[Perspectives and time as relevant phenomena in the psychopathology of obsessive-compulsive neuroses].

Perspectivity and time are central terms of the process of cognition in the phänomenological philosophy and anthropology. In this context, "perspective"' means the terms of freedom, act, orientation and sense. The "world of obessional neurotic" however is characterized by the lack of intersubjective relations, by loss of orientation and senselessness. The time, which the obessional neurotic experienced not as continual protention, but by compulsive rituals and compulsions of control, did deprive him of each liberal relation to future. From this context implications for a psychotherapeutic relation can be derived.

[Psychoanalytic treatment of anxiety disorders. Therapy concepts and results of evaluation research].

Since the beginning of the eighties, a change of paradigm concerning anxiety disorders has taken place. It has its roots in new international classifications such as DSM-III-R or ICD-10 and in the biological concept of Panic disorder. Psychoanalytic treatment of anxiety disorders has the longest tradition among all other psychotherapeutic methods. In the main part of this paper the following aspects are discussed: the psychoanalytic theory about etiology of anxiety disorders, the main characteristics of psychoanalytic intervention techniques, findings of evaluation studies, and new psychoanalytic aspects of Panic disorder. In the concluding part the great chance of personal growth through psychotherapeutic coping with anxiety is emphasized.

[Dying in the hospital. Medical and psychological aspects].

For a variety of reasons, the process of dying is taking place ever more frequently in the institution hospital. Apart from the fact that the problem of death continues to be a taboo subject, this is leading to medico-somatic and psychological problems for both the dying person and the so-called "helper". To enable the latter to deal with this existential challenge, he or she must undergo adequate preparation.

[Biographical research methods in psychosomatic medicine, psychology and psychiatry].

This review is centered around the current discussion regarding quantitative vs. qualitative research methodology in psychosomatic medicine, psychology, and psychiatry. In this context, the relevance of biographical methods is discussed. Furthermore, developmental aspects and the importance of biography and individual history in internal and psychosomatic medicine, psychiatry, psychology, and sociology are evaluated. Recent developments in biographical research methodologies such as biographical interviews and case control studies, longitudinal studies and biographical aspects of coping research, are presented as potentially valuable research tools.

[Death and neurosis. Anxiety, the death instinct, object loss and narcissism as the background of the human experience of death].

The present study gives an overview of concepts which try to explain the meaning of death in neurosis. Phenomenological-anthropological and psychoanalytic approaches are drawn into consideration. After an introductory phenomenological description the significance of death in symptomatology, in the inducing situation and in psychodynamic is discussed. The attempts to integrate the phenomenon of death into psychoanalytic metapsychology are represented by means of drive theory and objectrelation-theory. In the "anthropological-integrative" concept of death (D. Wyss) death is proved in the experience of time and separation. D. Wyss emphasized the connection between the experience of death on one side and the development of reflection or the process of structuring consciousness on the other side. Experience of time, separation and thinking are closely related with anxiety which may become another keyword for understanding the meaning of death. Finally the author discusses the question whether Narcissistic Neurosis and Narcissistic Personality Disorders could be connected with the unsolved death problem or a denial of death. The "narcissistic human being" is described as someone who is thrown back on himself because he is not able to cope with his basic anxieties, his limitations and his transitoriness.

[Cardiac anxiety syndrome. Etiologic significance of orientation crises and orientation conflicts and their consequences for therapeutic practice].

In the framework of a phenomenological approach to cardiac neurosis the author investigated 20 patients suffering from this disease regarding their kind of communication disorder and conflicts in order to find out the main conditions for etiology. This study is based on the conception of so-called "Anthropological-integrative Psychotherapy" developed by D. Wyss. He conceptualized fundamental structures of communication representing the structural aspect of a psychosomatic disease and modi of communication in order to show its dynamic aspect. The purpose of this investigation was to examine disproportions, deficiencies, compensatory hypertrophy and conflicts in the six fundamental structures of communication (Living Space, Orientation, Order, Time, Body, Performance). In accordance with many other publications separation anxiety, ambivalence between tie and separation, aggressive restraint, sexual disorders and conflicts in the structure of "performance" are confirmed as significant etiological factors. In addition, conflicts, crises and deficiency of orientation have proved to be of major importance. This is the essential result of this study and an important new contribution to our understanding of cardiac neurosis. The therapies of 3 patients suffering from cardiac neurosis are reviewed to illustrate the three main kinds of disorders in orientation. Finally the therapeutic consequences are discussed. The constitution of new orientation in an intersubjective and dialogic process seems to be an essential aim for psychotherapeutic treatment of cardiac neurosis.

[Significance of the D. Wyss communication mode for the interpretation of dreams in psychotherapeutic practice].

The author illustrates how dreams are interpreted in " Anthropologic -integrative Psychotherapy" created by D. Wyss . A case report of a patient suffering from "cardiac neurosis" is given. Many dreams which were communicated in this therapeutic process are reviewed. The modi of communication pointed out by D. Wyss are used to clarify the communicative potentialities of these dreams.

[Dreams and communication].

Since S. Freud in psychoanalytic theories of dreaming the communicative function of dreaming attained more and more importance. Phenomenological researchers - e. g. L. Binswanger, V. E. v. Gebsattel , M. Boss , D. v. Uslar and especially D. Wyss - consider communication and meeting as origin of dream-experience. They emphasize the possibilities of "being" and "existence" which are inaugurated by dreaming. D. Wyss widened the horizon of understanding by pointing out the relation of dreams to "perspective/ aperspective " and to the category of "possibilities". He describes the therapeutic interpretation of dreams as an intersubjective and dialogic process. The modi of communication created by D. Wyss turned out to be very suitable in order to illustrate the communicative power and potentialities of dreams.

Emotional state, coping styles, and somatic variables in patients with chronic hepatitis C.

The authors in a cross-sectional study examined 113 patients with chronic hepatitis C (CHC) without widely progressed or decompensated liver disease. The patients were investigated for emotional state (depression, anxiety, coping styles) and somatic/sociodemographic variables. A high percentage of patients had positive scores for depression (22.4%) and anxiety (15.2%). Mode of acquisition (e.g., former drug abuse) and histological grade of liver damage had no significant influence on emotional state or coping strategies. Older patients (> or = 50 years) were significantly more depressed (P = 0.024). Patients with a recently diagnosed CHC (> 4 weeks, < 6 months) had significantly lower scores for depression (P = 0.003) and anxiety (P = 0.001) than the subgroup with a time interval since initial diagnosis of more than 5 years. Recently diagnosed CHC patients also showed the highest levels of problem-solving behavior. Patients who were advised not to undergo an interferon therapy were significantly more depressed (P = 0.001) and anxious (P = 0.028). Older patients with CHC and patients with a long period since CHC diagnosis or who were advised not to undergo interferon therapy should be carefully and regularly assessed for depression, anxiety, and inappropriate coping styles.

[Fear of environmental poisons--justified fear or psychiatric disorder?].

A typical anxiety of our time is the anxiety of environmental poisons. Although some of the fears may be reasonable, anxiety diseases and somatoform disorders are important differential diagnoses. The limited value of toxicological and immunological parameters is discussed in depth, and risk factors associated with the development of environmental phobia are presented. This establishes that the concept of "clinical ecology" lacks credible scientific foundation. It must be stressed, that "clinical ecology" may seriously hinder a proper psychosomatic diagnosis and its relevant therapy.

[Coping with the illness by patients with chronic polyarthritis. Forms of coping and therapeutic implications].

In rheumatoid arthritis, coping exerts a major influence on the feelings and the compliance of a patient. This signifies its great clinical relevance. This study presents data of 40 patients using a questionnaire for assessing their ways of coping. The main coping strategies proved to be the search for more information and ways to get cognitive control over the threat imposed by the disease. The individual strategies were not influenced by somatic parameters. Unexpectedly, the duration of disease was tightly correlated to a more intensive approach for gaining information. The results have a major influence on the way of treating patients with RA. They open up new therapeutical regimens for the patient/doctor relationship, including patient oriented seminars and various psychological therapies.
