[Correlation between clinical status and psychological variables in juvenile diabetics (author's transl)].

Correlations between clinical status and psychological variables in 56 juvenile diabetics were studied. Intelligence was only partly associated with clinical status. Among personality variables extraversion proved to be correlated with insufficient clinical control. This result is significant, both from a theoretical and from a practical point of view. There were no correlations of parental personality and clinical control.

Adolescent

Juvenile type of generalized ceroid-lipofuscinosis (Spielmeyer-Sjögren syndrome) I. Clinical findings.

Thirteen patients with the clinical course of the juvenile type of generalized ceroid-lipofuscinosis were examined clinically, ophthalmologically, neurologically and psychiatrically. This included registration of EEG, x-ray, brain scintigraphy, motor nerve conduction velocity, ERG, fundus photography and bioptical investigations including electron microscopy. The children suffered from progressing decrease of visual acuity, decline of mental capacities, and later on speech and gait disturbances. The most characteristic findings are presented in case reports, two tables and fourteen pictures.

Adolescent

Juvenile type of generalized ceroid-lipofuscinosis (Spielmeyer-Sjögren syndrome1 II. Biopsy findings.

Rectal biopsies were obtained from 11 children, clinically presumed to suffer from the juvenile type of generalized ceroid-lipofuscinosis. In addition, sural nerve biopsies were performed in 4 cases. Of the various staining procedures applied for light microscopy, Sundan Black B was found to be the most useful, revealing in each of the rectal biopsies at least some nerve cells with coarse black droplets as well as numerous mucosal macrophages with a more greyish, dustlike material. In sural nerve biopsies a slight reduction of the thick myelinated nerve fibers was seen in two cases, accompanied by regeneration phenomena. The latter were also detectable in one other case, the remaining sural nerve biopsy being histologically unremarkable. By electron microscopy neuronal perikarya revealed lipopigment bodies with a fingerprint-like pattern in each of the rectal biopsies. In one case only was this pattern combined with distinct curvilinear profiles. In 4 cases, however, the fingerprint-like deposits were intermingled with different lamellated bodies, some of them reminiscent of MCB's or zebra bodies. In two of the 4 cases, these were even seen exclusively in some neurons. In each of the rectal as well as the sural nerve biopsies, most of the non-neuronal cell types presented lipopigment bodies, likewise, displaying both curvilinear and fingerpring-like profiles, with some characteristics related to the various cell types. With regard to generalized ceroid-lipofuscinosis the diagnostic value of rectal biopsies, though being disputed in the literature, appears reestablished by the findings presented.

Biopsy

[State of disease and social situation of adult hemophiliacs].

54 adult patients suffering from hemophilia responded to a questionnaire regarding their state of illness and social situation. As far as state of the illness was concerned items pertained to type of hemophilia, factor activity, degree of physical handicap, number of bleedings and hospitalizations as well as etiology of bleedings. Social data contained items on marital status, education, occupation and leisure activities. The handicap of the illness as well as the possibility of positive social integration is underlined by the findings of the study.

Persons with Disabilities

Intelligence structure and personality in various types of physical handicap in childhood and adolescence.

Intelligence structure and personality were assessed in a sample of 104 physically disabled children of normal intelligence (IQ is greater than or equal to 85). Findings were compared to those of healthy controls matched by age, sex, rank order and number of siblings and socioeconomic status. Physically handicapped children had lower scores in all subtests of a multi-factorial intelligence test. Furthermore five different subgroups of physically disabled children could be discriminated along one factor marked by different variables of visual perception. Personality of the total group of handicapped children was different when compared to normal controls. This specific personality pattern may be labeled: lack of emotional integration into social environment without conflict. Various subgroups of physically handicapped children differed only little as far as personality is concerned.

Adolescent

Psychoendocrinological studies in dwarfed children and adolescents.

Thirty-two dwarfed children and adolescents were studied clinically, by laboratory assessment, by a battery of psychological tests and a structured interview. Growth hormone deficiency was present in 16 cases, but in the remaining 16 cases there was no endocrine disease. Dwarfed children differed from nornal controls in perception, and a specific personality pattern emerged in the dwarfed children. The effects of age, sex, and socioeconomic status on personality traits were similar for dwarfs as for controls. Intelligence and personality variable were similar in dwarfs with and without endocrine disease. However, symptoms of the psychoendocrine syndrome, namely appetite and thirst disturbances, hypersensitivities, and impulse reduction, were more frequently seen among hypopituitary dwarfs. Social and coping behaviour was impaired in the majority of dwarfs. It is concluded that psychological disturbance occuring inchildren of small stature is a response to being small and is not attributable to any endocrine effect.
