Noncontact detection of ocular melanoma with L-3-123I-iodo-alpha-methyltyrosine: first clinical results.

The results of the first clinical trial are presented of the newly designed tumor-seeking radiopharmaceutical L-3-123I-iodo-alpha-methyltyrosine (IMT) for noncontact detection of malignant eye melanoma by means of a nuclear medicine technique. Eye scintigraphy (ES) was performed on ten outpatients clinically suspected of harboring an intraocular melanoma. ES was carried out with a twin pinhole eye collimator attached to a high resolution gamma camera; 7-10 mCi of IMT was given intravenously to each patient 100-150 min prior to the nuclear medicine investigation. Gamma camera eye scintigraphy and the use of IMT offer a useful noninvasive alternative to the radiophosphorus uptake test. Of the ten patients, five were found not to have a melanoma either clinically or by scintigraphy, three had a melanoma by scintigraphy that was confirmed histologically, and two yielded inconclusive results.

L-3-123I-alpha-methyltyrosine for melanoma detection: a comparative evaluation.

L-3-123I-alpha-methyltyrosine (IMT) was compared to [67Ga]citrate, [203Pb]Tris and [131I]iodochloroquine (ICQ) with respect to their potential as melanoma seeking radiopharmaceuticals in two tumor lines, a malignant melanotic melanoma of the Sofia type and a malignant amelanotic melanoma of the Greene-Harvey type, transplanted onto Syrian Golden hamsters. [203Pb]Tris and ICQ showed significant accumulation only in melanotic melanoma. In contrast, [67Ga]citrate and IMT accumulated in both tumor lines. [67Ga]citrate has very high tumor-to-skin, tumor-to-eye, and tumor-to-blood ratios at 48 h after injection, but has low selectivity for melanoma and unfavorable physical (high gamma-energy) and biological (half-time) characteristics. IMT has a convenient gamma-energy at 159 keV and a short biological half-time. Maximum of melanoma accumulation is reached at 1-2h after application; tumor-to-tissue ratios are suitable for scintigraphy.

Noncontact detection of experimental amelanotic ocular melanoma with L-3-123I-iodo-alpha-methyltyrosine.

An application of L-3-123I-iodo-alpha-methyltyrosine for the detection of ocular melanoma is presented. In rabbits with malignant Greene amelanotic melanoma transplanted to the anterior chamber of the eye, all tumors were detected in positive contrast using a gamma camera with a single pinhole collimator. The smallest tumor identified weighed 65 mg. Our data are compared with those obtained with 32PO4, 67Ga, 203Pb or labelled iodochloroquine.

[Noncontact detection of intraocular lesions by means of a gamma scintillation camera].

A gamma scintillation camera is used for noncontact detection of intraocular mass lesions. A double aperture pinhole collimator was designed for this special application. The collimator projects a magnified image of the radioactivity present in each eye onto the detector crystal of the camera. Details of the collimator design are described. The image-making properties of a double aperture pinhole collimator for ocular scintigraphy in regard to resolution and response were studied and demonstrated by phantom experiments. So far we have studied 50 cases with this new nuclear medicine procedure, using mainly technetium-pertechnetate as the radioindicator. Case histories and records of some typical results are presented. Better designed radiopharmaceuticals with appropriate tumor-seeking properties should be developed. Labeled melanin precursors are promising for detection of these lesions.

[Diagnosis of breast carcinoma. Value of x-ray findings for the diagnosis of carcinoma in situ of the breast from the clinical point of view].

The x-rays-results of 35 mammae with a carcinoma in situ are presented and compared with those of 2 groups without carcinoma. In contradiction to invasive malignancies the carcinoma in situ do not show pathognomonic signs on x-ray examination. However, suspicion is raised by the appearance of microcalcifications and by striped differences in tissue density. These findings should compell to perform a biopsy, even though the clinical and roentgenologic findings may otherwise suggest a fibrocystic disease. Therefore, provided the discrete signs described in this paper are carefully considered, the technique of mammography may contribute to the pre-operative presumptive diagnosis of carcinoma in situ.

[The effect of contrast media containing iodine on selected in vitro thyroid test procedures (author's transl)].

The effect of 6 commonly used contrast media on the results of 3 different methods for determination of thyroid hormone concentration in the serum is examined in vivo. T4 determination by column chromatography proved to be very liable to distortion for contrast media. With some contrast media the Res-O-Mat-ETR test proved unreliable. T4 determination by means of protein binding analysis is not affected by contrast media in the direction of false raised values.

Halothane on irradiated liver tissue.

The results of animal experiments show that distinction must be made between acute and chronic tissue changes produced by irradiation of the liver. The pathologico-morphological findings obtained by irradiation of the liver combined with repeated halothane administration during short-term experiments are scanty and dosage-dependent. Long-term observation has revealed higher-dosage irradiation to be followed by minor inflammation which, if combined with regular halothane administration, progresses to produce the patho-anatomical picture of chronic hepatitis with diffuse cell damage and focal necrosis even in those groups of animals which had been exposed to low-dosage irradiation. It must be assumed that even low-dosage irradiation of the liver which fails to produce measurable changes at the beginning, may influence the structure and function of the hepatic parenchyma. The latent radiation damage manifests itself under additional toxic irritation of the liver, such as that caused by halothane.
