[Thrombopenia induced by heparin in orthopedic surgery. Apropos of 12 cases].

Treatment with heparin for the prevention of thrombosis in orthopaedic surgery may sometimes be responsible for thrombopenia. This can be associated with arterial or venous thrombotic complications. Twelve cases of heparin-induced thrombopenia are reported here. Three cases were demonstrated by a diminution of the platelet count which reversed when heparin treatment was discontinued. Nine patients presented with arterial and/or venous thromboses whose progress was favourable in the long term. The replacement of heparin treatment by anti-vitamin K from the 5th or 6th day can avoid the thrombotic complications of heparin-induced thrombopenia.

[Arthroplastic resection of the shoulder. Results and indications apropos of 35 cases].

The only aim of this retrospective study of 35 patients who had undergone resection of shoulder is to report overall results of this well established operation. Although no result could be rated as excellent, those in one half of the patients were evaluated as good and in one quarter as acceptable. The shoulder can be used in two-thirds of cases and of major importance is that pain is absent in two-thirds and tolerable in 80% of cases. In addition to cases of septic arthritis and poorly tolerated recurrent luxations, this operation is indicated for elderly patients with a fracture-luxation or compound fracture of upper end of humerus. However, the operation requires very strict conditions and re-attachment of the cap of rotator muscles. Resection of shoulder should therefore conserve a place in the therapeutic arsenal of shoulder disorders.

[Technical development and radiological surveillance of the Schmitt minimadreporic uncemented total hip prosthesis. Evaluation of 275 arthroplasties with a follow-up of more than 6 years for the oldest cases].

The authors describe a porous-coated uncemented total hip prosthesis which has been used for more than six years: the Schmitt minimadreporic prosthesis. A homogenous series of 275 arthroplasties has been reviewed with a mean follow-up of three years and five months. The method of radiological assessment of the position of the femoral and acetabular components and their possible loosening is described in detail. Infection (0.72%) and aseptic loosening (1.8%) specific to this type of prosthesis was rare. For the series as a whole there were 92% of good, very good and excellent results and for 81 arthroplasties with a follow-up greater than four years the figure was 90%. The frequency of secondary displacement of the femoral component is emphasised, with 15 prostheses sinking downwards and 96 displacing into varus. Statistical analysis has shown the principal factors affecting the clinical result and has demonstrated the effect of resorption of the calcar in causing downward displacement of the femoral stem and insufficient filling of the medullary cavity on the development of varus angulation of the prosthesis.

[The articular cartilage of the patella in scanning electron microscopy].

The authors have studied 50 samples of patellar cartilage, normal and pathological, obtained by biopsy, using a scanning electron-microscope. The normal cartilage shows characteristic regular wave-like undulations. The size of these waves varied between 5,000 and 7,000 nm and the undulations were linked by regularly-spaced fibrillary bridges. In a case with an osteochondral fracture, the fracture site was covered by peripheral cartilage, as had previously been shown by Ghadially. Eighteen cases of patellar chondropathy were divisible into three groups: group I (four cases) showed fibrillary chondropathy, of cerebriform appearance and without fibres. Group II (12 cases) showed a fibrillary chondropathy with loss of geometrical regularity and a staghorn appearance. Group III (two cases) showed an oedematous chondropathy with destruction of fibres. In group II, the presence of bundles of fibres with microfractures indicates a potential for progress to frank arthrosis.

[Urinary concentration of cobalt and chromium in patients with a non-cemented total hip prosthesis].

The concentrations of cobalt and chromium have been measured in the urine of 22 patients who had received uncemented hip prostheses made from cobalt, chromium and molybdenum alloy, and in 21 patients who had not received prostheses, using absorption spectrophotometry. The results were statistically analysed using the Wilcoxon test. The levels of cobalt in the urine in the patients with uncemented prostheses did not show a significant increase, indicating that there is no greater risk of cobalt toxicity. The concentration of chromium was significantly raised (p less than 10(-2) in the urine of patients with uncemented prostheses, and in three of these patients significant corrosion had occurred.

Anatomical basis for study of acetabular migration of the prosthetic cup used in arthroplasty of the hip: a new technique for measurement of migration.

A retrospective study was carried out on 96 partial hip prostheses. Results of this study led us to define new radiological criteria for evaluation of the anatomical position of the prosthetic cup in the acetabulum. A method of measurement of the migration of the prosthetic cup in the acetabulum is proposed based on the results of roentgenological and anatomical study. The anatomical landmarks, geometric constructions and methods of computation of such measurement are described. The application of this technique to radiological/anatomical study was found to be reliable and allowed the determination of prognostic factors indicative of poor acetabular tolerance of the prosthetic cup.

[Solitary spinal osteochondroma. Apropos of 4 cases].

The spinal osteochondroma has been studied in 4 personal cases. This benign tumor unfrequently develops on the spine and even less insolitary. In the 4 cases reported, the sites of the tumor were thoracic (2 cases) cervical (1 case), cervicothoracic (2 cases) which are most frequently mentioned in publications. The tumors were located either on the "posterior arc" either on the "antero-lateral" region of the vertebral body on the level of the "costo-transversary" angle. The clinical manifestations were a growth of a tumefaction, spinal pain and sometimes radicular or medullary neurological signs. The X-Ray image is evident. The best diagnostic procedure is C.T. Scan which determines the exact position and size of the tumor. Surgery is the best treatment. The excision must be large and the pedicule totally removed in order to avoid recurrence. This occurred in one of the cases reported. The osteochondroma becomes rarely malignant. If the tumor involves several spinal segments, post-operative X-Ray investigations must be done for a long period in order to watch out for recurrence.

Therapeutic aspects of limb lesions due to hunting weapons: a study of twenty cases.

The authors report 20 cases of limb lesions provoked during hunting accidents by traditional weapons (gauges 12 and 20) or by 5.5 mm bullets of 22 L.R. Lesions caused by shotguns (gauge 12, 20) are much more serious when done point-blank or at very short shooting distance. The treatment of skin and bone lesions shall be realized in several steps: emergency removal of the foreign bodies with cutting out of non viable bone fragments. Immobilization and depending on the development, two therapeutic solutions may be chosen: either skin graft then bone graft or Papineau's method. Lesions caused by 5.5 mm bullets are much less serious because losses of substances are less important.

Aseptic loosening of cemented total arthroplasties of the hip in relation to positioning of the prosthesis. New utilization of the Tschuprow-Cramer statistical test.

A statistical study of 1200 McKee-Farrar cemented total prostheses of the hip demonstrated the importance of a horizontal position of the cup and a valgus position of the prosthetic stem, in avoiding aseptic loosening of the prosthetic parts. A new test (the Cramer test) showed that a statistical bond exists between the position of the prosthesis and the occurrence of aseptic loosening.

[Osteosynthesis for fracture of the trapezium. One case (author's transl)].

Fractures of the trapezium are rare but their clinical and radiological features are perfectly known. By contrast, their treatment (orthopaedic or surgical) is less clearly defined. The authors wished to make their contribution to the study of this fracture by presentation of a case of fracture-dislocation of the 1st radiation of the hand. Such lesions are generally unstable and require osteosynthesis. In the present case, the latter was facilitated by the use of mini-screws which made it possible to begin physiotherapy almost immediately and to obtain an excellent result.

[The childs-philips intestinal plication. A long term follow-up of 11 patients (author's transl)].

The authors report a long term follow up of 11 children who underwent a Childs-Philips intestinal plication from one to fourteen years ago. One death was caused by this operation. Besides, the tolerance of the Childs-Philips operation seems to be good, the nine other patients present a normal growth without any recurrence of the occlusion. The authors emphasize the interest of this bowel plication in the treatment of occlusion during the childhood.

Arthroscopic assessment of full-thickness rotator cuff tears.

SUMMARY: To evaluate the reliability of the arthroscopic assessment of full-thickness rotator cuff tears, 117 cases were prospectively investigated by imaging, arthroscopy, and open surgery. The confidence of the surgeon, his accuracy, and the surgeon-dependent character of arthroscopic assessment were evaluated in terms of the description of the main anatomic parameters. The surgeons were confident and accurate in diagnosing a full-thickness tear of the supraspinatus, but they underestimated its coronal and sagittal extent and its reducibility to the greater tuberosity. Conversely, the technique appeared very accurate in describing the rotator interval. Endoscopic assessment was particularly operator-dependent in the anteroposterior analysis of the tear. This study shows the limits of endoscopic assessment of full-thickness rotator cuff tears. It illustrates the need for an adequate arthroscopic technique with a thorough knowledge of normal and pathological anatomy of the rotator cuff.
