Rapid detection of Legionella antigen in tissues and body fluids.

We report on a study in which a modification of the enzyme-linked immunosorbent assay (ELISA) using solid-phase microtiter plates has been applied to detect Legionella pneumophila (serogroup 1) antigen in urine, secretions and tissue homogenates. The radioimmunoassay (RIA) and ELISA techniques were compared with respect to sensitivity and range of detection of individual species' antigens. We present results of adsorption studies in which positive specimens were treated with specific IgG fractions raised in the rabbit against individual Legionella species. The advantage of the rapid ELISA or RIA diagnosis over the more time-consuming diagnostic measures is discussed.

Rapid detection of pathognomonic blood cells in patients with infectious mononucleosis.

Veneous blood-citrate samples of patients with infectious mononucleosis were stained with DAPI, a newer fluorochrome (4',6-diamidino-2-phenylindole) to demonstrate the "large atypical lymphocytes" associated with this disease. As a result of the capacity of DAPI to stain DNA and certain acid mucopolysaccharides, a rapid detection of the pathognomonic white blood cells could be achieved. Most of them revealed the well known indented or lobulated nuclei and vacuolated (foamy) cytoplasm. Others exhibited kidney-shaped nuclei, often filled with yellow-fluorescent tiny granula. In our view, the rapid, specific and sensitive DAPI-technique for detecting pathognomonic blood cells in patients with infectious mononucleosis can be considered an improved microscopic method.

[Chronic Meningococcemia--case report and pathogenetic discussions (author's transl)].

The clinical course of chronic meningococcemia in an alcoholic patient is described. Serum IgG-, IgM- and IgA- antibody titers were followed by indirect immunofluorescence using the homologous bacterial isolate as antigen. The patients serum titers were compared with those obtained from other patients with either persistent meningococcemia or meningitis. The data revealed that antibody titers rose in both cases. The yet unexplained finding that the homologous bacterial strain persists in the bloodstream though high antibody titers were detected is discussed.

[Sepsis - pathogenesis and clinic].

Sepsis is a pathogenetically homogeneous, but phenomenologically very different syndrome on whose clinical appearance numerous factors may have influence. It is determined by the triad: focus of the sepsis, presence of an infectious agent in the flowing blood and septic settlements. Only the knowledge of the pathogenetic connections renders the prerequisite for the understanding of symptomatology and picture of the course and allows the adequate diagnostic and different therapeutic measures which must be adapted to every individual case.

[Purulent meningitis in older children and adults (author's transl)].

Purulent meningitis is a clinical syndrome whose appearance is particularly influenced by heteropathogenesis and polyetiology in addition to a multitude of individual factors. Symptomatology and type only become comprehensible, diagnosis, therapy and prognosis only become reconciled when the individual pathogenetic connections have been grasped and taken into consideration. The fate of the by no means rare meningitic diseases is so closely related to the professional experience of the doctors treating the case that quite certainly their degree of experience must also be evaluated as a prognostic parameter.

[A case of chloroquine-resistant (R1) falciparum malaria from the East African Comoros Islands (author's transl)].

An African from the Comoros Islands, who has been living in Berlin for eleven years and who had made his last journey to Africa two years before, fell ill with high fever upon returning from a visit to Mayotte and Grande Comore. His blood smear revealed trophozoites of Plasmodium falciparum. After intake of 25 mg of chloroquine base per kg body weight within 66 hours, defervescence and clearance of blood ensued and by the fourth day after start of treatment the parasites could no longer be detected in the blood. On the 25th day high fever occurred and P. falciparum was found again. After chloroquine treatment had been repeated with an additional single dose of 75 mg pyrimethamine and 1.5 g sulphadoxine the patient remained healthy. During observation over a period of eight months plasmodia could no longer be detected in thick films.

[Bacteremia associated with subfebril urinary tract infections (author's transl)].

Blood samples of 115 patients of both sexes admitted to the hospital with urinary tract infections were examined for bacteremia. Out of 115 patients 12 (10%) had positive blood cultures associated with significant urinary counts (greater than or equal to 10(5) organisms/ml). Although 10% of the patients showed positive blood cultures indicating bacteremia none of them had shaking chills, fever or any other clinical signs of septicaemia. None of the patients from whom blood samples were taken had yet received antimicrobial therapy. Bacterial isolates from urine and blood were identical. Microorganisms most frequently isolated were found in the following order: E. coli, Proteus species, K. pneumoniae and coagulase negative staphylococci. Neoplasms, obstruction of the urinary tract and age were found to be high risk factors.
