[Topical H1 antihistaminics in the therapy of acute conjunctival allergic reactions].

AIM: To determine, whether the immediately originated allergic reaction of conjunctivas may be treated with local H1 antihistaminic drugs in a monotherapy (Allergodil gtt, Emadine gtt). GROUP AND METHODS: The group included 30 patients at the age of 17 to 40 years at the mean age of 33.2 years, where chemosis and hyperemia of bulbar conjunctiva originated suddenly as well as edema of eyelids. The reported subjective symptoms were itching, burning sensation in the eyes, lacrimation, and the feeling of pressure under the eyelid. Local treatment with azelastin (Allergodil gtt. oph., Pliva), was applied in 15 patients, the other 15 patients were treated locally with emedastin (Emadine gtt. oph., Alcon) at the dose of one drop twice daily into the conjunctival sac of both eyes. The time of onset of the therapeutic effect and the disappearance of subjective symptoms and objective signs were observed. RESULTS: The disappearance of subjective symptoms after the start of therapy developed within 20 minutes after Allergodil gtt and within 5 minutes after Emadin. Subjective complaints disappeared in the range of 86-100% after Allergodil gtt and in the range of 90-100% after Emadin gtt. A substantial diminution or even complete disappearance of objective symptoms developed in 74-80% cases after the treatment with Allergodil gtt and in 80-87% subjects after Emadin gtt. CONCLUSION: The investigated local H1 antihistaminic drugs proved to be effective in monotherapy of acute phase of allergic conjunctivitis. The drugs influence subjective complaints faster and more efficiently than the objective signs. The therapy of objective manifestations requires longer period of time for the effect to develop.

[Ketotifen fumarate (Zaditen) eye drops in the treatment of seasonal allergic conjunctivitis].

A group of 25 patients with signs of seasonal allergic conjunctivitis treated during the period of 2 to 6 month with ketotifen fumarate 0.05% eye drops 2-3 times daily (Zaditen eye drops, Novartis Ophthalmic, AG, Switzerland) is described here. As subjective symptoms of itching, stinging, tearing, and mucous discharge were observed and evaluated, as well as objective signs of hyperemia of the conjunctiva and hypertrophy of the tarsal papillae. Similarly the onset of action, duration time, and toleration of the drug were assessed. Subjective symptoms disappeared after 10 days of treatment duration in 60% to 80% patients, after two months of treatment were present in the range 0% to 20%. Objective signs diminished after 10 days of treatment in 60%-72% patients, and after 2 months in 72% to 96% of patients. Hypertrophy of papillae of the tarsal conjunctiva did not totally disappear even after 6 month of treatment with ketotifen fumarate eye drops. Onset of action in all patients was 20 minutes or less, the duration of action was in the range of 8-13 hours. The drug was found as a good tolerable by all patients. Sixteen per cent of patients felt some burning following the instillation. The study states that ketotifen fumarate 0.05% eye drops are effective in a monotherapy of seasonal allergic conjunctivitis. Due to its multiple action, the drug prevents the allergic reaction as well. Subjective symptoms are influenced faster and more effective than the objective signs. The treatment of objective signs needs prolonged use of these drops.

[Determination of the antioxidant activity of potential scavengers of nitric oxide].

Nitrogen oxide is a relatively stable, highly reactive radical, which develops in the organism by oxidation of the guanidine nitrogen of the amino acid L-arginine by the action of NO-synthase with the development of L-citrulline. It participates in a number of physiological and pathological processes. The difference between the physiological and pathological concentration of nitrogen oxide is very small, and for that reason we search for suitable methods of its determination and the substances influencing the level of nitrogen and thus decreasing its overproduction. Testing of scavenger activity against NO was performed by the method following Griess (spectrophotometric determination of nitrites as the oxidation products of NO), which was compared with the HPLC (high-performance liquid chromatography) method developed by the present authors. The antiradical effect of the compounds being assayed (acetylsalicylic acid, piracetam, paracetamol, serotonin, stobadin dihydrochloride, 5-hydroxy-L-tryptophan, N-acetyl-L-cysteine, L-glutathion, and N-acetyl-DL-penicillamine) against NO was compared with the so-called total antioxidant activity--TRAP (total radical-trapping antioxidant parameter), which corresponds to the capability to trap peroxyl radicals. On the basis of the obtained data, piracetam and acetylsalicylic acid show no antioxidant activity, paracetamol shows probably moderate scavenging action against nitrogen oxide, and other compounds tested (primarily 5-hydroxy-L-tryptophan, serotonin, stobadin) are strong antioxidants against both NO and peroxyl radical.

[Allergodil eye drops in the treatment of allergic conjunctivitis].

The authors describe a group of 20 patients with symptoms of allergic conjunctivitis where for a period of 2-6 weeks twice a day into the conjunctival sac of both eyes Allergidol gtt, ophth., ASTA Medica AG FRG was administered. As to subjective sensations the authors evaluated itching, burning, the sensation of a foreign body and lacrimation, as to objective signs oedema of the eyelids, hyperaemia of the conjunctiva and papillary hypertrophy. The authors evaluated also the rate of onset of the effect, its duration and tolerance of the preparation. Subjective symptoms disappeared in 90% patients, hyperaemia of the conjunctivae and papillary hypertrophy receded in 12 patients (60%), in 8 (40%) they disappeared. Regression of subjective complaints occurred in all patients within 20 minutes, the duration of the effect was within the range of 11-13 hours in 18 patients (90%), 6-8 hours in two patients (10%). As to undesirable effects one female patient reported after administration of the eye drops a bitter sensation in the mouth, another one a burning sensation. Evidence was provided that the local H1 antihistaminic Allergodil gtt is effective in monotherapy in mild forms of allergic conjunctivitis.

Soluble isoforms of CEACAM1 containing the A2 domain: increased serum levels in patients with obstructive jaundice and differences in 3-fucosyl-N-acetyl-lactosamine moiety.

CEACAM1 (biliary glycoprotein or CD66a) is a member of the carcinoembryonic antigen (CEA) subgroup of the CEA family. Eleven RNA isoforms derived from the splicing of a single CEACAM1 gene have been described. Some of the CEACAM1 isoforms have been recognized by the CD66 antibodies in T and B lymphocytes, natural killer cells, granulocytes and epithelial cells in several human tissues. Although it is also present in soluble form in bile and serum, and elevated levels have been found in the serum of patients with liver diseases, it is not known which isoforms are primarily involved. In order to learn more about the distribution and properties of particular CEACAM1 isoforms, we have prepared a monoclonal antibody specific for the A2 domain of CEACAM1, designated TEC-11. This antibody does not cross-react with other members of the CEA family. Immunoblotting analysis revealed that the TEC-11 epitope was present in all cell types expressing CEACAM1 containing the A2 domain [CEACAM1(A2)], including granulocytes (160 000 MW isoform) and sperm cells (140 000 MW isoform). A 115 000 MW isoform of CEACAM1(A2) was present in human serum, bile, saliva and seminal fluid. Human bile, saliva and seminal fluid also contained the 160 000 MW CEACAM1(A2) isoform. Significantly higher serum levels of the 115 000 MW CEACAM1(A2) isoform were detected in patients with obstructive jaundice. The 160 000 MW isoform of CEACAM1(A2) in bile, but not a 115 000 MW isoform in serum and bile, carried the 3-fucosyl-N-acetyl-lactosamine moiety. The combined data indicate that various isoforms of CEACAM1(A2) are present in different body fluids where they could take part in different CEACAM1-mediated functions.

Vitamin E and A and periodontium in pregnant diabetics.

The authors followed physiological levels of vitamin E and A in four-week intervals in the course of physiological pregnancy in 11 insulin dependent diabetics. The highest mean levels of both vitamins were found in the IIIrd trimester. Simultaneously, the periodontal condition and the standard of oral hygiene were evaluated by means of epidemiological indexes in two-week intervals. The most marked inflammatory changes of periodontium occurred in the 8th month and were followed by an improvement before delivery, though the standard of oral hygiene was unchanged. The possibility of the eventual contribution of both vitamins in the improvement of periodontal condition in the final stage of pregnancy seems evident.

[Periodontopathies and fixed prosthetic appliances].

The authors examined a group of 979 subjects with fixed prosthetic appliances, admitted for treatment at the periodontological department on account of periodontopathy. The group was subdivided according to sex, age, periodontological diagnosis and type of fixed prosthetic appliance. Based on the results of the initial examination, the authors compared the state of the periodontium of teeth with a prosthetic appliance and the other teeth using Russell's clinical and X-ray PI index. They investigated the oral cavity, using the OHI-S index according to Green and Vermillion and the DMF index. The results confirmed the negative influence of fixed prosthetic appliances on the periodontium and revealed the necessity of X-ray check-ups of the periodontium of teeth white these appliances.

Periodontium and vitamin E and A in pregnancy.

The evaluation of the clinical condition of periodontium by means of the epidemiological indexes and the level of oral hygiene in two weeks intervals in the course of physiological pregnancy in 39 women in good general health revealed the maximum of inflammatory changes of periodontium in the 8th month of pregnancy with the amelioration shortly before delivery. Simultaneous follow-up of the physiological levels of vitamin E and A in four weeks intervals showed the decline of the mean levels of both vitamins in the course of the 8th month and their marked elevation shortly before delivery; therefore remains questionable, if this elevation reaching over their physiological range, contributes to the amelioration of the condition of periodontium observed at the same time.

Stable line of turkey bone marrow cells transformed by the myelocytomatosis virus strain MC31.

A stable bone marrow-derived cell line LSTC-SF2, producing transforming avian acute leukaemia virus, was established. The cell line has been maintained in culture for over 4 years, and 450 passages can be frozen and easily recovered in viable form. The cells grow rapidly in monolayers with multilayer clumps, have low serum requirements, display specific morphology, form colonies in semisolid medium and release transforming MC31 virus. A helper virus was also detected by the reverse transcriptase assay.

Concerning the results of the initial examination of individuals who have undergone periodontal surgery.

The results of initial examination of 1,021 patients (460 men and 561 women), admitted for the periodontal surgical treatment during the time period of 5 years at the periodontal department of the 1st Clinic of Stomatology in Olomouc, were retrospectively evaluated from the records in out-patients' clinical notes. The results are essentially in accordance with the generally acknowledged conclusions of epidemiological studies with the only difference consisting in the finding that in this specific group of patients the oral hygiene did not deteriorate with increasing age. From the point of view of the necessity of periodontal surgical treatment the men and women in the age groups between 30 and 39 years have been identified as individuals at the highest risk.

Antiphlogistics in periodontology.

In 146 individuals with inflammatory periodontal disease the authors verified the therapeutical effect of Sanchelin (the mixture of sanguinarine and chelerythrine in 0.05% concentration) in gel (4% hydrogel carboxymethylcellulose) applied into pockets and in aqueous solution administered in the intrapapillar route. The effect of Sanchelin in solution was compared with the effect of 2% hydrocortison in the same way of administration. The clinical condition of periodontium and the quality of oral hygiene was evaluated by means of indexes and papillae were examined histologically. The authors demonstrated that Sanchelin in gel is an effective drug in treatment of inflammatory periodontal disease. They observed lesser effectivity when intrapapillar administration of Sanchelin in solution was used.

Analysis of epidermal growth factor and epidermal growth factor receptor expression in human renal carcinoma cell cultures.

In cells derived from two human renal carcinomas only the precursor form of epidermal growth factor (EGF) was found. The binding assay revealed a high level of EGF receptor expression in both cell types tested. However, these receptors are not involved in the growth activity of the cells under in vitro conditions used. The source of DNA synthesis-stimulating activity found in conditioned media of the cells tested is discussed with respect to possible participation of TGF beta.
