[Surgical treatment possibilities of severe urinary incontinence in women].

Severe female urinary incontinence type 3 is a most difficult challenge to a urologic surgeon. In 40 of more than 800 patients evaluated for stress incontinence, type 3 urinary incontinence was diagnosed by clinical, urodynamic, and radiographic examination. The etiology was neurogenic or non-neurogenic. The first step of treatment in all patients was a modified pubovaginal sling to increase urethral resistance; this procedure was successful in 65%. Severely damaged periurethral tissues resulting from earlier multiple pelvic surgery (111 operations in 28 patients with non-neurogenic etiology), radiation, or prior local infections were responsible for failures. Patients whose incontinence was not corrected by this initial treatment required further surgical procedures, such as suburothelial Teflon injection, urethral reconstruction, and continent urinary diversion. This additional surgery resulted in an overall 92% success rate after a minimum follow-up of 18 months. In view of the severe nature of the incontinence of these individually problematic patients, this is a satisfactory results.

Establishment and characterization of a tumorigenic trophoblast-like cell line from a human placenta.

A trophoblast-like cell line, TL, was established from a normal-term human placenta. The TL cells were epithelial in morphology with relatively large vesicular nuclei, prominent nucleoli, and numerous microvilli on the cell surface. Cytoplasmic organelles were generally sparse but mitochondria and polysomes were abundant. The cells grew as compact sheets with close membrane approximation interconnected occasionally by desmosome-like junctions. TL cells contained placental alkaline phosphatase, a placenta-associated antigen, cytokeratin, and prekeratin, but not keratin. In parallel, they were negative for factor VIII, vimentin, and fibronectin. Population doubling time was estimated to be about 34 h. TL cells were tumorigenic in nude mice and an increase in tumorigenicity was observed after a certain number of passages in vitro. Chromosome analysis revealed that TL cells were highly heterogenous and had a female aneuploid karyotype with a hypotriploid mode. Unlike trophoblastic cell lines established from neoplastic tissues, TL cells did not synthesize human chorionic gonadotropin or other gonadal hormones, and only a small amount of ferritin (40.3 ng/10(6) cells) could be detected in the cell supernatant and cell extract. Based on various morphological and histochemical criteria, we suggest that the TL cells are derived from the Langhans cells (villous cytotrophoblast), and due to their special features, the cells may be valuable for the study of the differentiation and tumorigenesis of trophoblastic cells.

The impact of detubularization on ileal reservoirs.

For many years, parts of the large or small bowel have been used for bladder augmentation and substitution. Recent controversy over the advantages and disadvantages of continent urinary diversion using detubularized ileum (the Kock pouch) and tubular ileum (the Camey procedure) focussed on how a highly compliant urinary reservoir should be formed. We compared the compliance of isolated intact ileal segments and ileal pouches constructed after transection of the antimesenteric border. Hydrodynamic data was obtained at four different points in time: acute (immediately after pouch construction), and after two, six and twelve weeks. Over the first six weeks the reservoirs were connected to the bladder for drainage. At six weeks, subtotal cystectomy and separate anastomosis of the tubular ileal loop and the detubularized ileal pouch to the trigone was performed to study the influence of cyclic reservoir distention. Statistical analysis of the pressure-volume curves revealed significantly better compliance of the detubularized ileal pouch as compared to the intact ileal segments. The area under the pressure curve values (AUC) were p less than 0.025, p less than 0.02, p less than 0.005 and p less than 0.02 for the acute experiment, after two weeks, after six weeks, and after 12 weeks respectively. Our findings suggest that transection of the circular intestinal wall is an important step in the creation of a good-compliant urinary reservoir.

Urodynamic use of fiberoptic microtipped catheter.

Solid state microtransducers are used widely in urodynamics to record bladder and intraluminal urethral pressures. They are sensitive and capable of giving high quality, reproducible pressure recordings. They do not require the determination of a reference point but they are expensive and fragile, and calibration may be difficult to perform. We describe our experience with a new catheter for pressure recording based on fiberoptic technology. In addition to the advantages of the conventional microtipped catheters, the new catheter is thinner (4F), easier to calibrate and less expensive. Controlled testing of the new catheter was performed in an experimental setting and in clinical trials during urodynamic evaluation. The catheter provides reliable pressure recordings compared to standard microtipped catheters.

Surgical treatment for female geriatric incontinence.

Surgery may be the most effective treatment for incontinence in the elderly female patient. Today, most of these incontinent patients are cured with surgery using the transvaginal approach, an access at low risk (especially for the geriatric female population). Surgery for stress urinary incontinence of all grades is established; however, surgical treatment for urge incontinence due to bladder instability is still controversial.

Establishment and characterization of a new human lymphoma-derived cell line--HBL-2.

A new human B-cell lymphoma-derived cell line, designated HBL-2, was established from peripheral blood of a 17-year-old male during terminal leukemic phase. HBL-2 cells were single cell suspensions when cultured in RPMI 1640 with 10% fetal calf serum promoting a doubling time of about 20 hours. Cell marker studies revealed Ia and surface immunoglobulin positive, cytoplasmic immunoglobulin and Epstein-Barr nuclear antigen negative. Chromosome analysis showed a male aneuploid karyotype with 14q+ and other abnormal markers. Under electron microscopy, the cells exhibited clear nucleoli, fine chromatin, and were in a primary differentiated state. The establishment of HBL-2 cell line provides a powerful tool in studying the oncogenesis of B-cell malignancy.

The role of astrocytes in the formation of cartilage in gliomas. An immunohistochemical study of four cases.

The occasional presence of focal cartilage in gliomas is generally attributed to metaplasia of the mesenchymal supportive elements. While this mechanism undoubtedly exists, the present report describes a different mode of development of cartilage in four gliomas occurring in young individuals. Two of the tumors were pontine astrocytomas, one was a mixed ependymoma and astrocytoma involving the fourth ventricle and the brainstem, and one was an extraspinal malignant astrocytoma in the lumbar region of a young boy who earlier had been diagnosed as having a pontine glioma for which he received radiation treatment. In all four tumors, transitions from astrocytic to cartilaginous elements were seen, characterized by an increasing deposition of chondroid ground substance between the astrocytes and a gradual morphologic changes of the glial cells to more rounded forms with a vacuolated cytoplasm, indistinguishable from chondrocytes of mesenchymal origin. Many of these cells retained positive staining for glial fibrillary acidic protein by the immunoperoxidase method, attesting to their astrocytic nature. The production of cartilage by neoplastic astrocytes may be related to their ability to secrete, in certain circumstances and occasionally in large amounts, basement membrane material and other forms of mucopolysaccharides, which may become condensed to form a chondroid ground substance. The process appears analogous to that of cartilage formation by epithelial cells in pleomorphic adenomas of the salivary glands.

Establishment and characterization of a human monocytoid leukemia cell line, CTV-1.

A new human monocytoid leukemic cell line, CTV-1, was established from a patient with relapsed acute monoblastic leukemia. The characteristics of this cell line were evaluated by morphologic and cytochemical analyses, electron-microscopy, chromosome study, surface marker analysis and a study of differentiation potential with tumor-promoting agents.

Immunological classification of T-cell acute lymphoblastic leukaemia using monoclonal antibodies.

We had been able to classify 70 cases of acute lymphoblastic leukemia (ALL) into different immunological subtypes based on surface marker analysis with the aid of a panel of monoclonal antibodies. The relative proportions of different ALL subclasses appears to be similar to those reported from Europe and the U.S.A. Our results also indicated that monoclonal antibodies OKT11, UCHT-2 and WT1 were very useful in defining pre-T or early T subtypes of ALL. The latter leukaemias lack most maturation-linked T-cell markers but share with the other T-ALL the characteristic of low density or undetectable insulin receptors.
