BACKGROUND: Malignant tumors of the retinal pigment epithelium (RPE) are exceedingly rare. We describe the histopathologic and immunohistochemical features of a RPE neoplasm that was found accidentally in a blind and painful phthisical eye. METHODS: The enucleated eye was investigated by light microscopy, and tumor tissue was also studied by electron microscopy. Immunohistochemistry was performed using antibodies against HMB-45, S-100 protein, NSE, cytokeratins, vimentin, desmin, GFAP, the HNK-1 carbohydrate epitope and alpha-smooth muscle actin. RESULTS: The tumor was located mainly in the vitreous cavity with practically complete destruction of the retina, but foci of choroidal infiltration and extraocular extension along vascular channels were identified. The mitotic rate was high, and large areas of necrosis were present. No features of differentiation were seen, apart from occasional desmosome-like junctions and deposition of basal lamina at the ultrastructural level. Adjacent to the tumor, reactive hyperplasia and metaplasia of RPE cells was prominent. By immunohistochemistry, tumor cells revealed intense immunoreactivity with anti-vimentin and weak staining with anti-S-100 protein. The hyperplastic RPE cells also reacted for cytokeratins 8, 18 and 19 and for alpha-smooth muscle actin. At more than 1 year post enucleation the patient is well and shows no signs of recurrence or metastatic disease. CONCLUSION: We present the features of a malignant tumor of the RPE with unequivocal extraocular extension. These findings raise the possibility that RPE hyperplasia may transform into a malignant tumor.
Cimetidine inhibits hepatic drug metabolism, but its effect on ethanol elimination is less well established. The influence of cimetidine on the kinetics of ethanol is studied with three standardized drinking experiments. Cimetidine had no influence on the absorption or elimination of ethanol. There were significant differences in distribution of ethanol in consequence of inhibition of vascular histamine receptors after cimetidine application.
It is reported on a case of lyme-borreliosis with cardiac participation. Here the case in question were disturbances of the transmission of impulses in form of a third-degree atrioventricular block, which temporarily underwent a cardiac pacemaker treatment. By means of the antibiotic treatment recommended in the literature a restitutio ad integrum was achieved.
The possible significance of subconjunctival bleedings (hyposphagmas) as a symptom for other pathological changes of the eye was studied in a group of 354 postpartum patients. They were found in 10.5% of the cases. A relatively higher frequency occurred in primiparas. No other maternal factors than parity were detected as of statistically significant influence. A slightly higher percentage in mothers with large newborn (above average cranial circumference) indicates a relationship to the necessary bearing-down effort. In a second study, 49 postpartum women with subconjunctival bleeding were examined by an ophthalmologist. Neither any intraocular or retrobulbar bleeding nor any other grievous damage to the eye were found. In accordance with the literature we recommend, that highly myopic pregnant women (even with a previous history of retinal detachment, but ophthalmologically sufficiently treated) should undergo induced delivery. However, the approach to pregnant women with retinal detachment occurring and treated during pregnancy, or to glaucoma-patients with risk to a small remaining visual field, should be more careful. In those cases, a vaginal operative delivery procedure (forceps, vacuum-extraction) is recommended. Nevertheless, one should consider individual wishes of the mother, when striving to diminish the risk to the foetus. The same applies to irrational fears related to previously damaged (and possibly only functional) eye.
Blood diseases such as acute leukemia can now be cured by bone marrow transplants. Complications such as a chronic host-versus-graft reaction lead to typical conjunctival changes, as described for Sjögren's syndrome and ocular pemphigoid. A patient with typical eye complications is described and the probable immunological course discussed.

Permalens contact lenses for extended wear offer further possibilities for correcting aphakic eyes. In some cases they may constitute an alternative to intraocular lenses. Preliminary results of a long-term study are presented.
A portable instrument for examination of the anterior segment and of the fundus is described. With the slitlamp-microscope examinations of the cornea, anterior chamber, iris and lens can be performed which are comparable to those carried out with the office slitlamp. The instrument also serves as a monocular or binocular indirect ophthalmoscope for fundus examinations.

Follow-up studies of diabetic retinopathy cause difficulties through the altering numbers and figures of the retinal changes. If the various pictures of retinopathy are changed furthermore by coagulation, we have to distinguish, between spontaneous improvements or deteriorations and changes which are due to therapy. Follow-up studies with various evaluation methods have shown that the chosen criteria do not lead to corresponding judgements. The visual acuity seems to be the only criteria with decisive and relatively objective statements.
The development of a new vitrectomy instrument has contributed to advancement in vitreous surgery to a large extent. The principle of this instrument is the suction of the vitreous into a needle and its simultaneous resection. In the meantime several constructions have been described. The resection apparatus, observation possibilities and illumination are discussed.
