Sensorimotor cortical activity in patients with complete spinal cord injury: a functional magnetic resonance imaging study.

Residual activation of the cortex was investigated in nine patients with complete spinal cord injury between T6 and L1 by functional magnetic resonance imaging (fMRI). Brain activations were recorded under four conditions: (1) a patient attempting to move his toes with flexion-extension, (2) a patient imagining the same movement, (3) passive proprio-somesthesic stimulation of the big toes without visual control, and (4) passive proprio-somesthesic stimulation of the big toes with visual control by the patient. Passive proprio-somesthesic stimulation of the toes generated activation posterior to the central sulcus in the three patients who also showed a somesthesic evoked potential response to somesthesic stimulation. When performed under visual control, activations were observed in two more patients. In all patients, activations were found in the cortical areas involved in motor control (i.e., primary sensorimotor cortex, premotor regions and supplementary motor area [SMA]) during attempts to move or mental imagery of these tasks. It is concluded that even several years after injury with some local cortical reorganization, activation of lower limb cortical networks can be generated either by the attempt to move, the mental evocation of the action, or the visual feedback of a passive proprio-somesthesic stimulation.

Functional MR imaging and traumatic paraplegia: preliminary report.

PURPOSE: To evaluate residual activity in the sensorimotor cortex of the lower limbs in paraplegia. METHODS: 5 patients suffering from a complete paralysis after traumatic medullar lesion (ASIA=A). Clinical evaluation of motility and sensitivity. 1. Control functional MR study of the sensorimotor cortex during simultaneous movements of hands, imaginary motor task and passive hands stimulation. 2. Concerning the lower limbs, 3 fMRI conditions: 1-patient attempts to move his toes with flexion-extension, 2-mental imagery task of the same movement, 3-peripheral passive proprio-somesthesic stimulation (squeezing) of the big toes. RESULTS: Activations were observed in the primary sensorimotor cortex (M1), premotor regions and in the supplementary motor area (SMA) during movement and mental imaginary tasks in the control study and during attempt to move and mental imaginary tasks in the study concerning the lower limbs. Passive somesthesic stimulation generated activation posterior to the central sulcus for 2 patients. CONCLUSION: Activations in the sensorimotor cortex of the lower limbs can be generated either by attempting to move or mental evocation. In spite of a clinical evaluation of complete paraplegia, fMRI can show a persistence of sensitive anatomic conduction, confirmed by Somesthesic Evoked Potentials.

[Changes in urethral pressure after intravenous injection of moxisylyte hydrochloride in urethral instability in women. Preliminary results].

OBJECTIVE: To study the action of an alpha blocker, Moxisylyte hydrochloride, during an intravenous test on the course of urethral pressure in women with urethral instability associated with urethral hypertonia. METHODS: The population consisted of 20 women with a mean age of 38 years, presenting with a clinical disorder of micturition (urinary incontinence: 15 cases, urgency: 17 cases, frequency, 17 cases) present for an average of 4 years and associated with resting urethral pressure variations ranging from 22 to 88 cm H2O (mean: 44.8 cm H2O) and static urethral pressures ranging 72 to 150 cm H2O (mean: 102.5 cm H2O). An urodynamic assessment was performed before and after intravenous injection of Moxisylyte hydrochloride at the dose of 0.5 mg/kg. RESULTS: Moxisylyte hydrochloride induced a significant reduction of urethral pressure variations, ranging from 8 to 42 cm H2O (mean: 21.9 cm H2O) and static urethral pressures, ranging from 47 to 102 cm H2O (mean: 68.8 cm H2O). Treatment was well tolerated in every case. CONCLUSION: These preliminary results need to be completed by a randomized placebo-controlled study to confirm a statistically significant effect of Moxisylyte hydrochloride on urethral pressure stability in women presenting with urethral instability.

[The war victim].

Just as the concept of war itself, the concept of the war victim is progressive, necessitating legal, economic, social, sanitary, ethical and political adaptations. In France, the laws of 1919, effective from 2nd August 1914, brought radical reform as laws of public solidarity, which guaranteed by the nation, the support of invalids of the most savage war in history. The collective nature of this new social risk obliged the state to replace a purely financial compensation by a solution of rehabilitation. The "Office National des Mutilés et Réformés", created in March 1916, was put in charge of the organisation of professional reeducation. The "war invalids" category was being transform a logic of assistance into one of social action. Later, the legislative structure made extensions, enlarging the beneficiaries in the "war victim" category. The "Service de Santé des Armées" in its basic mission of support to the armed forces covers many areas. The "Anciens Combattants et Victimes de Guerre" administration disposes of specific instruments, such as the "Institution Nationale des Invalides", the "Centre d'Etudes et de Recherche sur l'Appareillage des Handicapés", the "Office National des Anciens Combatants". These joint actions, added to the ones of very influential autonomous associations, contribute to give handicapped war victims an honourable citizenship.

[A Polynesian case of sideropenic dysphagia].

This first case has been found in a polynesian woman, a casual finding of the above condition during a whole heart decompensation. Clinical, biological, endoscopic and therapeutic aspects of Plummer-Vinson syndrome are reviewed.

[Medical aspects of hepatic amebiasis in Yaoundé (apropos of 23 cases)].

The authors report a short but homogenous group of 23 cases (15 men and 8 women) of hepatic amebiasis in Yaoundé (Cameroun), all of them confirmed by immunology. They state 2 isolated fevers, 4 jaundices. Nearly all cases were treated by tinidazole with very fast and evident success. One patient was operated and died. The great interest of immunology is underlined for the diagnosis and we wish its generalization, particularly in tropical countries.

[Idiopathic mega-oesophagus with an "historic" evolution (author's transl)].

Report of an african case of idiopathic mega-oesophagus observed after 6 years of evolution without any treatment. Surgery (Heller technic) gave a good result and brought evidence of a non-expected peritoneal tuberculosis related to cachexy. The patient having discarded all medical follow-up, death occured 2 years later.
