A transmission electron microscopical study of dysplastic naevi.

In this study those features of naevi that fulfil the clinical and light microscopical criteria of dysplastic naevi have been further examined with transmission electron microscopy. The results have been compared with the structure of normal control skin and compound naevi. In dysplastic naevi most melanosomes were abnormal, with spherical melansomes, an incomplete inner structure and uneven melanin deposit, cigar-shaped melanosomes and macromelanosomes. The intraepidermal border between the nests of dysplastic naevi were uneven and the dysplastic melanocytes extended their cell bodies among surrounding keratinocytes with a tendency to invade the epidermis in an upward direction. These findings will serve as additional criteria for dysplastic naevi.

The effect of grenz ray therapy on pustulosis palmoplantaris. A double-blind bilateral trial.

The effect of grenz ray therapy in the treatment of pustulosis palmoplantaris was assessed in 15 patients by randomly allocating active treatment of the lesions of one side of the body, while the lesions on the other side, which received stimulated therapy, served as a control. Four Gy of grenz rays 10 kV were applied on 6 occasions at intervals of 1 week. A significantly better therapeutic result was recorded on the lesions which had received active grenz ray therapy. However, the therapeutic response was moderate. It is concluded that grenz ray therapy could be useful in pustulosis palmoplantaris mainly as an adjunct to other therapies.

Immediate pigment-darkening reaction.

The immediate pigment-darkening reaction (IPD) is a gray-brown discoloration of the skin induced mainly by long-wave ultraviolet (UVA) radiation. The threshold value of IPD can be used as an important parameter in a series of light tests. The principle mechanism of the reaction is a photo-oxidative process causing darkening of preformed melanin and the production of free radicals. It has also been suggested that the reaction involves micromorphological changes in the melanocytes and a changed distribution pattern of melanosomes in the keratinocytes. This concept has recently been questioned by independent investigators. The physiological function of IPD still remains largely unknown. No photoprotective effect of the reaction has been observed. It has been suggested that IPD might have been of value during human evolution.

The effect of high dose long-wave ultraviolet radiation (UVA) on epidermal melanocytes in human skin: a transmission electron microscopic study.

Five healthy subjects with skin type III and IPD (immediate pigment darkening reaction) were irradiated with a glass-filtered UVA-sun 2000 lamp and exposed to 50, 100 and 150 J cm-2 of UVA. Transmission electron microscopy showed a statistically significant increase in the number of epidermal melanocytes with enlarged Golgi apparatus with associated vesicles, predominantly of the coated variant, swollen mitochondria with ruptured cristae, and extensive endo/exocytotic activity. Cells with 2 nuclei were also observed. It is concluded that UVA alone, in doses producing erythema, can give rise to these ultrastructural changes, interpreted as an increased secretory activity in a portion of the epidermal melanocytes.

A qualitative and quantitative transmission electronmicroscopic study of the immediate pigment darkening reaction.

Five healthy Caucasians, 3 having skin type II and 2 having skin type III, were UVA irradiated up to 14.5 J/cm2. Punch biopsies were taken before and immediately after irradiation and processed for transmission electron microscopy. Qualitative observations of the keratinocytes were obtained from the 5 subjects, while morphometry was performed on 1 subject with immediate pigment darkening reaction (IPD) (skin type III) and 1 without IPD (skin type II). The number of preformed melanosomes was 10-fold higher in the melanocytes and keratinocytes of the subject showing IPD as compared with the one who did not. No statistically significant difference, however, was observed in either skin type regarding the number of mature (stage IV) melanosomes of basal keratinocytes or melanocytes, size of melanosomes or redistribution of melanosomes in keratinocytes after UVA irradiation. No changes of the ultrastructure in the keratinocytes could be observed. It is concluded that the IPD consists of a photochemical reaction in preformed melanosomes and that the absence of IPD in skin type II could be due to the presence of too few preformed melanosomes.

The immediate action of long-wave ultraviolet radiation (UVA) on suprabasal melanocytes in human skin: a transmission electron microscopical study.

The immediate effects of long-wave ultraviolet irradiation (UVA) in non-erythemal doses on suprabasal melanocytes in normal skin of healthy caucasians with skin types II and III was investigated with the transmission electron microscopy (TEM) in order to detect possible morphological changes. In skin type III the melanocytes remained essentially unaltered, but in skin type II, multiple pinocytotic vesicles, large vacuoles, swelling, and partial to total dissolution of the inner membranes of the mitochondria and numerous small vesicles associated with an enlarged Golgi apparatus were observed after 3.6 to 14.5 J/cm2 of UVA irradiation. No IPD reaction was observed in any of the subjects with skin type II irradiated with UVA up to 20 J/cm2.

Further evidence for increased light sensitivity in patients with malignant melanoma.

Minimal erythema dose (MED), immediate pigment darkening (IPD) and skin typing were performed on 130 patients with malignant melanoma and on seventy-three controls. The tumours were histologically classified according to Clark as superficial spreading melanoma, nodular melanoma, and lentigo maligna melanoma. The tumours of twelve patients were not possible to classify. Patients belonging to skin type IV and patients with lentigo maligna melanoma had a significantly higher mean age. The IPD reaction was significantly higher in skin type II than in skin types III and IV both in patients and in controls. IPD and MED showed no significant correlation to age and sex. MED was significantly lower among patients with malignant melanoma than controls. These data indicate that light sensitivity is of aetiological importance in malignant melanoma.

Prostaglandin E1 treatment of leg ulcers caused by venous or arterial incompetence.

An open and a double-blind study were undertaken on patients with leg ulcers caused mainly by venous (VI) or arterial incompetence (AI). They were treated with intravenous infusions or injections of prostaglandin E1 (PGE1). Eight of 10 patients in the open study experienced relief of pain and a complete or almost complete healing of their ulcers. In the double-blind study (20 patients) 4 out of 5 patients with a history of leg ulcers due to VI for more than 5 year responded to the PGE1 treatment, compared with one of 5 treated with saline. In the saline group 3 more patients with VI of shorter duration improved. In 3 of 5 patients of PGE1 with ulcers due to AI the original ulcer area was reduced by 78--65% after 70 days, while in the 2 remaining cases healing occurred later on. No effect was noted in the 2 patients with ulcers due to AI who received saline infusions. The results indicate the beneficial effect of PGE1 on pain and healing in leg ulcers caused by peripheral vascular disease.

Clinical aspects of the immediate pigment darkening (IPD) reaction in normal individuals.

In order to investigate the clinical characteristics of the immediate pigment darkening (IPD) reaction for normal healthy Caucasian individuals and its conceivable relation to age, sex, skin type and to the minimal erythema dose (MED), a standardized light testing procedure was used and 72 subjects were studied. Skin type III was the most common, being found in 78% of the individuals; skin types II and IV were less common, found in only 12.5% and 9.5% respectively. Skin type I was not observed at all. Individuals with skin type II were found to have a significantly lower mean MED than those with skin types III and IV, and the IPD threshold dose was 9.0 J/cm2 or more, which was significantly more than for skin types III and IV. The IPD reaction was absent in several individuals with skin type II under the experimental conditions used. Some differences were observed concerning the mean MED and IPD threshold doses between skin types III and IV but they were not statistically significant. Age or sex did not appear to be significantly related to the MED and IPD threshold doses observed, and, furthermore, no significant correlation, either positive or negative, was found between the MED and the IPD threshold doses when investigated for individuals with skin type III.

Expression of p21WAF1/CIP1, p53, bcl-2 and Ki-67 in venous leg ulcers with and without squamous cell carcinoma.

In order to gain understanding of the cellular mechanisms of malignant transformation in chronic venous leg ulcers, we analysed by immunohistochemistry the presence of p21, p53, bcl-2 and Ki-67 in ulcers with and without squamous cell carcinoma. The material consisted of 41 archived histopathological samples from 33 patients with chronic venous leg ulcers and 28 samples from 21 patients suffering from squamous cell carcinoma in chronic venous leg ulcers. All samples derived from the chronic ulcers were negative for p53, p21 and bcl-2. Cells positive for Ki-67 were seen in certain ulcer areas. In the cancers, 14 samples showed immunopositivity for p53 and 22 samples were positive for p21, with expression mainly in the suprabasal layer. Expression of p21 seemed independent of p53. None of the samples was bcl-2-positive. Ki-67 was positive with basal expression in 16/28 of the tumour samples, 15 of which also expressed p21. There was no perilesional p53 or p21 activity in the cancer samples. The results regarding perilesional expression are different from those reported in UV-induced squamous cell carcinomas and probably reflect a different carcinogen.
