Development and evaluation of an air-curtain fume cabinet with considerations of its aerodynamics.

In order to avoid the inherent aerodynamic difficulties of the conventional fume hood, an innovative design--the 'air curtain-isolated fume hood' is developed. The new hood applies a specially designed air curtain (which is generated by a narrow planar jet and a suction slot flow at low velocities) across the sash plane. The hood constructed for the study is full size and transparent for flow visualization. The aerodynamic characteristics are diagnosed by using the laser-light-sheet-assisted smoke flow visualization method. Four characteristic air-curtain flow modes are identified in the domain of jet and suction velocities when the sash remains static. Some of these characteristic flow modes have much improved flow patterns when compared with those of the conventional fume hoods. From the viewpoint of the aerodynamics and mass transport, the results indicate that the air curtain properly setup across the sash opening allows almost no sensible exchange of momentum and mass between the flowfields of the cabinet and the outside environment. Two standard sulfur hexafluoride (SF6) tracer gas concentration measurement methods following the ANSI/ASHRAE 110-1995 standard and the prEN14175 protocol for static test are employed to examine the contaminant leakage levels. Results of the rigorous examinations of leakage show unusually satisfactory hood performance. The leakage of the tracer gas can approach almost null (<0.001 p.p.m.) if the jet and suction velocities are properly adjusted.

Refined localization of dyschromatosis symmetrica hereditaria gene to a 9.4-cM region at 1q21-22 and a literature review of 136 cases reported in China.

BACKGROUND: Dyschromatosis symmetrica hereditaria (DSH) is an autosomal dominant pigmentary genodermatosis characterized by hyperpigmented and hypopigmented macules on the extremities, which has recently been mapped to an 11.6-cM interval on chromosome 1q11-21. So far, most cases of DSH have been reported in Japan and dermatologists around the world might think this disorder mainly occurs in Japan. In fact, there are 17 DSH families including 136 cases reported in China since 1980, but most of them are described in Chinese. OBJECTIVES: To refine the previously mapped region that facilitates the identification of the DSH gene and to delineate the clinical and genetic features of Chinese DSH cases by a literature review of 136 cases reported in China. METHODS: We performed genotyping and linkage analysis using polymorphic microsatellite markers at 1q11-22 in two Chinese DSH families, and reviewed all of the DSH cases reported in China since 1980. RESULTS: A cumulative maximum two-point lod score of 3.68 was produced with marker D1S506 at a recombination frequency of theta = 0.00 in these two families. Haplotype analysis refined the DSH locus to a 9.4-cM interval flanked by D1S2343 and D1S2635. The genetic and clinical features of Chinese cases with DSH were summarized. In some Chinese cases, hyperpigmented and hypopigmented macules were scattered on the neck and chest, but among Japanese patients there were no similar skin lesions to be reported on these sites. CONCLUSIONS: This study confirms linkage of DSH to a previously mapped region and refines the DSH gene to a 9.4-cM interval at 1q21-22. Likewise, the literature review indicates that DSH is not an uncommon disorder in China and the differences in the distribution of skin lesions could be related to race and environment.

Memory CD8+ T cells in heterologous antiviral immunity and immunopathology in the lung.

A potent role for memory CD8+ T cells in heterologous immunity was shown with a respiratory mucosal model of viral infection. Memory CD8+ T cells generated after lymphocytic choriomeningitis virus (LCMV) infection were functionally activated in vivo to produce interferon-gamma (IFN-gamma) during acute infection with vaccinia virus (VV). Some of these antigen-specific memory cells selectively expanded in number, which resulted in modulation of the original LCMV-specific T cell repertoire. In addition, there was an organ-selective compartmental redistribution of these LCMV-specific T cells during VV infection. The presence of these LCMV-specific memory T cells correlated with enhanced VV clearance, decreased mortality and marked changes in lung immunopathology. Thus, the participation of pre-existing memory T cells specific to unrelated agents can alter the dynamics of mucosal immunity and disease course in response to a pathogen.

Familial koilonychia.

A 2-year-old boy with flat and spoon-shaped nail plates was referred to us. His mother had noticed that his fingernails were quite soft when he was born. After 2 months, the nails became flat or spoon-shaped; the toenail plates also became rough and flat. Systemic examination revealed nothing unusual. All his fingernail plates were quite thin, and had concave surfaces; the distal edges of some nail plates were rough and darkened (Fig. 1). His great, second and middle toenails on both sides showed the same abnormalities as the fingernails. Routine laboratory tests were normal; serum levels of iron, zinc, calcium, and magnesium were within normal limits. Repeated microscopic examination and culture of fungi from both fingernails and toenails were negative. Histologic examination of the distal edges of some of his fingernail plates was basically normal. When checking members of the family, we noticed that the boy's mother and maternal grandmother had similar nail problems (out of 14 members of three generations), although no systemic or other skin disorders were noticed. All the fingernails of the boy's mother were lusterless and concave with darkening of the distal edges (Fig. 2). All her toenails were rough, lusterless, and brown; some of them had longitudinal ridges. His maternal grandmother had light brown colored fingernail plates with concave profiles (Fig. 3), and dark brown or even blackish colored toenails. Both the mother and maternal grandmother acknowledged that their nails were abnormal from early childhood. Repeated microscopic examination and culture of fungi from both fingernails and toenails were negative. Several distal portions of the nail plates were collected for histologic examination. The fingernail plates of the boy's mother showed numerous parakeratotic cells in the middle zone, and those of his maternal grandmother showed diffuse parakeratotic cells; melanin granules were not found in those nail plates that were checked. Taken together, a diagnosis of familial koilonychia was established.

A case of Kaposi's sarcoma associated with pemphigoid nodularis.

Pemphigoid nodularis is a rare variant of bullous pemphigoid. Bullous pemphigoid complicated by internal malignancy has been well documented. There have been only a few reported cases of bullous pemphigoid associated with Kaposi's sarcoma. We described a case of pemphigoid nodularis who later developed Kaposi's sarcoma that might be related to immunosupressive therapy, a situation that has not been reported previously.

Attrition of bystander CD8 T cells during virus-induced T-cell and interferon responses.

Experiments designed to distinguish virus-specific from non-virus-specific T cells showed that bystander T cells underwent apoptosis and substantial attrition in the wake of a strong T-cell response. Memory CD8 T cells (CD8(+) CD44(hi)) were most affected. During acute viral infection, transgenic T cells that were clearly defined as non-virus specific decreased in number and showed an increase in apoptosis. Also, use of lymphocytic choriomeningitis virus (LCMV) carrier mice, which lack LCMV-specific T cells, showed a significant decline in non-virus-specific memory CD8 T cells that correlated to an increase in apoptosis in response to the proliferation of adoptively transferred virus-specific T cells. Attrition of T cells early during infection correlated with the alpha/beta interferon (IFN-alpha/beta) peak, and the IFN inducer poly(I:C) caused apoptosis and attrition of CD8(+) CD44(hi) T cells in normal mice but not in IFN-alpha/beta receptor-deficient mice. Apoptotic attrition of bystander T cells may make room for the antigen-specific expansion of T cells during infection and may, in part, account for the loss of T-cell memory that occurs when the host undergoes subsequent infections.

[The clinical application of distally based neurocutaneous flaps by anastomosis of superficial veins].

OBJECTIVE: To investigate the clinical results of the distally based neurocutaneous flap by anastomosis of superficial veins. METHODS: From June 1996, 19 cases with composite skin defects of the distal part of limb were repaired by the transposition of distally based neurocutaneous flaps, including traumatic defect in 10 cases, chronic ulcer in 3 cases, scar contracture in 6 cases. The distally based sural neurocutaneous flaps were used in 9 cases, the reverse-flow saphenous neurocutaneous island flaps were used in 2 cases, and the retrograde neurocutaneous island flaps of the forearm were used in 8 cases. The flap area ranged from 15 x 24 cm to 4 x 6 cm, the pedicle of the flap ranged from 6 cm to 15 cm in length. The superficial vein of the flap were anastomosed with the subcutaneous superficial vein of the recipient site to improve the venous drainage. RESULTS: The composite flap survived completely in 17 cases. One cases with retrograde-flow forearm neurocutaneous flap and another case with reversed sural neurocutaneous flap were partially survived because of thrombosis in anastomosed veins postoperatively. Sixteen cases were followed-up for 6 to 24 months, the color and texture of the flap were excellent, the protective sensation were recovered, the configuration and function were satisfactory. CONCLUSION: Anastomosis of superficial veins of the composite flaps with the subcutaneous superficial veins of the recipient site can significantly improve the venous drainage, enlarge the survival area of the flap and the reparable area.

Inflammatory pseudotumor of the spleen--a case report.

A 36-year-old woman had suffered from epigastric fullness for half a year. A splenic mass was found by ultrasonography. She was treated with splenectomy. Inflammatory pseudotumor of the spleen was diagnosed pathologically. This benign tumor in the spleen is extremely rare. To our knowledge, only 67 cases had been reported in the literature. Recognition of this rare entity is important because it may mimic splenic malignancy clinically and radiographically.

Molecular basis for differing antineurogenic effects of GATA-1a and GATA-1b in Xenopus.

The erythroid transcription factor GATA-1 in Xenopus has been cloned as a pair of presumably duplicated genes designated as xGATA-1a and xGATA-1b. Although both xGATA-1a and xGATA-1b are able to stimulate erythropoiesis, only xGATA-1b is capable of inhibiting neurogenesis in Xenopus embryos. Chimeras of these two genes were constructed by permuting coding and untranslated regions (UTR) on both ends of these two xGATA-1, and their neurogenesis-inhibitory effects were studied. These results reveal that (1) sequence variations between the coding regions alone do not account for the neurogenesis effect; (2) 3' UTR of xGATA-1a causes the loss of the neurogenesis inhibition of xGATA-1b; (3) 3' UTR of xGATA-1b is essential to inhibit neurogenesis. In addition, the presence of either UTR does not affect the stability of the mRNA in vitro. These observations suggest the influence of 3' UTR in xGATA-1 on the inhibition of neurogenesis.

Allele loss on chromosome 9q22.2-22.3 in sporadic basal cell carcinoma in Chinese.

OBJECTIVE: To investigate the role of allele loss on chromosome 9 in the pathogenesis of basal cell carcinoma (BCC) by examining the loss of heterozygosity in sporadic basal cell carcinoma and other cutaneous tumors. METHODS: DNA samples were isolated from the tumors, the matched adjacent normal skin and the blood of patients with sporadic basal cell carcinoma (14), squamous cell carcinoma (25) and Bowen's disease (5) by phenol-chloroform extraction. In our study, we used two microsatellite markers on chromosome 9, one was locus D9S319 (9p21), and the other was D9S299 (9q22.2-22.3). After PCR was performed, the samples were electrophoresed through 5% denatured polyacrylamide gels which were dried and exposed to Fuji XR films. RESULTS: Loss of heterozygosity with D9S319 (9p21) marker was not observed in the 10 informative cases of sporadic basal cell carcinoma, 19 squamous cell carcinoma and 4 Bowen's disease. Allelic deletion of D9S299 was not seen in the 21 informative cases of squamous cell carcinoma and 4 Bowen's disease. D9S299 (9q22.2-22.3) allele loss occurred in 2 of the 10 informative cases of sporadic basal cell carcinoma, indicating that there might be a susceptible tumor suppressor gene on chromosome 9, and loss of this allele might play an important role in the development of basal cell carcinoma. Inactivation of Drosophila patch (PTCH) (MSSE gene ESS1) in BCC might be necessary, if not sufficient, for BCC carcinogenesis. CONCLUSION: Chromosome 9q22.2-22.3 may contain a putative tumor suppressor gene, and the loss of this gene may play an important role in the development of sporadic basal cell carcinoma.

[The effect of combined treatment of recombinant human growth hormone and insulin-like growth factor-1 on wound healing and protein catabolism in burned rats].

OBJECTIVE: To investigate the effect of combined treatment of recombinant human growth hormone (rhGH) and insulin-like growth factor-1 (IGF-1) on wound healing and protein catabolism in burned rats. METHODS: Forty Wistar rats with deep II degree scald injury were divided randomly into four groups and received rhGH (0.1 U/kg.d), rhGH (0.1 U/kg.d) plus IGF-1 (2.0 mg/kg.d), IGF-1 (2.0 mg/kg.d) and Ringer's solution (2 ml/kg.d, as control group) respectively. The wound healing time and protein catabolism levels of every groups were compared after 2 weeks. RESULTS: Total body weight began to increase after 2 weeks in rhGH group and rhGH plus IGF-1 group, but in control group, it was occurred after 4-5 weeks. The body weight of rhGH plus IGF-1 group was 1.65 times than that of rhGH group. The wound healing time in rhGH plus IGF-1 group (17.1 +/- 4.4) days was significantly lower than that of rhGH (20.5 +/- 4.8) days and control group (29.7 +/- 6.3) days. The protein level of rhGH plus IGF-1 group was significantly higher than that of control group and rhGH group. CONCLUSION: It suggests that rhGH plus IGF-1 with synergism is more effective in promoting wound healing and increasing the protein catabolism.

Transcriptional regulation of BMP-4 in the Xenopus embryo: analysis of genomic BMP-4 and its promoter.

Recent experiments in the Xenopus embryo suggest that proper regulation of BMP-4 signaling is critical to the dorsal ventral specification of both mesoderm and ectoderm. Regulation of BMP-4 signaling is known to occur extracellularly by direct binding with chordin, noggin, and follistatin, and intracellularly through the antagonistic signal interaction with dorsalizing TGF-beta family member activin. However, tight repressional regulation of BMP transcription may also be required to sustain the dorsal and neural status of the induced cells. Here we demonstrate that the dominant negative mutant of the BMP receptor (DN-BR) or the BMP-4 antagonizers, chordin and noggin, negatively regulate BMP-4 transcription in animal cap explants. We suggest that repression of BMP-4 transcription is important in the maintenance of dorsal fate and that continuous input of BMP-4 signaling is required to sustain the expression of BMP-4 transcription in the maintenance of epidermal/ventral fate. Consistent with this postulation, we found that the promoter region of the isolated BMP-4 genomic DNA includes several consensus binding sites for transcriptional regulators functioning under BMP-4 signaling such as GATA binding and ventralizing homeobox genes. In a functional assay we found that the GATA binding and ventral homeobox proteins can positively modulate BMP-4 promoter activity. We also observed that DN-BR decreases BMP-4 promoter activity. This was likely due to a repression of the above-mentioned transcription factors. The significance of these observations to embryonic patterning is discussed.

Evaluation of global and regional left ventricular function using technetium-99m sestamibi ECG-gated single-photon emission tomography.

We evaluated a method for the assessment of left ventricular (LV) function with technetium-99m sestamibi ECG-gated single-photon emission tomography (GSPET). GSPET was performed at rest in 21 patients. Images were reconstructed to obtain end-diastolic (ED) and end-systolic (ES) images. Endocardial and epicardial edges of the left ventricle for the ED and ES images were defined using the gradient images and the algorithm developed. LV wall thickness was measured for the mid-ventricular slices of ED and ES images at 10 degrees intervals. The systolic thickening (ST) and the LV ejection fraction [LVEF(GSPECT)] could be determined. These values were compared with the LV ejection fraction (LVEF) estimated using the gated blood pool method. There was a linear correlation between LVEF and ST (r=0.79), and LVEF and LVEF(GSPET) (r=0.87). Histograms of LV thicknesses were generated. Agreement for evaluation of regional wall motion between the method using histograms of LV thicknesses and the gated blood pool study was 92.8% (kappa=0.75). It is concluded that with an appropriate method for LV edge detection, GSPET with 99mTc-labelled perfusion agents is of use for simultaneous evaluation of myocardial perfusion and assessment of LV function.

Prevalence of diabetes mellitus and impaired glucose tolerance in Aborigines and Chinese in eastern Taiwan.

The aim of this study was to determine the prevalence of diabetes mellitus and impaired glucose tolerance (IGT) in three different ethnic groups in eastern Taiwan. The study was performed among Han Chinese, aboriginal Ami and aboriginal Atayal in six rural villages. Inhabitants aged 40 years and over were invited to participate. A 75-g oral glucose tolerance test (OGTT) was used as suggested by the World Health Organization (WHO) for population screening purposes. WHO criteria were used for determining the results. A total of 1013 adults (460 men and 553 women) were examined with a response rate of 62.1%. There were no significant differences in sex, age and history of diabetes between responders and non-responders. The age-adjusted prevalence of diabetes was 11.0% (9.8% in men and 12.3% in women) in the Chinese, 9.1% (12.1% in men and 7.4% in women) in the Amis, 10.8% (11.5% in men and 9.4% in women) in the Atayal and 9.9% (11.5% in men and 8.5% in women) for the two aboriginal groups together. Although sex differences in prevalence of diabetes were not statistically significant, it seems that diabetes was slightly more prevalent in Chinese women than in Chinese men (12.3 versus 9.8%). Nonetheless, the prevalence rate of diabetes was somewhat higher in the aboriginal males than in the aboriginal females (12.1 versus 9.8% for Ami and 11.5 versus 10.7% for Atayal). The prevalence of IGT was 7.2, 8.5, 10.8 and 9.7% for the Chinese, Amis, Atayal and the two aboriginal groups, respectively. There were no substantial differences in the prevalence of diabetes and IGT among these three ethnic groups. Environment, rather than ethnicity, was more likely the predominant factor. It was found that the prevalence of diabetes in this study was about twice as high as that reported in a 1987 survey in Taiwan. The discrepancy may be due to different screening methods and assessment periods.

Presence of human papillomavirus DNA and expression of L-fucose moiety in some vulvar intraepithelial lesions and vulvar squamous cell carcinoma.

BACKGROUND: Human papillomavirus could reside and play an etiological role in some vulvar epithelial lesions. Human papillomavirus-infected keratinocytes might have certain biochemical changes that could be of significance in helping clinical diagnosis or elucidating the pathogenesis of some vulvar epithelial diseases. OBJECTIVE: To detect the presence of human papillomavirus DNA and observe the expression pattern of L-fucose moiety in some vulvar intraepithelial lesions and vulvar squamous cell carcinoma. METHODS: Nineteen cases of vulvar intraepithelial lesions and 13 cases of vulvar squamous cell carcinoma from the inner aspect of labia majora or the outer aspect of labia minora were selected. Polymerase chain reaction was employed to detect the presence of human papillomavirus DNA in lesional skin. Ulex europeaus agglutinin-1 histochemistry was used to observe the L-fucose expression pattern. RESULTS: A large proportion of vulvar intraepithelial lesions had the presence of human papillomavirus DNA, but the positive rate was low in vulvar squamous cell carcinoma and squamous cell hyperplasia. L-Fucose expression was much more pronounced in human papillomavirus DNA-positive lesions than those negative ones. CONCLUSIONS: Human papillomavirus had an important impact on some vulvar intraepithelial lesions in this Oriental population. The expression pattern of L-fucose may be used as an indicator for vulvar keratinocyte transformation by human papillomavirus.

Characterization of a prolactin-synthesizing rat pituitary cell line immortalized by the temperature-sensitive mutant SV40.

A prolactin (PRL) synthesizing rat anterior pituitary (AP) cell line named tAP-CLC-2 was established by transformation with mutant temperature-sensitive A (tsA) simian virus 40 (SV40). The transformed cells were temperature-sensitive for morphology, cell propagation, and expression of phenotypic genes. Monolayer primary rat AP cells were transfected with a mutant SV40 (tsA 205) at 10 times the multiplicity of infection (MOI) at 34 degrees C. After cell clones formed, each clone was isolated and transferred to separate flask until sufficient cells were grown for freezing and characterization. At the permissive temperature (34 degrees C), these cells were spindle-shaped and grew rapidly like tumor cells. However, at the nonpermissive temperature (40 degrees C), the cells exhibited a rounded shape and ceased to propagate because the gene for maintenance of transformation was not expressed. Cell extracts from the cell line tAP-CLC-2 showed an inhibition curve parallel to that of normal rat pituitary cell extracts in a PRL radioimmunoassay (RIA). The gel filtration profile of immunoassayable PRL obtained from fast performance liquid chromatography (FPLC) showed that tAP-CLC-2 cell extract exhibited a PRL peak coincidental with primary rat pituitary cell extracts. Western blot showed that cell extracts from the tAP-CLC-2 cell line and from normal rat pituitary glands shared a similar, major immunoreactive PRL band. The tAP-CLC-2 cell line was responsive to estradiol (E 2; 10(-7) M), progesterone (10(-8) M), and gonadotropin-releasing hormone (GnRH; 10(-9) M) treatments. These hormonal treatments increased (p < 0.05) cell PRL content. Interestingly, treatment with a high dose (10(-7) M) of ovine luteinizing hormone (oLH) also increased (p < 0.05) PRL content in the cell line; a low dose (10(-9) M) of oLH did not. The cell line appeared to synthesize growth hormone (GH) as well. These results indicate this cell line has properties shared by primary AP cells and can provide a unique model for the study of the synthesis and gene regulation of PRL in vitro.
