[Serum uric acid as a prognostic parameter in prenatal monitoring of gestoses].

A correlation between gestosis and increase of serum uric acid level has been known for a longtime. From the 6th month of pregnancy we found significantly increased values in all hypertensive pregnant women. In the 7th and 8th month of pregnancy there was a high significant deviation in gestoses second degree (classification of the organisation gestosis). In the two last months of pregnancy all gestoses deviated high significantly from the uric acid level in serum of the normal collective. There is a correlation between pathologically increased mean uric acid values in the 9th and 10th lunar month and increased systolic and diastolic blood pressure values. The uric acid level in pregnant women with gestosis, whose illness or its consequences led to cesarean section, is significantly higher than in patients with normal delivery. In accordance with literature we could not find a limit above the fetus dies. In our opinion determination of plasma uric acid in late pregnant hypertensive women is one additional parameter calculating both maternal prognosis and fetal risk.

[Plasma aldosterone in pregnancy, during labor and in the newborn infant].

Plasma aldosterone levels of mother and child during pregnancy were measured by means of radio-immuno assay without chromatography. In cases of unconspicuous pregnancy average aldosterone concentrations were found to increase from 23.2 ng/100 ml in the first trimester to 37.2 ng/100 ml in the second trimester and further to 64.0 ng/100 ml in the third.--High values were recorded from mothers (71.9 ng/100 ml) after birth and from umbilical cord blood of vaginally delivered newborns (83.4 ng/100 ml).--Aldosterone values as low as 60.9 ng/100 ml were recorded from newborns delivered by caesarean section with reduced parturitional stress. These were lower with significance than the levels recordable from newborns after spontaneous vaginal delivery (83.4 ng/100 ml). Values lower with significance were recorded from plasma of pre-eclamptic women in the third trimester (41.9 ng/100 ml).--No difference was found to exist between aldosterone concentrations in umbilico-arterial blood, on the one hand, and those in umbilico-venous blood, on the other.

[Continuous transcutaneous measurement of pO2 in foetuses - critical appraisal of the method (author's transl)].

An attempt is made for critical assessment of the method of continuous intrapartum transcutaneous measurement of pO2 in foetuses and its importance to research and practice, with reference being made to the authors' own experience and to evaluation of data published elsewhere. - The method has stood its test when used on newborns but proved to be quite problematic when applied to foetuses. Safe application of the electrode, pressure by surrounding tissue on the electrode, and pressure on the measuring point (variation of O2 diffusion path and circulatory disorders) were found to be the major problems. Feigned foetal oxygen curves may be produced even from erroneously applied electrodes. Very critical evaluation of the curves is, therefore, necessary, and false representation of foetal O2 supply cannot be ruled out. - Transcutaneous O2 measurement is used in research and can provide valuable information on physiological and pathological developments. It can be a useful complementary technique to cardiotocography. The method, in its present form, is of little clinical usefulness to the obstetric practitioner.

[Continuous transcutaneous pO2-criterion of newborn condition, depending on obstetric circumstances (author's transl)].

Arterial pO2 was transcutaneously recorded under standardized conditions from 55 newborns, right after birth. Two values, pO2 with air breathing 30 minutes after birth and the pO2 peak following oxygen breathing, were compared, with reference being made to their dependence on obstetric conditions. Significant deterioration of oxygen supply, as compared to a control group, was recordable from high-risk births, infants delivered from breech presentation by the vaginal route, and from newborns with neonatal acidosis. Oxygen supply to surgically delivered high-risk infants came very close to that recorded from normally born children, while that established from infants born by caesarean section was in agreement with values of hyperoxia. High-continuity measurement of pO2, immediately after childbirth, can be used as a criterion by which to assess obstetric performance. It can be also an objective parameter for an assessment of postpartum adaptation, and a tool by which to figure out endangered newborns.

Study of plasma aldosterone in normal pregnancy, in pre-eclamptic women and in cord plasma of newborns.

A radioimmunoassay without chromatography was used for the determination of plasma aldosterone in pregnancy. The mean values (+/- S.D.) of aldosterone concentration increased consistently from 23.2 +/- 5.3 ng/100 ml (n = 14) during the first trimester to 37.2 +/- 10.6 ng/100 ml (n = 17) during the second trimester and 64.0 +/- 18.8 ng/100 ml (n = 29) during the third trimester of pregnancy. The highest values were found at delivery (71.9 +/- 14.2 ng/100 ml; n = 21) and in the cord plasma of newborns (83.4 +/- 14.9 ng/100 ml; n = 21). Significantly lower plasma aldosterone values were found in the plasma of pre-eclamptic women during the third trimester of pregnancy (41.9 +/- 21.3 ng/100 ml; n = 11).

[Extreme antepartum fetal tachycardia].

On the occasion of a case with fetal paroxysmal tachykardia a synopsis is given on former observations of extreme fetal tachycardias ante-partum. Here is our conclusion: Even extreme and long lasting tachycardias alone are not an indication of operative delivery.

[pH-metry in vivo--use in obstetrics].

A new equipment and technique in order to analyse the pH in vivo will be presented. The special rod-shaped probe contains an ISFET pH sensor. There is pointed out a wide field of applications in the obstetrics.
