Whole genome expression analysis for biologic rational pathway modeling: application in cancer prognosis and therapy prediction.

Using semi-quantitative microarray technology, almost every one of the approximately 30 000 human genes can be analyzed simultaneously with a low rate of false-positives, a high specificity, and a high quantification accuracy. This is supported by data from comparative studies of microarrays and reverse-transcription PCR for established cancer genes including those for epidermal growth factor receptor (EGFR), human epidermal growth factor receptor-2 (HER2/ERBB2), estrogen receptor (ESR1), progesterone receptor (PGR), urokinase-type plasminogen activator (PLAU), and plasminogen activator inhibitor-1 (SERPINE1). As such, semi-quantitative expression data provide an almost completely comprehensive background of biological knowledge that can be applied to cancer diagnostics. In clinical terms, expression profiling may be able to provide significant information regarding (i) the identification of high-risk patients requiring aggressive chemotherapy; (ii) the pathway control of therapy predictive parameters (e.g. ESR1 and HER2); (iii) the discovery of targets for biologically rational therapeutics (e.g. capecitabine and trastuzumab); (iv) additional support for decisions about switching therapy; (v) target discovery; and (vi) the prediction of the course of new therapies in clinical trials. In conclusion, whole genome expression analysis might be able to determine important genes related to cancer progression and adjuvant chemotherapy resistance, especially in the context of new approaches involving primary systemic chemotherapy. In this review, we will survey the current progress in whole genome expression analyses for cancer prognosis and prediction. Special emphasis is given to the approach of combining biostatistical analysis of expression data with knowledge of biochemical and genetic pathways.

[Papillary tumors of the breast].

The term papilloma applies to benign proliferative epithelial breast lesions with a papillary architecture. The papillae in such lesions contain an arborizing fibrovascular core, glandular surface epithelium and a basal myoepithelial layer. A basement membrane encloses these structures. Papilloma may occur at any site in the ductal lobular system and according to its localization is subdivided into two types: solitary (central) papilloma which are located in the major nipple/subareolar ducts or large segmental ducts and multiple (peripheral) papillomas in cystically dilated terminal ductal lobular units (TDLU). Stromal changes, epithelial metaplasia and/or proliferations and neoplasia may alter the prototypical architecture. In a significant number of papillomas atypia can be identified which have to be classified as atypical proliferates of the ductal type. These lesions must be distinguished from the papillary type of ductal carcinoma in situ. Some 17% of all papilloma are associated with (synchronous) intraductal or invasive carcinoma, but these also act as an indicator for subsequent (metachronous) carcinoma. As a consequence, in minimally invasive biopsy papilloma has to be classified as B3 and usually has to be followed by surgical excision.

[The significance of "normal tissue" in the development of breast cancer: new concepts of early carcinogenesis].

Only little information on the primary molecularbiological events involved in early breast is available. In particular, the definition of postulated precursor lesions of invasive breast cancer, such as ductal hyperplasia or ductal carcinoma in situ, is under an intense, controversial discussion in terms of pathogenesis and tumor biology. The most recent research on biological regulation mechanisms and genetic alterations in morphologically normally appearing breast tissue give rise for a reinterpretation for the most common progression models of breast cancer. The detection of genetic alterations within normal breast tissue in particular challenges the commonly postulated relationship between invasive and in situ breast carcinomas on the one hand, and benign, proliferative breast lesions on the other. The concerns about these relationship are further supported by the description of different cellular compartments within the normal female breast, including a "progenitor cell compartment" with different cytokeratin expression patterns, which can be transferred towards well known or suspected precursor lesions of invasive and in situ breast cancer. The aim of this manuscript is to provide an overview of the most recent results and developments in breast pathology, and to describe the consequences of our changing understanding of breast carcinogenesis.

[Avascular necrosis of the capitate in athletes].

The possibility of osteonecrosis of the carpal bones should always be considered when athletes present with pain of unknown origin in the hand and wrist, in particular, if they are participating in sports such as gymnastics or weight-lifting that involve extreme loading of the wrist with axial compression and microtrauma. This sort of extreme loading of the wrist combined with a constitutionally "weak" blood supply to the individual carpal bones may lead to the formation of osteo-necrotic zones. A treatment method that can produce excellent results, depending on the pathomorphology, is available in the form of vascularized bone grafting.

High resolution infrared spectra and rovibrational analysis of the coupled nu2/nu5 bands of D3Si35Cl. Equilibrium rotational constants.

The Fourier transform infrared spectrum of monoisotopic D3Si35Cl in the region of the nu2/nu5 band system was recorded with a resolution of 2.4x10(-3) cm-1. More than 9000 lines of the strongly Coriolis x,y-coupled bands, (nu2)0=701.936, and (nu5)0=688.898 cm-1, have been assigned, among them 276 forbidden but perturbation allowed transitions around avoided crossings according to Delta(k-l)=+/-3 mechanisms. Three different models taking into account redundancies in the framework of unitary equivalent reductions of the rovibrational Hamiltonian have been employed to fit the data. All three models reproduced consistently the full data set employing 28 refined parameters with an rms deviation of 0.31x10(-3) cm-1. The equivalence of the parameter sets was established by the agreement of parameter sums obtained with the different models. The analysis of the avoided crossings, together with the fit of the forbidden lines, allowed an independent determination of the ground state parameters A0 and D(K)0. Combined with existing data for nu1, nu3, nu4, and nu6, the present results allowed the determination of experimental values, Ae=1.4371895(94) and Be=0.19823049(59) cm-1. The experimental results are compared with those of previous ab initio calculations of the anharmonic force field.

Deciphering a subgroup of breast carcinomas with putative progression of grade during carcinogenesis revealed by comparative genomic hybridisation (CGH) and immunohistochemistry.

Distinct parallel cytogenetic pathways in breast carcinogenesis could be identified in recent years. Nevertheless, it remained unclear as to which tumours may have progressed in grade or which patterns of cytogenetic alteration may define the switch from an in situ towards an invasive lesion. In order to gain more detailed insights into cytogenetic mechanisms of the pathogenesis of breast cancer, the chromosomal imbalances of 206 invasive breast cancer cases were characterised by means of comparative genomic hybridisation (CGH). CGH data were subjected to hierarchical cluster analysis and the results were further compared with immunohistochemical findings on tissue arrays from the same breast cancer cases. The combined analysis of immunohistochemical and cytogenetic data provided evidence that carcinomas with gains of 7p, and to a lesser extent losses of 9q and gains of 5p, are a distinct subgroup within the spectrum of ductal invasive grade 3 breast carcinomas. These aberrations were associated with a high degree of cytogenetic instability (16.6 alterations per case on average), 16q-losses in over 70% of these cases, strong oestrogen receptor expression and absence of strong expression of p53, c-erbB2 and Ck 5. These characteristics provide strong support for the hypothesis that these tumours may develop through stages of well- and perhaps intermediately differentiated breast cancers. Our results therefore underline the existence of several parallel and also stepwise progression pathways towards breast cancer.

First stretching overtone of BiH3: an extreme local-mode case for XH3-type molecule?

The first stretching overtone region of short-lived, formerly inaccessible BiH3 near 3405 cm(-1) has been measured by Fourier-transform infrared spectroscopy with a resolution of 0.0066 cm(-1). Only the 2nu1(A1)/nu1+nu3(E) band system has been observed. Rotational analysis, with transitions reaching J'max=14, has revealed almost perfect local-mode behavior for the upper states denoted as (200A1/E) in the local-mode notation. Ratios of vibration-rotation interaction parameters q(eff)/alpha(eff)(BB) and r(eff)/alpha(eff)(BC), and the appropriate rotational constant differences, are in good agreement with theoretical local-mode limit values. A simple stretching vibrational model reproduces the observed vibrational term values well, and the potential parameters obtained are close to true values.

Overexpression of vascular endothelial growth factor (VEGF) and its cellular receptor KDR (VEGFR-2) in the bone marrow of patients with acute myeloid leukemia.

Vascular endothelial growth factor (VEGF) and its cellular receptor VEGFR-2 have been implicated as the main endothelial pathway required for tumor neovascularization. However, the importance of the VEGF/VEGFR-2 system for angiogenesis in hematologic malignancies such as AML remains to be elucidated. In 32 patients with newly diagnosed untreated AML, we observed by immunohistochemical analysis of bone marrow biopsies significantly higher levels of VEGF and VEGFR-2 expression than in 10 control patients (P <0.001). In contrast, VEGFR-1 staining levels in AML patients were in the same range as in the controls. Expression of VEGF and VEGFR-2 was significantly higher in patients with a high degree of microvessel density compared to those with a low degree (VEGF: P =0.024; VEGFR-2: P =0.040) and correlated well with bone marrow microvessel density (r(s)=0.566 and 0.609, respectively; P <0.001). Furthermore, in patients who achieved a complete remission following induction chemotherapy VEGFR-2 staining levels decreased into the normal range. In conclusion, our results provide evidence for increased expression of VEGF/VEGFR-2 of leukemic blasts and correlation with angiogenesis in the bone marrow of AML patients. Thus, VEGF/VEGFR-2 might constitute promising targets for antiangiogenic and antileukemic treatment strategies in AML.

[Chondrosarcoma of the sphenoethmoid complex: Case report].

BACKGROUND: Chondrosarcomas are rare malignant mesenchymal tumors of unknown etiology. Only 5 to 10 % appear in the head and neck region. PATIENT AND RESULTS: We report the case of a 63 year old woman with a chondrosarcoma of the sphenoethmoidal complex extending into the anterior cranial fossa. The tumor was subtotally resected via an osteoplastic lateral rhinotomy. Postoperatively, a fractionated radiation therapy with carbon ions (Schwerionen) was applied. There was no evidence of recurrence one year after therapy. CONCLUSION: Recent prognostic improvements of skull base chondrosarcomas by combination of skull base surgery and innovative radiation therapy are discussed and the literature is presented.

Synthesis and properties of the tetrakis(trifluoromethyl)borate anion, [b(CF3)4]-: structure determination of Cs[B(CF3)4] by single-crystal X-ray diffraction.

Salts of the tetrakis(trifluoromethyl)borate anion, M[B(CF3)4], M=Li, K, Cs, Ag, have been prepared by two different routes for the first time. The colorless compounds are thermally stable up to 425 C (Cs salt) and soluble in anhydrous HF, water, and most organic solvents. Single crystals of Cs[B(CF3)4] were grown from diethyl ether by diffusion of CH2Cl2 vapor into the solution. The molecular structure was obtained by single-crystal X-ray diffraction. Crystal data: rhombohedral space group R3m (no. 160); a =7.883(1), c=13.847(4) A: V=748.2 A3; Z=3; T=150K; R1=0.0118, wR2=0.0290. The internal bond parameters of the [B(CF3)4] ion were compared to those of the C(CF3)4 molecule. Due to a disorder of the anions in the cesium salt, it is not possible to distinguish between T and Td symmetry by X-ray diffraction experiments alone. However, a comprehensive IR and Raman study demonstrated that in the potassium and cesium salt as well as in aqueous solution, the anion exhibits T symmetry with all CF3 groups rotated off the staggered position required for Td symmetry. The vibrational study is supported by DFT calculations, which provide, in addition to the equilibrium structure and vibrational wavenumbers, estimates of IR and Raman band intensities. The anion is resistant against strong oxidizing (e.g., F2) as well as reducing agents (e.g., Na) and is not affected by nucleophiles like C2H5O or electrophiles such as H3O+. It is very weakly coordinating, as demonstrated by the low-equilibrium CO pressure over the [Ag(CO)x][B(CF3)4] (x=1, 2) co-adducts and the formation of [Ag(CO)x][B(CF3)4] (x=3,4) at higher CO pressure. The 11B, 13C, and 19F NMR data as well as the structural parameters of the anion are compared with those for other borates containing F, CN, and CF3 ligands.

The multicentric epitheloid hemangioendothelioma of bone: a case example and review of the literature.

Epitheloid hemangioendothelioma of bone is a rare primary bone malignancy. Diagnosis can be difficult for physicians who are not experienced with bone neoplasms. We also had difficulties in diagnosis and treatment in one of our patients with a multicentric epitheloid hemangioendothelioma involving the pelvis, left femur, and left tibia. We recommend a complete skeletal survey with magnetic resonance imaging because it can reveal previously undetected lesions on conventional radiographs. We performed an extraarticular resection of the hip joint including the spina iliaca anterior inferior, resection of the femur, and amputation of the lower leg. The femur was replaced by a modular endoprosthesis. The patient is provided with an above-knee prosthesis and is able to walk even longer distances with a cane 2 years after surgery. In our opinion it is necessary to perform a wide resection of this tumour in order to treat a patient with curative intention. Palliative radiotherapy should only be used for a non-resectable tumour or in metastatic disease. Chemotherapy is not a treatment option.

Cytogenetic analysis of multifocal bladder cancer supports a monoclonal origin and intraepithelial spread of tumor cells.

Bladder cancer is often characterized by a multifocal growth pattern. This observation has given rise to the hypothesis of "field cancerization," predicting a polyclonal origin of multiple tumors rising from an area of independently transformed mucosa cells. On the other hand, genetic studies suggested a monoclonal origin. To address these contradictory hypotheses, we performed comparative genomic hybridization (CGH) on 32 tumors originating from six bladder cystectomy specimens. All tumors derived from the same patient showed a set of 7-13 identical chromosomal aberrations and additional individual alterations. Most striking were the findings of 17p losses in all (32 of 32) tumors of the six cystectomy specimens and 20p gains in all tumors of four bladders, as well as an unexpected high number of chromosomal changes (20.4 alterations per tumor on average). To clarify a possible role of the TP53 tumor suppressor gene on 17p13, we applied immunohistochemistry and sequence analysis on the tumors and additional 52 mucosa samples. Identical TP53 mutations and protein overexpression was found in individual tumors only as well as in mucosa samples from continuous areas. Our results not only provide further evidence for a monoclonal origin of multifocal bladder cancer but also point at intraepithelial migration of tumor cells carrying specific chromosomal aberrations.

High-Resolution Infrared and Millimeter-Wave Study of the Fermi-Coupled v(2) = 1 and v(3) = 2 States of DSiF(3).

The nu(2) (nu(eff.) 854.841 cm(-1)) and 2nu(3) infrared bands (nu(eff.) 840.083 cm(-1)) of DSiF(3) have been studied with a resolution of 2.5 x 10(-3) cm(-1). Moreover, millimeter-wave transitions in the v(2) = 1 and v(3) = 2 states up to J" = 33 have been measured. The assignments and fit of the poorly resolved, compressed cluster-type 2nu(3) IR transitions have been confirmed by a simultaneous study of the 2nu(3)-nu(3) band. The constant W = 5.116 cm(-1) of the Fermi interaction between the v(2) = 1 and v(3) = 2 levels has been determined from frequency effects which are in agreement with relative intensities of the nu(2) and 2nu(3) bands. The deperturbed (B(0) - B(v)) and (C(0) - C(v)) values of the states involved agree with their ab initio predictions within 7% in the worst case. Copyright 2001 Academic Press.

Soft tissue reconstruction of megaprostheses using a trevira tube.

In soft tissue reconstruction of megaprostheses, the reattachment of soft tissue and joint capsules is essential. Sixty-nine megaprostheses were implanted and a trevira tube was applied to support reconstruction of the capsule and soft tissue. In cases of proximal femur replacement (33 patients), total femur replacement (five patients), and proximal humerus replacement (16 patients), the trevira tube allowed for reconstruction of the capsule and refixation of the muscles and helped to minimize dislocation. In cases of proximal tibia replacement (seven patients), arthrodesis of the knee (three patients), total knee replacement (two patients), and distal femur replacement (three patients), the trevira tube allowed for attachment of muscle flaps and extensor apparatus. Dislocation was observed in two of 54 patients who had proximal femur replacements. No dislocation was observed in patients with a total femur endoprosthesis or a proximal humerus endoprosthesis. The trevira tube also was used to attach the gastrocnemius muscle in patients with a proximal tibia endoprosthesis and to reattach the rotator cuff in patients with a proximal humerus prosthesis. There was no significant increase in the rate of infection. The histopathologic findings in six patients showed tissue ingrowth into the tube.

Increased angiogenesis in the bone marrow of patients with acute myeloid leukemia.

The importance of angiogenesis for the progressive growth and viability of solid tumors is well established. In contrast, only few data are available for hematologic neoplasms. To investigate the role of angiogenesis in acute myeloid leukemia (AML), bone marrow biopsies from 62 adults with newly diagnosed, untreated AML (day 0) were evaluated. Further studies were done after the completion of remission induction chemotherapy (day 16 of induction chemotherapy, n = 21; complete remission, n = 20). Microvessels were scored in at least 3 areas (x500 field, 0.126 mm(2)) of the highest microvessel density in representative sections of each bone marrow specimen using immunohistochemistry for von Willebrand factor and thrombomodulin. Microvessel counts were significantly higher in patients with AML (n = 62) compared with control patients (n = 22): median (interquartile range) 24.0 (21.0-27.8)/x500 field vs 11.2 (10. 0-12.0)/x500 field, respectively (P <.001). On day 16 of induction chemotherapy, microvessel density was reduced by 60% (44-66) (P <. 001) in hypoplastic marrows without residual blasts, in contrast to only 17% (0-37) reduction in hypoplastic marrows with >/= 5% residual blasts (P <.001 for the difference between both groups). Bone marrow biopsies taken at the time of complete remission displayed a microvessel density in the same range as the controls. In conclusion, there is evidence of increased microvessel density in the bone marrow of patients with AML, which supports the hypothesis of an important role of angiogenesis in AML. Furthermore, these findings suggest that antiangiogenic therapy might constitute a novel strategy for the treatment of AML. (Blood. 2000;95:2637-2644)

Rovibrational Spectroscopy of the v(5) = 1 Level of (28)SiDF(3).

The nu(5) fundamental band of trifluorosilane-d (SiDF(3)) at 627 cm(-1) was studied for the first time by high-resolution FTIR spectroscopy at a resolution of 2.4 x 10(-3) cm(-1). The analysis was performed simultaneously with available microwave and newly measured submillimeter-wave data in the approximation of an isolated degenerate fundamental level of a C(3 Kv) symmetric top molecule leading to a standard deviation of 0.22 x 10(-3) cm(-1) for the reproduction of the infrared wavenumbers, 36 kHz for the microwave, and 198 kHz for the submillimeter-wave frequencies, respectively. The unitary equivalence between the two reductions (Q and D) of the effective Hamiltonian applied in the analysis is demonstrated. Copyright 2000 Academic Press.

High-Resolution Infrared Spectrum of BrCN in the nu(2) and nu(1)/2nu(2) Regions.

FT infrared spectra of BrCN have been recorded in the region of the nu(2) band near 340 cm(-1), the nu(1) band near 580 cm(-1), and the 2nu(2) band near 690 cm(-1) with a resolution between 2.9 and 4.7 x 10(-3) cm(-1). The vibrational levels (01(1)0), (10(0)0), (02(0)0), (02(2)0), (11(1)0), and (20(0)0) have been analyzed employing cold bands, hot bands, and new millimeter-wave transitions. Band-by-band polynomial analyses and a combined fit of all data relevant to the 2v(1) + v(2) = 2 polyad levels have been performed. The latter fit considered l-resonance interactions between the (02(0)0), e and (02(2)0), e levels and Fermi resonance between the two Sigma states (10(0)0) and (02(0)0). Altogether about 1000 pieces of data up to J = 100 were fitted for each of the two isotopic species with rms of the residuals of 2-8 x 10(-4) cm(-1) for the infrared and 10-120 kHz for the pure rotational data.
