[Value of ultrasonic diagnosis in the evaluation of hip dislocation in neonates and infants. Apropos of 340 cases].

Ultrasound imaging of hip (UIH) was performed in 170 children (340 hips) with normal (193) and dysplasic (147) hips and findings compared with results of clinical and radiologic examinations. Ultrasound demonstrated 100% sensitivity in cases with abnormal clinical and radiologic finding, and 94% specificity for UIH when clinical and radiologic examinations were negative. Sensitivity and specificity of ultrasound screening (respectively 96 and 81%) were comparatively superior to those of radiography of the pelvis (83 and 78%) in relation to the clinical examinations. These findings predict further development of UIH for screening of congenital dislocation of the hips and for follow up surveillance of treated children.

[Idiopathic severe valgus deformity of the knee in children. Apropos of 2 cases].

The authors have reviewed two cases of important and idiopathic deformity in children coming from Africa. In the third year of life, a valgus deformity of a knee occurs spontaneously and increases rapidly in a previously healthy child. Varus deformity of the other knee follows but seems to be just a mechanical consequence of the valgus deformity. The pathological lesions observed radiologically concern mainly the metaphyseses. General skeletal or metabolic disease, malnutrition or sickle-cell disease are not found.

[Pure epiphyseal detachment of the upper end of the femur in a young child: apropos of 2 cases].

Pure epiphysial detachment following injury to upper end of femur in two 2 year-old children was treated orthopedically by manual reduction under general anesthesia and plaster immobilization. Follow up radiographic and scintigraphic examinations showed favorable results with absence of necrosis after 10 and 17 months respectively. In contrast, long term surveillance will be necessary because of the rapid appearance of coxa magna.

Chiari's pelvic osteotomy in the treatment of Legg-Calvé-Perthes disease.

Results of Chiari's pelvic osteotomy in Legg-Calvé-Perthes disease were analyzed in 37 hips of 36 patients. The mean age at surgery was nine years. The results were assessed according to both the quality of sphericity and containment of the femoral head. The results were satisfactory in 26 hips (70%), acceptable in 10 hips (27%), and poor in only one hip (3%). In cases where preoperative arthrography revealed a saddle-shaped distortion of the protruding and flattened epiphysis, the results were less satisfactory. The authors conclude that Chiari's osteotomy is needed where other kinds of containment cannot be expected to be successful. The indications are smooth flattenings of severely protruding epiphyses in patients aged seven years or more, and severe epiphyseal protrusions without flattening in patients 10 years or older.

[Orthopedic aspects of congenital insensitivity to pain].

The congenital insensitivity to pain regroups some rare diseases which are mainly 5: congenital insensitivity to pain; congenital sensitive neuropathies; distal sensitive neuropathies; Riley-Day syndrome or hereditary dysautonomia; at last, miscellaneous troubles. Three different cases are reported in children: true congenital insensitivity to pain; hereditary dysautonomia or Riley-Day syndrome; congenital insensitivity to pain localised to a lower limb joined to amniotic disease and abnormality of this limb. The orthopedic symptoms (osteomyelitis, arthropathies as Charcot type, dislocations, fractures) lead often to diagnosis and they are an important step of the prognosis. Scoliosis seems to be frequent in this disease. The orthopedic and surgical treatment, according to each localization, is difficult and must emphasize the prevention of bones and joints injuries.

Atypical bone angiomas in children.

Bone angiomas can present in a variety of clinical, radiological and histological forms. The two cases reported illustrate unusual and fairly rare aspects of this vascular tumor.

[Arthrography in pathology of the hip in older children].

Arthrography of the hip was performed in outpatients on 25 children aged 5 to 14 years, using a supra-trochanteric latero-external puncture approach. Data obtained supplied anatomical details indispensable for the decision to operate or not, particularly in certain congenital dislocations or in Legg-Perthes-Calvé disease. The examination findings were useful not only for analyzing articular cartilages, limbus and chondro-articular surfaces in abduction and adduction but also for evaluating anteversion of bony and cartilaginous acetabula using an axial projection. The scanner provides comparable data with respect to the latter feature, but access is still more difficult.

[Therapeutic remarks on the congenital elevation of the scapula. Apropos of 16 cases].

The authors review a series of 16 cases of congenital high scapula including 4 bilateral cases. The pathological aspects are detailed including investigations for scapular hypoplasia (3 cases), omovertebral bone (7 cases), loco-regional muscular anomalies (5 cases), and various deformities of the cervical spine (present in all cases). The entire spine and the thoracic cage was examined in all cases. The Woodward procedure was performed with some modifications described in the text. The operative results in 3 cases were judged to be failure based on cosmetic and functional criteria. The age of the patients at operation and the presence of an omovertebral bone did not influence the results, however, the presence of important deformities of the cervical spine had a poor prognosis. The authors insist on the loco-regional character of the deformity and on the importance of the reorientation of the scapulo-humeral joint. Results were judged on cosmetic and functional criteria with 79% excellent or fair results.
