long lasting decrease of blood pressure is not yet be fully clarified.

[Osmotherapy of acute kidney failure with special reference to plasma renin activity and mineral balance].

13 dogs were used to generate an acute toxic renal failure by sublimate. The alterations both of plasma renin activity (PRA) and of the acid-base and mineral balance were observed in uremia and after "osmotherapy" with mannitol (13 dogs). In the course of toxic renal failure the PRA increases on the average. A decompensated metabolic acidosis with general transmineralization is observed. Since in the stage of anuria the Na+ in urine fails to reach the macula densa cell canicularly, the theory is advanced that only the intracellular sodium in the macula densa cell is responsible for the regulation of the position of the vas deferens. Following "osmotherapy" the PRA is significantly lowered. The acidosis is still worsened under conditions of anuria. For the regulation of renin it has to be assumed that the enzyme in acute anuria after osmotherapy is being altered especially by volume receptors. Thus, osmotic therapy seems to be reasonable only with existing residual diuresis. This is valid especially for the administration of mannitol, because this substance remains strictly outside the cell.
