[Color-coded duplex ultrasonography compared to phlebography in deep leg and pelvic vein thrombosis].

Between March 1990 and March 1991, in a prospective study, 100 patients (53 men, 47 women, median age 61.5 years) with suspected deep venous thrombosis of the lower extremities, colour duplex ultrasonography (CDU) (Ultramark 9, ATL) were investigated using the slow-flow technique. The CDU findings obtained for the external iliac vein (distal segment), the common femoral, the superficial femoral and popliteal veins, together with those of the veins of the lower extremity, were compared with the results of phlebography, which is considered to be the gold standard for this type of investigation. The sensitivity/specificity of CDU in diagnosing deep venous thrombosis was, from proximal to distal: 95.4%/100%, 100%/99%, 96.8%/97.1%, 98.5%/95.5% and 96.3%/98%. Fibrinolytic therapy (ultra high-dose short-term therapy: n = 16 patients; conventional fibrinolytic therapy: n = 4 patients) was indicated in 20 patients. In this group of patients identical results were obtained with CDU and ascending phlebography. Our experience shows CDU to be a supplementary diagnostic procedure to real-time B mode ultrasonic imaging in the establishment of the primary diagnosis, and to be superior to it in the follow-up evaluation of the results of fibrinolytic treatment. Both in the establishment of the primary diagnosis and follow-up of thrombosis of the deep leg and pelvic veins, CDU is the equal of phlebography. The advantage of the latter is in the documentation of the entire venous system and staging of post-thrombotic syndrome.

[Ultrasonographic findings in varicose phlebitis of the great saphenous vein. Evidence of thrombus growth and detachment with asymptomatic lung embolism].

In five women (mean age 58 [31-74] years) with clinically diagnosed varicose phlebitis of the long saphenous vein appositional thrombus growth was demonstrated by serial ultrasonography (B-mode compression and colour-coded duplex sonography). In one patient the appositional thrombus was found to be free-floating as far as the common femoral vein, but this was not seen by phlebography. During therapeutic heparinization there was ultrasonographic evidence of softening and partial liquefaction of thrombus material in a cranial direction. There were no clinical signs of pulmonary embolism, but pulmonary perfusion scintigraphy demonstrated perfusion deficit characteristic of emboli. Four of the five patients had a surgical crossectomy and partial saphenous vein resection. The congruence between ultrasonographic and macro- as well as micropathological findings demonstrates the great value of B-mode and duplex ultrasound examination in the area of the epifascial veins. Ultrasonography should be performed in every case of varicose phlebitis of the large veins to exclude the presence of apposition thrombi.

[Unapparent course of pancreatitis with pseudocyst development and cholestatic jaundice in anticonvulsive therapy with valproate].

Valproic acid is a reliable and comparatively safe drug in the treatment of special forms of absence seizures. Severe but very rare complications include acute liver necrosis and acute pancreatitis which may end lethally. A case of a previously unreported association of pancreatic pseudocyst and development of cholestatic icterus in conjunction with valproic acid therapy is presented. The clinical signs of icterus and chronic pancreatic failure disappeared when we terminated the therapy.

Response of plasma catecholamines and blood pressure to clonidine and to sudden withdrawal of the drug in subjects with essential hypertension.

In this retrospective study in ten patients with essential hypertension a short-time treatment of 3 days with 0.15 mg clonidine 2 times/day orally, as well as a second short-term treatment period with the slow-release oral preparation (.025 mg clonidine once/day) significantly lowered blood pressure, heart rate, and plasma concentration of norepinephrine in the supine and standing subject. After sudden withdrawal of clonidine all these parameters returned to pretreatment levels within 2-3 days. No rebound or overshoot phenomenon was observed.

[Increase of renal failure after treatment of malignant hypertension by captopril (author's transl)].

In a 25-year-old woman with malignant hypertension based on primary nephrosclerosis, captopril, an oral inhibitor of angiotension I-converting enzyme, lowered blood pressure effectively, which had been resistant previously to other oral antihypertensive drugs. Concomitantly with the decrease of the high blood pressure, an increase of creatinine from 2.1 mg% to 8.5 mg% occurred. There is evidence that this increase of renal failure is a consequence of blood pressure reduction and is not caused by a toxic effect of captopril.

[Reversible calcification of skeletal muscles in acute renal failure (author's transl)].

The clinical course in two patients with acute renal failure and focal calcifications of skeletal muscle are reported. In the first case renal failure was due to a hypovolemia or shock combined with supercooling and alcoholic intoxication. In the second case a rhabdomyolytic crisis with myoglobinuria occurred. This patient was known to have a McArdle disease. Dialysis treatment was necessary in both cases in order to overcome the oligoanuric phase. Biopsy specimens from biceps muscle showed intense calcium deposits within the necrotic muscle fibres. In the beginning of oliguria remarkable hypocalcemia occurred followed by hypercalcemia. During the polyuric phase which was accompanied by hypercalcemia and hypercalcuria the calcium deposits disappeared completely. This could be demonstrated in our first case by a control biopsy.

[Fibre type and glycoside concentrations of human skeletal muscle (author's transl)].

In 2 patients digitalized with digoxin or betamethyldigoxin, postmortal glycoside concentrations were determined in 7 different skeletal muscle specimens by radioimmunoassay. In the same specimens, planimetric measurements of histochemical fibre types I and II were carried out. There were higher glycoside concentrations in predominantly type I fibre muscle biopsies.

[Effects of longterm HCG administration on testicular function in hemodialysis patients (author's transl)].

Thirtheen male patients with chronic renal failure undergoing regular dialysis treatment (2 X 8-10 hours/week) were treated with gonadotropins (HCG, Primogonyl) primarily 2 X 2,000 IU/week and later 2,000 IU/week. Before HCG administration and during 4-months HCG-therapy testosterone, dihydrotestosterone, Androstandiol, LH and FSH levels were determined by RIA-methods in 7-14 days intervals. Before HCG-application plasma testosterone levels were low and did not increase in the course of regular dialysis treatment. Derivates from testosterone like dihydrotestosterone and Androstandiol were elevated in plasma, presumbably because of accumulation in renal failure. LH-levels were slightly elevated on the average. FSH-levels showed a high individual variation but also seemed to be elevated on the average. HCG stimulation by exogenous HCG administration for short time resulted in a insufficient rise of testosterone levels as compared to normals. During prolonged HCG administration plasma testosterone levels increased to normal but dropped immediatly after cessation of therapy or reduction to less than 2,000 IU HCG twice/week. Body weight, plasma proteins, haematocrite and fertility did not improve significantly. These results indicate that in chronic renal failure androgen synthesis by testicular tissue is seriously impaired and does not improve under usual dialysis treatment. Feedback regulation of testosterone levels by increase of LH levels seems not to be sufficient although pituitary response is found to be normal. This may be explained by elevated levels of testosterone derivates which exert negative feedback effects.

Myopathy in experimental uremia.

M. quadriceps and diaphragm were studied in Wistar rats 2, 3, und 4 weeks after 5/6 nephrectomy. Control animals were sham operated and pair fed. In 10 mu transverse cryostat sections, the following histochemical reactions were performed: NADH-dehydrogenase, myofibrillar ATP-ase, modified trichrom stain, hematoxylin-eosin. Three fibre types (I, II, intermediate) were analysed quantitatively by planimetry. Sarcolemnal nuclei per unit area and fibre cross section area were determined. In muscle specimens from uremic animals, a uniform atrophy of all three fibre types could be demonstrated. In contrast to findings in man, preferential atrophy of one of the three fibre types, increase of sarcolemnal nuclei per fibre cross section area or structural abnormalities within single muscle fibres could not be detected. Neurogenic damage could be excluded (electrophysiology, sciatic nerve planimetry). The fibre changes point to a primary disturbance of muscle metabolism.

[Myopathia myotonica. A new type of hereditary muscle disease (author's transl)].

A new type of hereditary muscle disease, characterized by weakness and painful spasms during effort, without electrical activity in the shortened muscles, is described. These phenomena are limited principally to the upper limbs. In addition we found electromyographical signs of a generalized myotonic syndrome. The histological and histochemical investigations reveal only minimal non-specific signs of myopathy. The activities of CPK and aldolase in the blood serum are increased at times. A normal elevation of venous lactate was observed during ischemic work. The biochemical studies of muscular tissue exhibit normal activities of the analyzed enzymes, especially as regards phosphorylase. An increased concentration of calcium ions in blood serum may be related to the contraction during strenuous work; it is known that calcium ions are an important factor in the contraction-relaxation cycle of striated muscle. The age of manifestation varied from 4 to 33 years in 4 cases of the relatives observed. The disease shows no signs of aggravation as to the severity and extent of the disorders. The nature of the underlying metabolic defect is still unknown.

[Myotonic dystrophy (urological features and histochemical findings in muscle) (author's transl)].

Dilatation of the renal pelvis, ureters and bladder appeared in two brothers with myotonic dystrophy (aged 26 and 29 years), probably as a result of involvement of the smooth muscles of these organs. Histochemical differentiation of skeletal muscle fibres (main fibre types I and II, after Engel) indicated a predominant size reduction in type I fibres on demonstrating myofibrillary ATPase. In addition, demonstration of this enzyme also revealed a marked shift of the fibre type proportions in favour of type I. Isolated reduction in gammaG-globulins was also demonstrated. A defect of cellular and humoral response was excluded.
