[Serous (microcystic) pancreatic adenoma].

Microcystic pancreatic adenoma is an extremely rare tumor, arising in exocrine pancreatic tissue, which may lead to severe abdominal pain. To date malignant transformation has not been reported. There is a conspicuous, statistically significant, association of microcystic pancreatic adenoma with various other diseases. This report describes a 57 year-old woman with associated clinical signs of scleroderma. Therapeutic modalities are primarily aimed at complete removal of the tumor, which may--depending on the localisation--require major pancreatic resections.

Hepatotoxicity of acetaldehyde in rats.

The ability of acetaldehyde to initiate hepatotoxicity as evidenced by enzyme leakage, hepatic fat accumulation and histological alterations was studied in rats. Neither oral nor intraperitoneal treatment with acetaldehyde had any hepatotoxic effect, even following aldehyde dehydrogenase inhibition by disulfiram. This is probably due to the inability of exogenously added acetaldehyde to penetrate liver cell membranes. In contrast, acetaldehyde derived metabolically from ethanol was capable of inducing moderate hepatotoxicity when it accumulated upon pretreatment with disulfiram. Acetaldehyde may thus be partly responsible for alcohol-induced liver damage.

[Benign and malignant breast tumors with squamous cell differentiation].

Three epidermal tumors with squamous celled differentiation are described. Two appeared as epidermoid cysts, the other was a squamous cell carcinoma in a 100 year old female patient. Epidermoid cysts of the breast are rare compared to lesions occurring in other regions of the human body. They are important, since malignant transformation is possible in 0.5 to 19 per cent of the patients. The mode of origin of squamous cell carcinomas is either metaplastic change in duct epithelium of the nipple or transformation of fibroadenomas or other, malignant tumors. Preoperative differentiation between benign and malignant epithelial cysts is almost impossible. Therefore local excision is the best treatment, since benign cysts, treated only by needle aspiration usually recur. Malignant cysts are best treated by radical amputation, because irradiation and chemotherapy are ineffective in the treatment of squamous celled lesions.

[Radiation-induced scars in mammography].

Six patients with radiation scars are described. In each case the diagnosis was confirmed histologically in five cases corresponding mammograms were available. The histological appearances of radiation scars are described and the radiological features are presented. These lesions can be diagnosed mammographically in vivo. Macroscopically differentiation from a scirrhous carcinoma is not possible and therefore a radiation scar must always be excised; this also leads to definitive cure. On mammographic screening the incidence is 0.5 to 0.9 per thousand. The significance of radiation scars depends on the fact that they are pre-cancerous and therefore are equivalent to the early diagnosis of a carcinoma with the possibility of a complete cure.

Morphologic and cell kinetic investigations of the spleen after repeated in situ freezing of liver and kidney.

In order to evaluate a possible primary or secondary immunologic response of the spleen after single or repeated in situ freezing of parenchymal organs such as liver and kidney within a four week period, light microscopic and cell kinetic investigations with tritiated thymidine were performed on spleens of non-germfree rats. Sham operations served as controls. The sham operations did not induce any significant morphological or cell kinetic changes in the splenic white pulp. After cryolesions were produced in the liver and kidney, the percentages of activated germinal centers, labeled germinal center cells, and cells in the perifollicular area of the lymphatic mantle and marginal zones increased, with maxima during the first 3 days. The investigations show that the cellular reaction of the spleen starts earlier and is more prominent after repeated in situ freezing than after a single cryolesion. These findings point to an immunologic response of the anamnestic type, and correspond to results after repeated freezing of normal and malignant tissue of the urogenital tract. These cell kinetic results are important in the evaluation of further immunologic studies involving the cryotherapy of malignant tissues.

[A comparative morphological and enzyme-histochemical study of experimental pneumoconiosis and experimental allergic-hyperergic pulmonary granuloma (author's transl)].

Six endotracheal applications of 15 milligrams of cadmium sulfide (CdS) given to rats at weekly intervals induce pneumoconiosis with massive adhesions. The same quantities of lead sulfide (PbS) given under the same conditions have much less effect, causing only the formation of fairly small fibrous nodules. The tissue reactions to CdS and PbS were studied morphologically (light and electron microscope) and enzyme-histochemically. The results were compared with those observed in experimental anthracosilicosis in rats and with those seen in allergic-hyperergic reactions of the lung in guinea pigs (induced by several applications of the complete Freund adjuvans). A considerable increase in macrophages in the alveoli and interstices of the lungs with increased activity of acid phosphatase and oxydoreductases had occurred in every animal. None showed primary damage to type 2 pneumocytes. The results indicate that the two toxic agents differ in their capacity to induce fibrosis not because they interfere to a different degree with the clearing function of the lung or surfactant production, but because they differ in their capacity to stimulate fibroblast formation.

[Electronmicroscopic findings in a malignant hepatoma after oral contraceptives (author's transl)].

In this paper we report the electronmicroscopic findings in a hepatocellular carcinoma of a 24 years old woman after taking oral contraceptives. We saw alterations of the nucleus (finger-like or finger-formed nuclei, nuclear invaginations and intranuclear vesicular membranbodies), changes of the mitochondries (number, polymorphy and osmiophil bodies) as well as changes of lysosomes (myelinshaped residualbodies). The observed and described changes are not specific for malignant tumors of the liver.

[Diffuse segmental lipomatosis of the ileum (author's transl)].

Case of unusual diffuse segmental lipomatosis of a part of the small bowel is reported. Other cases from the literature are compared to various pathological deposits of fat in the intestine. Aetiology, clinical signs, diagnosis and differential diagnosis, as well as surgical treatment, are discussed.

[Morphologic changes of the renal arteries following instrumental compression (author's transl)].

In the present work, light-microscopic changes in the renal arteries of the rabbit following instrumental compression for specific intervals are described: 1. Compression led to necrosis of whole layers of the vascular walls, which could already be observed under the light microscope after 3 h.--2. The degree of wall necrosis depends only on the pressure of the clamp: not on the duration.--3. Experience shows that, independent of the type of clamp, there is complete regeneration of the adventitia and media by the fourth postoperative week.--4. In all vessels, the residual effect is a 2--5 layer proliferation of the intima that extends over the entire clamped area for 4--8 weeks postoperatively.--5. In 2 out of 80 renal arteries, arterial thromboses developed.

[Morphological findings on the brain of rats after local freezing (author's transl)].

In continuation of our investigations of the wound healing process after cryonecrosis in the liver, kidney, spleen and stomach, the wound healing in the brain was studied under the same condition. Through a trepanation of about 3 mm diameter, the cortex of the parietal region of the cerebrum was frozen by a cryoprobe applied directly through the intact dura mater (-196degreesC/30s). The animals were killed at definite time intervals between 12 h and 21 days after freezing. Twelve hours after freezing, the necrosis of the brain cortex is complete. It extends wedgeshaped into the subcortical white matter. The central parts of the necrosis become colliquative and are demarcted by leucocytes. Two days after freezing, the necrotic area is almost totally invaded by leucocytes. Three days p.op. the brain wound is infiltrated by microglial cells, later on the mesenchymal cell proliferation extends to the meninges. There is new capillary formation and partially a wall like proliferation of the perivascular connective tissue cells. Ten days and later, an uni- or multiloculated pseudocyst develops, which can be considered as the final stage of the reparative wound healing of the cryonecrosis. Similar as in the other investigated organs (liver, kidneys, spleen) these investigations underline the rapid wound healing of the cryonecrosis in the brain. The advantages of cryosurgical manipulations with the well known good wound healing process of other organs, justifies therefore the short term tissue freezing on the cerebrum.

Wound healing of the brain of rats after cryonecrosis. Autoradiographic investigations with 3H-thymidine.

Histologic and autoradiographic studies were performed to investigate the cellular reactions during wound healing of the brain of rats after cryonecrosis. A parietal lobe was frozen for 30 s with a cryoprobe at a temperature of -196 degrees C. The survival time ranged between 12 h and 21 days. One h before killing, tritiated thymidine was injected intraperitoneally. Stripping film autoradiograms of the brain sections were made in the usual manner. A typical cryonecrosis develops 12 h after local freezing surrounded by an edematous tissue layer with activated mesenchymal and neurologlial cells. After 10 days a pseudocyst develops surrounded by highly cellular glial tissue. The pseudocyst is still recognizable 3 weeks later but without any remarkable cellular reaction. Autoradiographically the labeling indices of the fibroblasts, leptomeningeal,and endothelial cells in the periphery of the necrosis and the labeling indices of neuroglial, perivascular connective tissue, and microglial cells in the perinecrotic zone increase after 12 h and have their maximum between the 2nd and 3rd postoperative day. In the nonfrozen contralateral cerebral hemispheres, the labeling indices of neuroglial and mesenchymal cells show small peaks in the first 3 postoperative days,also. This can be explained by accompanying edema. There are no remarkable differences between frozen and nonfrozen parts of the brain 3 weeks after local freezing. The results underline the rapid repair of cryonecrosis.

[Healing process of colonic anastomosis after contamination with Staphylococcus aureus (author's transl)].

In 30 rats the healing process of colonic anastomosis contaminated with Staphylococcus aureus was studied. At the same time on each animals a single- and double-layer anastomosis was performed. All sutures were infected during the surgical procedure with highly infectious S. aureus. The rats were sacrificed after 1, 2, 3, 4 days, and 2 1/2 weeks. Criteria for estimation were mainly visible abscess formations, leakage spots, bacteriologic and histologic examinations. After apparent inflammatory reactions were seen on the first postoperative days with a maximum on the 4th day, all sutures were completely healed and free of S. aureus after 2 1/2 weeks. As for the extent of infectious inflammation, the single-layer suture technique proved to be more resistant than the double layer.
