CIR sand casting system for trans-tibial socket.

The authors have developed a trans-tibial socket fabrication system based on the "dilatancy" principle, a process that has been commonly used in forming wheelchair Seating. The CIR Sand Casting System replaces plaster of Paris with sand for forming both a negative sand mould and a positive sand model, which can be modified for either thermoplastic socket formation or resin lamination. Initial clinical trials suggest that fabrication times are approximately 90 minutes from patient evaluation and casting to dynamic alignment. Compatibility with all existing prosthetic components is retained. It is believed the CIR socket fabrication system may be a competitive alternative for prosthetic service providers in developing countries.

Rheology and Flocculation of Oil-in-Water Emulsions Made with Mixtures of alphas1-Casein + beta-Casein.

The influence of the composition of a mixed binary protein emulsifier composed of alphas1-casein + beta-casein on the rheology of concentrated oil-in-water emulsions (45 vol% oil, 5 wt% protein, pH 7) has been investigated over the temperature range 0-40 degreesC. Controlled stress viscometric data are reported over the shear stress range 0.1-30 Pa for systems with alphas1-casein/beta-casein ratios of 100:0, 98:2, 95:5, 90:10, 75:25, 50:50, and 0:100. The pure casein emulsions showed substantially different temperature-dependent rheology, and there was observed to be a pronounced maximum in the small-deformation complex modulus of the pure alphas1-casein emulsion in the range 30-40 degreesC. In the emulsions containing >/=90% alphas1-casein in the emulsifier mixture, all of the beta-casein present was found to be associated with the surface of the droplets. Average droplet sizes and protein surface coverages were higher in the mixed casein systems than in the equivalent pure casein systems. The strongly pseudoplastic character of the emulsions is consistent with extensive reversible flocculation caused probably by a depletion mechanism involving unadsorbed protein. The degree of flocculation is sensitive to temperature and to the alphas1-casein/beta-casein ratio. The results can be interpreted in terms of changes in protein self-assembly and adsorbed layer structure which influence the strength of the interdroplet interactions and hence the rheological behavior of the emulsions. There is some evidence of a specific role for alphas1-casein-beta-casein complexes in these systems. Copyright 1998 Academic Press.

Influence of Protein Interfacial Composition on Salt Stability of Mixed Casein Emulsions.

The influence of casein surface composition on the stability toward flocculation by sodium chloride has been investigated for oil-in-water emulsions (20 vol % oil, 1 wt % total protein) prepared with a mixture of alpha(s1)-casein + beta-casein. It has been observed that the poor salt stability of emulsions containing 100% alpha(s1)-casein is greatly enhanced by the replacement of about one-third of the alpha(s1)-casein by beta-casein. With around half of the adsorbed protein layer consisting of beta-casein, emulsions remain stable at ionic strengths of 2 M NaCl in the presence or absence of calcium ions (5 mM). At the point of droplet flocculation in these emulsions, most of the casein present is associated with the surface of the droplets. These results provide definitive evidence for the important influence of the compositional balance in sodium caseinate on the colloidal stability behavior of casein-based emulsions.

[Epidemiologic background and changes in patients infected with human papilloma virus].

Contagion with certain types of HPV was supposed to have a causal relationship with cervical neoplasia of the uterus. The rate of prevalence of HPV was investigated in pre-cancer and cancer patients with uterine cervical smear using virapap or viratype. According to the cytologic classification, among those whose cytology was diagnosed as class I or II, were found a few positive HPV, however, in cases in classes cytology IIIa, III and IIIb, the positive rate turned out to be 22.5, 41 and 72.4%, respectively. About 65% of patients whose post-operative diagnosis was cervical carcinoma, had been found positive in the pre-operative HPV.DNA check up. The statistical profiles of virally infected subjects were regarded as slightly younger females with early onset of menarche. A higher positive rate was found in such groups such as unmarried single and divorced single women, career employees with special skills, housewives and dwellers in residential and commercial sections. The follow up study of HPV infection was checked with subjects with dysplasia, and no case was recognized in which initially HPV negative dysplasia turned to positive during the observation period. But, in about 50% of those checked, initially HPV positive dysplasia turned to negative, during the follow up period. In the cases with long term (more than 8 years) dysplasia which was followed up, only one out of 10 was found to be HPV positive, while in middle term (more than 2 years but less than 8 years) followed up dysplasia, the positive rate was calculated as 47.8%.

[Effects of chemotherapy of advanced malignant ovarian tumors].

Malignant ovarian tumor is regarded as the disease with the worse prognosis among obstetrical and gynecological malignancies. In the past, a great number of trials have been done with single or combined treatments. Because of the rather low incidence of malignant ovarian tumor in any one hospital over a short period of time and the numerous factors affecting the prognosis of patients, no definite procedures have been established so far. In order to evaluate the effects of chemotherapy, 460 cases were investigated retrospectively. These cases were treated primarily with surgery from 1974 to 1979 and adjuvant chemotherapy was selected freely by the doctors responsible. Cumulative 5-year survival rates were calculated as 88.8% for a low potential malignant group, 65.9% for stage I ovarian cancer, 40.6% for stage II, 9.7% for stage III, 0% for stage IV and 10% for metastatic tumor from other organs. The effects of chemotherapy were investigated especially for stage III and IV and the metastatic tumor group. Cumulative survival curves were justified by generalized Wilcoxon test. An MFC combination, a double-agent, a single-agent and a no-chemotherapy group were compared by survival curves. There were statistical significances between the MFC and no-chemotherapy group, and the double-agent and no-chemotherapy group. Significant tendency was present between the MFC and single-agent group. Several popularly-used combinations in this period such as MFC, VEM, METT, FAMT, METVFC and a group with other combinations were compared. There were no statistical significances among the 6 combinations, but an MFC combination seemed to be a better treatment for malignant ovarian tumors judging from accumulative survival curves.
