Enumeration of Klebsiella spp. in cold water by using MacConkey-inositol-potassium tellurite medium.

MacConkey-inositol-potassium tellurite agar was field tested for its ability to selectively enumerate Klebsiella species from the waters of the Saint John River Basin, which include fresh and marine waters. Water temperature varied from 1 to 6 degrees C during the survey period. Results of the study indicated that 77% of the typical colonies on MacConkey-inositol-potassium tellurite medium were Klebsiella species, but the total Klebsiella population enumerated was greatly underestimated.

Pseudo-chondrosarcoma: solitary osseous metastases from atypical bronchogenic carcinoma.

A patient with bronchogenic carcinoma with a solitary metastases to the femur in whom the clinical, radiologic, and pathologic setting suggested that the primary tumor was the bone lesion (chondrosarcoma) and the pulmonary lesion was a solitary metastasis is presented. This case is added to the limited literature indicating that mucin-secreting tumors metastatic to bone may simulate primary bone tumors radiographically.

Comparison of serum calcium measurements with respect to five models of atomic absorption spectrometers using NBS-AACC calcium reference method and isotope-dilution mass spectrometry as the definitive method.

Utilizing the recently described reference method for calcium (NBS-AACC) and the recently developed definitive (referee) NBS method for serum calcium measurement by isotopedilution mass spectrometry (IDMS), an evaluation of five recent-model atomic absorption spectrometers was carried out. Under optimal conditions of instrument operation using aqueous standards, significant differences were found during the comparative analyses of three lyophilized pool samples and one liquid serum pool sample. Use of the NBS-AACC serum calcium protocol did not guarantee analytic results within +/- 2% of the IDMS value. In four of eight comparisons, differences from IDMS greater than 2% were observed. Several variables were studied to account for these differences. It was shown that a serum matrix, when present in standards used to bracket the unknown sample, reduced differences between instruments in four of four instances and improved the accuracy of the results from a range of -1.1 to +3.5% to +0.1 to +1.0%. It is concluded that a serum sample with a verified IDMS calcium value is a valuable tool that establishes an accurate and stable reference point for serum calcium measurement. The use of transfer-of-NBS-technology multipliers is suggested. Regional quality control serum pools and clinical chemistry survey sample materials that have been analyzed for calcium concentration by the NBS-IDMS definitive method are examples of these multipliers.

A review of Clinical Terms Version 3 (Read Codes) for speech and language record keeping.

There may be bugs, there may be viruses, but computers are here to stay. Access to computers may vary greatly from 'seen one once' to daily usage. However, with an increasing dependence on electronic records, even the most remote therapist will ultimately be faced with computerization. To make this process as painless as possible some commonality in approach is logical. A common vocabulary, for instance, would ensure that dysphasia in Cape Town is the same as dysphasia in Dundee. This is great in theory but is Clinical Terms Version 3 (Read Codes) (CTV3) sufficient to encode records in clinical practice? It is to this end that the Speech and Language Therapy Department at Burton Hospital took part in a multidisciplinary project with the NHS Centre for Coding and Classification (NHS CCC). (NHS CCC became the NHS Information Authority, Coding and Classification on 1 April 1999.) Their CTV3 offers a standardized clinical terminology. An audit of patient case-notes found that 78% of the terms used by therapists were available in CTV3. Although there are many issues raised concerning electronic patient records, CTV3 presented as a potential vocabulary for recording patient information in this acute setting.
