[Management of liver cell adenoma and focal nodular hyperplasia].

Since 1975 six cases with hepatic adenoma (5 females, 1 male) and five cases with focal nodular hyperplasia (3 females, 2 males) have been treated at our hospital. Four of the five women with hepatic adenoma took oral contraceptive pills before the diagnosis was made, only one patient took the pills in the group with focal nodular hyperplasia. Hepatic adenomas were resected electively in three patients; one patient underwent operation because of ruptureed adenoma with intraabdominal hemorrhage. In the other two cases with hepatic adenoma resections were not possible, but both tumors have regressed after the cessation of oral contraceptives. One patient in the group with focal nodular hyperplasia underwent liver resection because of intratumoral hemorrhage with recurrent pain; in the other four cases no treatment was done as symptoms were absent. We recommend elective resection for hepatic adenoma because of the high risk of spontaneous ruptur. Embolization or ligation of the hepatic artery are alternative procedures when resection is not possible and tumors do not regress after the cessation of oral contraceptives. On the other hand a more conservative attitude is suggested for focal nodular hyperplasia because those patients do not frequently bleed; resection is indicated in case of symptoms or growth of tumor.

[Drug-induced esophageal ulcers].

In cases without a history of gastrointestinal or cardiac disease, acute odynophagia prompts the tentative diagnosis of drug-induced esophageal ulcer. Possible causes are tetracycline, clindamycin, emepronium bromide, potassium chloride, etc. Other diseases such as carcinoma can be ruled out by endoscopy and biopsy. To avoid such esophageal lesions drugs should be taken with sufficient fluid and not immediately before bedrest.

[PABA test versus chymotrypsin and fat determinations in the stool in pancreatic functional diagnosis].

32 patients with proven chronic pancreatitis and 56 controls without evidence of pancreatic disease were studied by the PABA test, the fecal chymotrypsin method and the fecal fat method. The sensitivity of the fecal chymotrypsin method for detection of pancreatic disease was significantly higher (p less than 0.005). The sensitivity of the PABA test and the fecal fat method were comparable. The specificity of the PABA test and the fecal chymotrypsin method was of the same order.

[Large colonic polyp: endoscopic or surgical removal?].

93 neoplastic polyps measuring more than 2 cm in diameter have been removed from the colon by endoscopy. 15% of the polyps showed severe atypia (focal carcinoma), and in 15% an invasive carcinoma was found. Polypectomy was complicated in 14 cases by bleeding, in 12 cases it was cured by endoscopy. In 1 case in which the colon was injured by coagulation the lesion healed spontaneously. The surgery rate due to complications of the endoscopic removal procedure or due to invasive carcinoma was 7% for the polyps with a diameter of 2-3 cm, 15% for those of 3-5 cm and 50% for polyps with a diameter of more than 5 cm. It is therefore suggested that neoplastic polyps of more than 5 cm in diameter should be removed by endoscopy in high risk patients only.

[Angiodysplasia of the colon: diagnosis and therapy].

Angiodysplasias of the colon are rare causes of intestinal bleeding. Diagnosis is by angiography or colonoscopy. In 6 patients with severe anemia in whom conventional methods had failed to reveal the source of bleeding, colonoscopy demonstrated angiodysplasias in the cecum or ascending colon. They were treated endoscopically. In a follow-up period of 3 months to 4 years only 1 patient has sometimes suffered occult fecal blood loss, but is adequately treated by oral iron therapy. No patient has had a severe recurrence of bleeding, nor was colonic resection necessary. It is concluded that endoscopic therapy of angiodysplasias is a valuable alternative to surgery with low risk and high efficiency.

[How effective are simple laboratory tests (Hb, BSG) in the early diagnosis of stomach neoplasms].

Up to now only early resection has proven of value in healing gastric carcinoma. It is therefore mandatory that the pathologic lesion be diagnosed as early as possible. Thus, patients with persistent epigastric complaints under symptomatic treatment should undergo endoscopy even if blood parameters are still within normal limits. The fact that, among all our gastric cancers diagnosed in 1977 by endoscopy, 23% were classified as early cancers indicates that the method is well suited to establishing true early diagnosis.

[A safe alternative for the diagnosis of pernicious anemia--a contribution to medical economics (proceedings)].

Mucosal biopsies of corpus and antrum were performed in 17 patients with pernicious anemia. In all patients the corpus mucosa showed atrophic gastritis, while in 16 the antrum was normal or showed only superficial gastritis. This combination is conclusive for pernicious anemia. Over 80% of megaloblastic anemias are caused by pernicious anemia. It is therefore proposed that endoscopy and biopsy be performed as the first examination in these patients. In this way the diagnosis is simple, rapid and economical.

[What is the contribution of colposcopic polypectomy?].

Coloscopic polypectomy saves patients with polyps of the colon from diagnostic laparotomy and also serves for the diagnosis and in the majority of patients even therapy of early colonic cancer. Complications are rare and most can be controlled by endoscopy. Recurrences of the polyp are possible, and therefore, follow-up is necessary.

[Drug-induced oesophageal ulcers (author's transl)].

Within a one-year period seven patients were observed who had developed ulcers of the upper and mid oesophagus after treatment with doxycycline hydrochloride (n = 3), emepronium bromide (n = 3) or Pantogar (n = 1). In each instance the drug had apparently been swallowed dry. The typical symptoms were a sudden onset of retrosternal chest pain and odynophagia during bed rest. Once the drug had been discontinued and treatment with antacid combined with topical anaesthetics and/or alginic acid instituted the symptoms disappeared within a few days. The authors stress that drugs should be swallowed only with good amounts of fluid and generally not immediately before bed rest.

[Esophageal function after fundoplication (Nissen)].

Manometric, pH-metric, radiological and histological examinations were performed in 15 of 18 consecutive patients with gastro-esophageal reflux before and after fundoplication. In 13 of these 15 patients, reflux symptoms largely or completely disappeared after the operation. In the successfully operated patients, the reflux provocation test markedly improved but lower esophageal sphincter pressure did not rise. Six successfully operated patients in whom hiatus hernia was present after the operation and 7 patients without hernia had similar esophageal function tests. Therefore, success of surgery does not depend on resting pressure and position of the lower esophageal sphincter.

[Multiple leiomyomatous hamartoma of the lungs].

Leiomyomatous hamartomas of the lungs are very rare lesions. As far as we know only 14 cases (including ours) have been published so far. In contrast to chondromatous hamartomas, they are exclusively found in women and as a rule are multiple. They are frequently an incidental finding in chest roentgenograms, where they appear as multiple round nodules which are often mistaken for metastases. However, as these lesions remain virtually the same size for long observation periods, differentiation is easily possible. This fact is well shown in our patient, in whom multiple nodules in the lungs were diagnosed at the age of 45 years and observed over a period of 35 years. They were first identified as leiomyomatour hamartomas at autopsy. No therapy is indicated as these lesions have no tendency to become malignant and only rarely and insignificantly interfere with respiratory function. Thoracotomy with biopsy for histological diagnosis is however indicated, since only thus can metastases be definitely ruled out.
