[The effect of the molecular triggers of hemostatic activity on thrombosis in early stage of electrical injuries].

OBJECTIVE: To evaluate the mechanisms of thrombosis in the early stage of electrical injury. METHODS: Endothelin-1(ET-1), prostacylin(PGI2), platelet alpha-granule membrane protein (Gmp-140), thromboxane (TXB2), and plasminogen(PLG-A) were measured in 26 patients with electrical injury. RESULTS: It was found that GMP-140 and ET-1 increased significantly(P < 0.01), PLG-A showed a significant change 2 weeks after the injury, while there was an imbalance between TXB2 and PGI2. CONCLUSION: It is believed that change in one or several mediators mentioned above may trigger thrombosis after electrical injury.

[The study of growth hormone on wound healing rate in adult burns].

OBJECTIVE: To determine the effects of rhGH on wound healing in adult burn patients. METHODS: 42 patients with burn wounds covering over 83% of total body surface area were enrolled in this placebo controlled prospective study. They were comparable in nutrient intake, TBSA, and full thickness burn area. For rhGH group patients rhGH subcutaneously was given in the dose of 0.2 U.kg-1 at 8 a.m. each morning for 14 days. The control group patients were given normal saline as placebo. Protein catabolism, immunocompetence, healing time of burn wounds and donor sites were assayed. RESULTS: 1. The patients in rhGH group had higher serum levels in albumin, transferrin, prealbumin and CD4 T-cells (P < 0.01). 2. The healing time of autologous skin grafting and donor site and the length of hospital stay were significantly shorter in rhGH group patients compared with control group. CONCLUSION: rhGH could enhance the wound healing rate, improve protein anabolism and reduce the length of hospital stay in severe burn patients.

Changing pattern of heart diseases in Shanghai from the 1950s to 1980s.

OBJECTIVE: To determine the pattern of dynamic change in types of heart diseases in the past 4 decades for providing a reference to treatment and prevention of heart diseases in China. METHODS: All adult cardiac patients admitted to the Zhong Shan Hospital and Hua Shan Hospital from 1948 to 1989 were analyzed. The constituent ratio of different heart diseases in the 1950s, 1960s, 1970s and 1980s was compared. RESULTS: The percentage of heart diseases among medical inpatients increased in each of the recent 4 decades, from 9.89%, 15.69%, 20.90% to 23.54% respectively. The constituent ratio of different heart diseases changed, coronary heart disease constituted the highest proportion, next came rheumatic heart disease, and congenital heart disease was in third place. Congenital heart disease, myocarditis, cardiac dysrhythmia without organic heart disease, cardiomyopathy and endocarditis increased, rheumatic heart disease, pulmonary heart disease and hypertensive heart disease apparently decreased; syphilitic heart disease could rarely be encountered. CONCLUSION: China is now facing a low overall death rate, a long life expectancy and an aging population. The incidence of heart diseases increased gradually and patterns of heart diseases kept changing. The overall trend is that heart diseases which were not related to infection are increasing while heart diseases related to bacterial infection are decreasing.

Relationship between remodeling and function of left ventricle and angiotensin II AT1 receptor expression after myocardial infarction in rats.

OBJECTIVE: To determine the relationship between remodeling and dysfunction of left ventricle (LV) and the expression of the angiotensin II AT1 receptor mRNA after myocardial infarction (MI) in rats. METHODS: Nine MI rats (Group A) and 8 sham-operated rats (Group B) were studied by both Doppler echocardiography and Dot blot using Digoxingenin-labelled cDNA probes. RESULTS: Compared with Group B, Group A showed the increase in LV internal diastolic diameter (0.87 +/- 0.06 mm vs 0.66 +/- 0.03 mm, respectively, P < 0.01) and volume (0.73 +/- 0.09 ml vs 0.51 +/- 0.05 ml, P < 0.01). In addition, thinning of anterior wall, thickening of posterior wall, increasing of peak early filling velocity (peak E), decreasing of late filling velocity (peak A) and increasing of the E/A were demonstrated in MI rats 7 weeks after MI. The levels of the cardiac angiotensin II AT1 receptor mRNA in Group A were higher than those of Group B (2.2-fold). CONCLUSIONS: Seven weeks after MI in rats, character of LV remodeling and dysfunction were developed and the expression of cardiac angiotensin II AT1 receptor mRNA was increased.

cDNA cloning and sequence analysis of hepatitis G virus genome isolated from a Chinese blood donor.

OBJECTIVE: To obtain full-length sequence of a Chinese hepatitis G virus (HGV) strain (HGVch) and investigate the genetic characteristic of HGVch and its identity to other isolates. METHODS: Reverse transcription (RT) and nested-PCR were used to screen HGV RNA positive serum and amplify cDNA fragments. A positive serum without known hepatitis virus markers was selected for isolating HGV RNA template. The HGV genome was divided into 12 overlapping fragments and directly cloned into pGEM-T vector. Sequences were determined by dideoxy terminus-end method of DNA sequencing and then analyzed by computer. RESULTS: The twelve fragments of HGVch cover 9213 nucleotides in length, containing a large open reading frame (ORF) encoding 2873 animo acids polyprotein that began with a methonine residue and ended at termination codon. HGVch is about 86.5%-89.5% identical to other known HGV isolates at the nucleotide level and about 93.9%-96.2% at the deduced animo acid level. CONCLUSION: HGV is a non-A-E hepatitis causal agent, proved to be related with posttransfusion hepatitis in all over the world. Chinese HGV isolate has very close relationship to other isolates from Africa, Europe, Japan, without significant difference across the entire genome. It is suggested that the sequences of HGV isolates are very conservative and the evolution is very slow.

[Expression of MAGE-1 gene in hepatocellular carcinoma].

OBJECTIVE: To investigate the expression of MAGE-1 mRNA in human hepatocellular carcinoma (HCC) and to discuss the possibility of using MAGE-1 antigens as a new peptide vaccine for immunotherapy for HCC. METHODS: The expression of MAGE-1 mRNA in HCC tissue and the adjacent non-HCC liver tissue was detected using RT-PCR in 45 samples of HCC, 16 samples of cirrhosis and 12 samples of normal liver tissue. The 6 samples of MAGE-1 mRNA positive PCR products were DNA sequenced. The sequenced fragments of MAGE-1 cDNA were used as template to which a [alpha 32P] labeled probe was synthesized and employed for Southern blot analysis. HLA class I-A and I-B typing of 43 HCC patients were assayed by ELISA test. RESULTS: Of the 45 HCC samples, 32 expressed MAGE-1 mRNA. None of the HCC adjacent tissue or liver tissue from cirrhosis or normal liver samples expressed MAGE-1 mRNA. The DNA sequence confirmed that the target gene fragment in all the 6 samples of PCR products was MAGE-1 cDNA. Point mutation occurred in 3 samples resulting in alternation of amino acid residues in 2 positions. Southern blot analysis showed that MAGE-1 was also detectable in HCC tissue which was MAGE-1 positive in RT-PCR assay, but not detectable in non-HCC liver tissue. In HCC patients, the predominant types of HLA were A2(53.5%), A11(25.6%), A24(20.9%), A33(20.9%), B13 (28.3%) and B35(23.2%). MAGE-1 mRNA expression in HCC showed no correlation with the level of alpha-FP in blood and the diameter of tumor. CONCLUSIONS: MAGE-1 mRNA is expressed at a high percentage of HCC samples. This tumor rejection antigen may have potential to be used as a new peptide vaccine for immunotherapy for HCC. There are point mutations of MAGE-1 gene sequence in some Chinese HCC patients.

[The cytologic ultrastructure and cytochemical features of human dendritic cells from peripheral blood].

OBJECTIVE: To observe the cytomorphological, cellular ultrastructure and cytochemical features of the dendritic cell (DC) enriched populations in vitro. METHODS: Fresh peripheral blood from healthy volunteers was collected and mononuclear cells (PBMC) were isolated on lymphocyte separating solution. The PBMC were cultured in mediums containing rhGM-CSF, rhIL-4 and rhTNF-alpha 9-12 days to generate DC-enriched populations, the morphological features and numbers of DC were identified and counted respectively by light microscope (LM) and confirmed by immunofluorescence assay (IFA), the ultrastructure characteristics of the DC populations were observed by transmission electron microscope, and the cytochemical features of them were also examined. RESULTS: The percentage of DCs was 70%-80% in the DC-enriched populations as confirmed by IFA using mABXII. Transmission electron microscopy revealed that the DC populations contained two subsets of DC, one was considered as precursor cell derived DC(PC-DC), the cell surface of which had many irregular dendrites with a irregular nucleus, small nucleoli, numerous mitochondria, but scanty lysosomes, ribosome, rough endoplasmic reticulum and free polysomes. Cytochemical staining showed the DCs were both peroxidase (POX) and non-specific esterase (NSE) negative. The other was indentified as monocyte-derived DC(MO-DC), the features of which included: irregular shape, many ruffles and some rough lames on cell surfaces, nephritic or horseshoe nuclei with a thick rim of heterochromatin, numerous mitochondria, much more lysosomes, ribosomes, rough endoplasmic reticulum and Golgiosome. The cytochemical reactions indicated that most of them had POX-positive granules, and were NSE slightly positive. CONCLUSION: Cytokines in vitro induced DCs from human peripheral blood are heterogeneous.

[Effect of running granule on repairing capacity of neuro-endocrine-immunity and DNA in treating mastoplasia].

OBJECTIVE: To explore the clinical effect and therapeutic mechanism of Runing Granule (RNG) in treating mastoplasia to elucidate its pathogenesis. METHODS: One hundred and eighty-seven patients suffering from mastoplasia were randomly divided into the RNG treated group (treated group, 147 cases), and the tabellae tamoxifen control group (control group, 40 cases). The follow-up underwent for 3 months. Before and after treatment, changes of levels of plasma neurotransmitters, serum endocrine hormone, peripheral lymphocyte unscheduled DNA synthesis, the count of T lymphocyte subsets in the luteal phase during the menstrual cycle of 104 cases were measured, their clinical effects were also observed. RESULTS: Before treatment, these cases showed disturbances in the secretion of 5-hydroxytryptamine (5-HT), epinephrine (E), estradiol (E2), follicle stimulating hormone (FSH), luteinizing hormone (LH), and the levels of norepinephrine (NE), prolactin (PRL), inducer-helper T lymphocytes (OKT4+), suppressor T lymphocytes (OKT3+), were significantly increased, progesterone (P), testosterone (T), total T cells (OKT3+), OKT4+/OKT8+ ratio, unscheduled DNA synthesis (UDS) were obviously reduced (P < 0.05, P < 0.01) in mastoplasia patients. After treatment, RNG showed regulation on disturbances of these parameters, the curative rate of the treated group was higher than that of the control group; the metabolic disturbances of 5-HT/NE, E2/P, T, OKT3+ in the treated group were improved more significantly than those in the control group (P < 0.05). During the period of clinical observation, no obvious side effect and toxicity of RNG were found. CONCLUSIONS: Mastoplasia is caused by the interactions among multi-factors in which the neuro-endocrine-immune network plays a key role in the pathogenesis of mastoplasia. RGN was effective in treating mastoplasia, the mechanism probably lays on the regulation of comprehensive coordination from the multi-layers, multi-links and multiple pathways in the neuro-endocrine-immunity network and elevation of internal environment-stabilizing capacity of the body.

[The role of nitric oxide on the functional change of interleukin-1 beta and interleukin-6 induced rat pancreatic islets of Langerhans].

OBJECTIVE: To observe the effects of interleukin (IL)-1 beta and IL-6 on nitric oxide (NO) production, islet cell DNA and insulin content, and MTT assay of rat pancreatic islets of Langerhans. METHODS: Isolated pancreatic islet cells from SD rats were cultured in monolayer in vitro. Nitrite production, islet cell DNA and insulin content, and MTT assay in rat pancreatic islet cells incubated with IL-1 beta and IL-6, singly or in combination, were measured. The levels of these parameters of the cytokines in the absence and presence of N(G)-monomethyl-L-arginine (L-NMMA), an inhibitor of nitric oxide synthetase, were compared. RESULTS: IL-1 beta induced a significant increase in nitrite production, a decrease of islet cell DNA and insulin content and reduction of cell activity (MTT, P < 0.001); these changes were blocked by L-NMMA (P < 0.001). IL-6 produced no significant change in those parameters (P > 0.05). The combination of IL-1 beta and IL-6 increased NO production, decreased islet cell DNA and insulin content and reduced cell activity (P < 0.001); the changes were similar to those with IL-1 beta alone (P > 0.05). CONCLUSION: The cytotoxic effects of IL-1 beta on pancreatic islets might be mediated by NO. IL-6 may not be directly cytotoxic to pancreatic islets. It may be related with the fact that IL-6 does not induce NO production.

[A clinical study on vaccine of Mycobacterium vaccae in treating pulmonary tuberculosis].

OBJECTIVE: To study the effect of vaccine of Mycobacterium vaccae on cell-mediated immunity and on treating patients with pulmonary tuberculosis. METHODS: Seventy cases of pulmonary tuberculosis with smear positive and initial treatment were classified randomly into group I (35 cases) and group II (35 cases), receiving 2HRZS/4HR and 2HRZS/4HR plus vaccine of Mycobacterium vaccae regimens respectively. Thirty-one multi-drug resistant pulmonary tuberculosis cases were classified into group III, receiving 4 - 6 sensitive antituberculous drugs and vaccine of Mycobacterium vaccae. Improvement of clinical symptoms, resolution of pulmonary lesions, negative conversion of sputum and changes of immunological functions were observed. RESULTS: No significant difference in improvement of symptoms was found in group I and group II (P > 0.05), and the improvement rate of clinical symptoms in group III was found more than 50%. X-ray resolution rates in 4th month were 83% and 89%, and cavity reducing rates 40% and 50% respectively in group I and group II, and no significant differences were found (P > 0.05). X-ray resolution rate was 29%, cavity reducing rate 7% and no deteriorated case was found in group III. Sputum negative conversion rates in 1st, 2nd, 3rd and 4th month were 23%, 51%, 83% and 97% respectively in group I, while 31%, 77%, 89% and 100% in group II, and 3%, 16%, 29% and 32% in group III. Significant difference was found between group I and group II in sputum negative conversion rate in 2nd month after treatment (P < 0.05). After treatment, values of lymphocyte transformation test (LTT), CD(3), CD(4) and CD(4)/CD(8) of the above 3 groups were all higher than that before the treatment (P < 0.05), level of tumor necrosis factor decreased in group II and IL-2, IL-6 increased in group III. CONCLUSIONS: Vaccine of Mycobacterium vaccae is a good immunotherapy preparation, which promotes sputum negative conversion and activation of cell-mediated immunity.

[Clinical application of expandable metal stent for palliation of malignant biliary obstruction].

OBJECTIVE: To investigated the long-term result of expandable metallic biliary stent implantation and evaluated preliminarily the factors that might impact the therapeutic effectiveness. METHODS: 134 patients with malignant choledochal stenosis were treated with metallic endo-prosthesis (128 transpapillary endoscopically and 6 transhepatic percutaneously). The follow-up results were compared with those of 97 patients with conventional plastic stents during the same period. RESULTS: The success rate was 95.7%, and the procedure related complications were found in 6.4% of patients. The jaundice was eliminated completely in 85.6% patients. During long-term follow-up, the metal stents were shown patent for a mean duration of 300 days, which was significantly longer than 80 days of polyethylene endoprostheses (P < 0.01). The mean survival was 210 days. From classified data assessment, the stent patency in primary bile duct neoplasm group was almost similar to that in metastatic group, but the median survival of the former was slightly longer than that of the latter. There were flow differences in patancy and survival rates between the hilar obstruction group and the middle and low obstruction group. CONCLUSIONS: The median patancy of expandable metal stents was significantly longer than that of conventional plastic prostheses. To get the highest benefit, however, the indication should be strictly selected.

[Studies on ACE gene insertion/deletion polymorphism, serum ACE activity, and diabetic retinopathy in type II diabetic patients].

To clarify the relationship among angiotensin 1-converting enzyme(ACE) gene insertion/deletion(I/D) polymorphism, serum ACE activity, and diabetic retinopathy in Type II diabetic patients, we examined 36 healthy controls, 40 Type II diabetic patients without diabetic retinopathy, and 68 Type II diabetic patients with diabetic retinopathy. All of patients suffered from Type II diabetes beyond 5 years and matched in age and body mass index. An insertion/deletion polymorphism of ACE gene was identified by polymerase chain reaction(PCR). Serum ACE activity was determined using spectrophotometry. The distribution of DD, ID, and II genotypes of the ACE gene did not differ among the three groups (16.7%, 33.3%, and 50.0% in the healthy controls; 22.5%, 35.0%, and 42.5% in Type II diabetic patients patients without diabetic retinopathy; and 20.6%, 30.9%, and 48.5% in Type II diabetic patients with diabetic retinopathy; respectively). The frequency of ACE I/D genotypes was not significantly different between non-proliferative retinopathy and proliferative retinopathy of Type II diabetic patients with diabetic retinopathy (P > 0.05). Serum ACE activity of three groups was similar (P > 0.05). These results do not support the hypothesis that the DD genotype of the ACE gene would be a clinically useful genetic marker for predicting the development of diabetic retinopathy in Type II diabetic patients. There is no association between ACE gene I/D polymorphism and the prognosis of diabetic retinopathy in Type II diabetic patients. ACE may not involve in forming diabetic retinopathy in Type II diabetic patients.

[The relationship between surgical staging, pathologic grading, operative type and postoperative recurrence in giant cell tumor of bone].

Sixty-eight surgical procedures of 58 patients with Giant Cell Tumor of Bone(GCT) were followed up over two years. The recurrence rate of GCT was Stage 1: 0 in 5, Stage 2: 15.4% in 39 and Stage 3: 37.5% in 24. It was significantly higher in Stage 3 than in Stage 2 (P < 0.05). The recurrence rate of GCT in Stage 2 was 30.8% in 13 intralesional curettage, and 0 in 17 wide or radical resection(P < 0.05). We conclude that the surgical staging of GCT has important value in predicting postoperative recurrence, in accurately evaluating the operative efforts and in guiding treatment, and that the operative type is a significant factor effecting on the recurrence of GCT.

[Influence of chemotherapeutants and cytokines on growth and transgene expression of bone marrow cells from MT/p210(bcr-abl) transgenic mice].

The bone marrow cells of CML(chronic myelogenous leukemia) transgenic mice carrying metallothionein(MT) promoter/enhancer, bcr-abl(p210) cDNA and SV40 splicing/poly(A) signal sequences were cultured in liquid and soft agar with hydroxyurea (Hu), 5-fluorouracil(5-Fu), mouse stem cell factor (mSCF) and mouse interleukin-3(mIL-3) independently or combinatively. The cell or colony counting and transgene RT-PCR analysis showed that the proliferation, colony formation and transgene expression of the bone marrow cells were advanced in the combinative culture of mSCF with mIL-3, but they were significantly inhibited in the culture without any growth factors, or with mSCF, mIL-3 and Hu or 5-Fu. These findings suggest that the combinative utilization of chemotherapeutants and cytokines is a potentially effective strategy of the clinical treatment for CML.

[Relationship between serum angiotensin I-converting enzyme activity and diabetic nephropathy in patients with type II diabetes].

To clarify the relationship between serum angiotensin I-converting enzyme (ACE) activity and Type II diabetic patients with or without diabetic nephropathy. We examined 36 healthy controls, 58 Type II diabetic patients without diabetic nephropathy, and 50 Type II diabetic with diabetic nephropathy. The diabetic duration of all of Type II diabetic patients was more than 5 years. All patients matched well in age and BMI. An insertion/deletion polymorphism of ACE gene was identified by polymerase chain reaction(PCR). Serum ACE activity was determined using spectrophotometry. Although the distribution of DD, ID, and II genotypes of the ACE gene did not differ among the three groups, serum ACE activity was significantly higher in Type II diabetic patients with diabetic nephropathy than in that without diabetic nephropathy(P < 0.05), especially in the groups with D allele of ACE gene. Serum ACE activity had no difference between healthy controls and Type II diabetic patients without diabetic nephropathy. There is no association between serum ACE activity and Type II diabetic patients without diabetic nephropathy in any genotype of ACE gene. The increasing serum ACE activity plays a role in the initiation of diabetic nephropathy or may serve as a risk marker for later development of overt diabetic nephropathy in Type II diabetic patients.
