[Light and scanning electron microscopy study of the pathogenesis of pilonidal sinus and anal fistula].

From various theories about the development of the pilonidal sinus Stelzner's opinion as to an acquired disease is generally accepted. Our scanning electron microscopic investigations strengthen this theory that hair splits and drills into the skin. Hooks keep it from retracting. Needlelike sharp ends may contribute to the hair piercing the skin. The anal fistula differs from the pilonidal sinus concerning morphology and pathogenesis. Surgical treatment must consider the localisation of the proctodeal gland.

[The incidence of recurrent laryngeal nerve paralysis following thyroid surgery].

We examined the histories of 789 thyroid resections for the influence of the type of disease and the kind of operative procedure on the frequency of recurrent laryngeal nerve lesions. In operations for benign goiter the amount of recurrent nerve palsy was 1.8%, in cases of recurrent symptomatic goiter it was 9.5%. In cases of total thyroidectomy for carcinoma it was 10% (7.9% when related to the number of nerves that could be damaged). By literature review and own results a dissection of the recurrent nerves usually does not seem to offer any advantages.

[Clinical application of a totally implantable catheter system].

A central-venous, totally implantable catheter system was used in 56 patients, aged 7 months to 75 years, predominantly for cytostatic treatment. The observation period was maximally 22 months. Infections occurred in four patients with leukaemia and severe agranulocytosis (after faulty puncture, and after rupture of the connection between catheter and injection port). Long period of usability with low complication rate distinguishes the system from the conventional central-venous catheter one.

[Therapy of hydrofluoric acid burns].

Treatment of hydrofluoric acid burns differs very much from treatment of other acid burns. Incorrect anamnestic data caused a delayed specified therapy on one of our patients, who had a cardiac arrest several hours after the accident. We suggest to control serum-electrolytes frequently in every major acid burn and have a chemist analyse the acid at the site of the accident, even if the anamnestic statements seem "reliable".

[Peridural use of buprenorphine. Case report].

This case-report of a 78-year old patient, suffering from extreme pain caused by metastatic formation and pathological bone fractures, shows, that the epidural injection of buprenorphine is a good alternative compared to other means of pain-treatment. During almost four days heart rate, blood pressure and breathing frequency, as well as the general reactions of the patient, were observed. Few side effects and little discomfort make the method advisable for patients in poor general condition.
