Embolization of cardiac myxomas masquerading as polyarteritis nodosa.

Cardiac myxomas are rare tumors which can mimic other diseases. We describe a 19-year-old man who presented clinically with polyarteritis nodosa (PAN). Muscle biopsy showed vasculitis, but corticosteroid and immunosuppressive therapy was initially effective. Cardiac myxomas were found by echocardiogram when the disease relapsed and were resected surgically. Segmental arterial aneurysms and stenoses were found on angiography after leg ischemia. We suggest that echocardiography should be performed in all cases of clinically suspected PAN.

[Membranoproliferative glomerulonephritis in 2 brothers].

In one sibship, two brothers had membrano-proliferative glomerulonephritis type I. The former died 3 years after the onset of the glomerulonephritis and the latter is still alive. The familial nature of the disease is an argument to strengthen the concept that genetic factors are involved in its origin.

[Atypical metastases disclosing vesicular carcinoma of the thyroid].

Differentiated carcinoma of the thyroid may generate bone or lung metastases. Some times, thyroid cancer remains latent, and is revealed by the presence of secondary bone tumors. In the present paper, we relate our personal experiences with two patients who had thyroid cancer diagnosed indirectly through the occurrence of distant metastases. The secondary tumors were localized in the right frontal sinus and the cranial vault, respectively. We discuss the diagnostic and therapeutic problems involved with such metastases. We further present a review of the literature dealing with this topic.

[Continuous ambulatory peritoneal dialysis in Tunisia].

CAPD covers about 4% of renal replacement therapy patients in Tunisia. From February 1983 to 1988, 47 patients, 21 males and 26 females whose mean age was 36, 14 +/- 15 were treated. Their socio-cultural level was variable. The mean duration of therapy was 14.5 months/patient/year. The five years patient survival rate was 46% and the technique survival rate 21%. Most patients had a good general condition and were professionally rehabilitated. Peritonitis was the main complication: its incidence was 1.8 episodes/patient/year. Yeasts peritonitis were relatively frequent, specially in patients who had previously been treated with antibiotics. Peritonitis was often cured with a local and general treatment. Yearly CAPD cost was 1/3 lower than center hemodialysis and a better management allowed to decrease the expenses due to laboratory and X ray investigations and to hospitalization. On the basis of this clinical experience, we believe that CAPD is an efficient and valid therapeutic modality, less expensive than hemodialysis. It is probably a suitable renal replacement therapy in other centers in Tunisia and in other developing countries.

[The first thirty months of kidney transplantation in Tunisia].

Kidney transplantation is actually the best replacement therapy for the end stage renal failure. It sets free the hemodialysed patient from the hemodialysis restraint and contributes to solve the socio-economic problems risen by chronic hemodialysis. The authors report the results of this technic during the first 30 months of kidney transplantation in the "Hôpital Charles Nicolle" of Tunis. They describe the first steps which led to kidney transplantation, the therapeutic regimens, the medico-legal problems and the specific complications observed during this start period.

Costs and Cost Analysis

[Intraorbital involvement in multiple myeloma].

According to data from the literature, intraorbital involvement in multiple myeloma is rare. Such involvement may result in exophthalmia, itself the first manifestation and presenting feature of myeloma. These lesions can respond remarkably to radiotherapy. The authors present a new case report.

[Involvement of the cranial nerves disclosing multiple myeloma].

Diseases of cranial nerves are very seldom associated with multiple myeloma. However, their occurrence as a harbinger of multiple myeloma seems to bear a particular significance and is worth mentioning. In the present paper, we discuss two cases in which disorders of cranial nerves heralded Kahler's disease. The patients presented with posterior laterocondylar space and cochleovestibular (otoliquorrhea) syndromes, respectively. We stress the diagnostic problems involved and analyze the pathogenesis of diseases of the nervous system associated with multiple myeloma. Diseases of cranial nerves are seldom associated with multiple myeloma. Nonetheless, the clinical setting and laboratory tests will direct the diagnosis. The same does not hold true when one is confronted with Collet-Sicard's syndrome or cochleovestibular syndrome accompanied by otoliquorrhea.

[Antibodies against extractable nuclear antigens. Clinico-biological correlations in 100 cases].

The clinical significance of antibodies directed against extractable nuclear antigens with currently well-known antigenic targets, is studied in 100 patients affected with various connective tissue inflammations. The anti-Sm is the most frequently encountered antibody in lupus disease and overlapping connective tissue inflammations with a lupus component while the anti-RNP is related to mixed inflammation of the connective tissue and the dry syndrome. These antibodies, without being specific markers for a given disease, represent an additional biological argument for a better nosological classification of connective tissue inflammations. The use of immuno-impression, a more sensitive technique than immunodiffusion, could provide a better understanding of the immune response during these diseases and specify the diagnostic and prognostic value of various auto-antibodies. Analysis by immuno-impression of 36 sera among the 100 with antibodies directed against extractable nuclear antigens has enabled us to compare the two techniques and study the different specificities of these antibodies.

Diabetes mellitus in Tunisia: description in urban and rural populations.

A prevalence survey of diabetes mellitus was carried out in Tunisia on two random samples of households. The first sample (3826 adult subjects) was drawn from the Gouvernorat of Tunis, the second one (1787 adult subjects), was drawn from a rural area, the Gouvernorat of Siliana. The families were investigated at home and diabetes assessed on the basis of an interview (to determine known cases) and of fasting blood glucose level in subjects having no personal history of diabetes (new cases). Prevalence rates were estimated considering known cases and newly found ones together. Overall, the age-standardized prevalence rate was found to be much higher in the urban sample compared to the rural one, especially for women (4.6% versus 2.3% in men, 3.5% versus 0.6% in women). Diabetes was often associated with obesity, especially in men. Within the urban sample, the prevalence rate was similar in subjects born in Tunis and in those born in the rest of the country, thus mainly of rural extraction. In contrast, a family history of diabetes was more often reported in the former group. The results are consistent with other epidemiological findings, showing that a dramatic increase in diabetes morbidity parallels the rapid westernization of urban centres in developing countries.
