[The physician as a legal expert witness].

Medical expertising is becoming more frequent in courts mainly because of the increase in liability lawsuits against physicians which is due, among other things, to the perfectionist trend in medical technique and an excessive claimant mentality of patients. All this results in an enhanced importance of the court-appointed medical expert, with which he can cope only if he considers himself to be an "assistant to the judge". The article describes his position and responsibility and also his possible liability.

[Gynecologic tumors in childhood and adolescence].

These tumors, although rarely found, are of high clinical interest because of the difficulty of diagnosis and the problems of therapy. Symptoms which lead to the (provisional) diagnosis of tumors in female adults such as atypical bleeding, virilisation, growth of the abdominal circumference and the like are often ignored or not recognized in childhood. As a result, therapy often is started too late. More complications and greater difficulty of therapeutical measures such as operations, radiation and chemotherapy are encountered in childhood than in adult life. Frequency and morphology of gynecoligic tumors are described and diagnosis and therapy are discussed.

Adolescent

[Unexpected findings in laparotomy. Clinical and morphological aspects of the malignant hemangiopericytoma (author's transl)].

A case of hemangiopericytoma with a component of fibrosarcoma is reported. The tumour was diagnosed three years prior to the first operation which was done for increasing growth of the tumour. Three years after the first operation and radiotherapy a local recurrence was removed. Histology of the tumour showed a pure fibrosarcoma. The patient remains well for more than one year after the second operation. Because of the prolonged course without general spread is questionable that retroperitoneal tumours show the same biological propensities as other fibrosarcomas.

[Hormone dependent, functional gynecologic morphology in infancy and adolescence presented on the example of the cervix uteri].

Signs of different activities in the dynamic morphology of the epithelium and mucus of the cervix uteri are demonstrated in the female fetus, newborn, child and adolescent. Besides the morphological description of the ectropium, transitional zone, s.c. "reserve-cells", squamous cell nodes and production and secretion of mucus other investigations were done by allometry, caryometry and histochemistry. The examinations allow insight into the hormonal activities specially of the estrogenes in different periods of female life.

Adolescent

[Adeno-epidermoid carcinoma of the cervix. An opinion on the so-called cervical adenocancroid].

The so-called adenocancroid of the cervix represents a relative rare type of tumors. The diverse morphological criterions are shown on hand of 10 personel observations and are distinguished between forms of combination and collision. The histological findings of these epithelial mixforms motify a closer occupation with the formal genesis of this cervical carcinoma. In the formal genesis of the cervical carcinoma. In the formal genesis of carcinoma of the cervix, a decisive role is being played by the proliferation of differentiated intermitotic cells (reserve cells) in the course of indirect metaplasia. Consequently the majority of carcinoma of the cervix, are not squamous-cell-carcinoma, but bipotent-cell-carcinoma. The bipotent qualities of these cells are also maintained in malignant degeneration and enable in rare cases a secondary differentiation running in two different directions. The confusing denotation "adenocancroid" should be replaced by the definition "adeno-epidermoid-carcinoma of the cervix".

Adenocarcinoma

[Pregnancy in teenagers (author's transl)].

In this study pregnancy, delivery and the newborn infant of 151 juvenile primiparas and 1034 adolescent primiparas were analyzed and were compared with a group of 3563 primiparas 21, 23 and 25 years of age. Regarding the obstetric history and the intrapartum period there was no increase incidence of complications among the young patients, however we found among the older women an increase in the incidence of operative deliveries. In the postpartum period it was noted that the young primiparas had a higher incidence of postpartum bleeding caused by atonia of the uterus and also an increased incidence of placental complications with a high rate of manual placental extraction and curettages. A positive correlation was found between premature birth and perinatal mortality and the younger the age of the mother. The newborn of the young mothers was also lighter and shorter at birth and had a higher incidence of malformations. This study has shown, that juvenile expectant mothers are a risk group for obstetricians and pediatricians.
