[NMR tomographic imaging of bone marrow infiltrates in malignant lymphoma].

10 normals and 37 patients with Hodgkins' and non-Hodgkin's lymphoma were examined by MRT as part of a prospective study (lumbar spine, pelvis, proximal femora). In about half the patients the MR images showed diffuse or focal areas of reduced signal intensity in the bone marrow. The observation of reduced signal areas corresponded with the pathological findings from iliac crest biopsies. To improve diagnostic accuracy, the signal intensity from a chosen region of interest was related quantitatively to a standard level.

Magnetic resonance imaging of bone marrow in lymphoproliferative disorders: correlation with bone marrow biopsy.

In a prospective clinical study 10 normal volunteers and 30 patients with lymphoma--11 patients with Hodgkin's lymphoma and 19 patients with non-Hodgkin's lymphoma (11 low grade, 8 high grade)--were examined. Proximal femora, pelvis and lumbar spine were imaged with a 1.5 tesla superconducting MR imaging system (Siemens Magnetom). Areas of malignant infiltration in the bone marrow were clearly detected by visual and/or quantitative assessment. In most cases bone marrow involvement was demonstrated by both magnetic resonance imaging and bone marrow biopsies. However, in three of 30 patients magnetic resonance imaging showed evidence of lymphomatous involvement despite normal bone marrow histology. In one patient bone marrow cytology revealed malignant infiltration in the absence of MRI abnormalities. Thus, MRI is a sensitive method for detecting bone marrow infiltration by lymphoma.

[MR tomography detection of bone marrow infiltration by malignant lymphomas].

In a prospective MRT study, both healthy volunteers (n = 10) and patients with Hodgkin and non Hodgkin lymphomas (n = 32) were examined (lumbar spine, pelvis and proximal femura). A biopsy from the posterior iliac crest was taken in all patients. About one half of the patients showed in MR-images diffuse or patchy areas of decreased signal intensity in the regions of the bone marrow, while the bone marrow of the volunteers showed an almost homogeneous pattern with high signal intensity. Our findings observed by MRT in bone marrow of these patients corresponds with pathological results obtained from crest biopsies. In order to improve the diagnostic accuracy, a quantitative evaluation of the signal intensities in selected ROIs, referring to a standard, was used. This method allows to correct variations of the signal intensity due to technical reasons. As our results show, this leads to a better differentiation of border-line cases. MRT has the advantage to detect localised lesions outside of the iliac crest. In cases of negative crest biopsies, but suspicious areas in MR-images, biopsies could be directed to these lesions.

[Recurrent cerebral emboli in nonbacterial thrombotic endocarditis].

A 28-year-old woman developed thrombophlebitis migrans and right hemiparesis with motor aphasia. Chest X-ray revealed a plum-sized central infiltrate in the right lower lobe of the lung. Transbronchial lung biopsy showed bronchoalveolar carcinoma. Subsequently recurrent cerebral infarcts developed from which the patient died. Autopsy revealed nonbacterial thrombotic endocarditis as the cause of the cerebral infarcts. This form of endocarditis is characterized by the parallel occurrence of arterial emboli and thromboses of the superficial and deep veins. It develops in association with various diseases, especially malignant tumors, and is an expression of a generalized thrombosis disposition.

Evidence for developmental changes of type IV collagen in glomerular basement membrane.

Digestion of adult glomerular basement membrane (GBM) with collagenase releases a number of peptides of which the noncollagenous region of type IV collagen is detected by antibodies from patients with anti-GBM nephritis. In our study, 17 of 19 sera reacting with GBM, in indirect immunofluorescence, with cryostat sections of adult kidneys were negative with cryostat sections of fetal kidneys. However, after digestion with collagenase, a similar pattern of peptides was released from both fetal and adult GBM. SDS-PAGE immunoblotting revealed comparable antibody binding of all 19 sera with the peptides from either fetal or adult GBM. These findings suggest conformational differences between type IV collagen in fetal and adult GBM or masking of the antigen by other GBM constituents which may shield the antigen.

[Self-help groups (author's transl)].

This report includes the preliminary results of a research project in which the origin, method of working and result of self-help health groups are investigated. An assessment of self-help groups in Hamburg revealed 210 disease-related, rather rehabilitatively oriented groups and 177 with life-problem-related, rather preventively effective conclusions. In an inquiry of contact persons from 47 groups it was shown that barely two thirds see their expectations fulfilled in the group. Of those questioned, 39 considered their group standing beside the care system, 3 within the system, 5 as counterinstitutions. An exploratory study of the attitude of professional helpers showed that relief of their own work by self-help groups was certainly desirable, but also mixed with the fear of becoming superfluous.

Molecular cytogenetic analysis of B-cell chronic lymphocytic leukemia.

The genetic alterations underlying the pathogenesis of B-cell chronic lymphocytic leukemia (B-CLL) are difficult to assess. Cytogenetic studies are hindered by the low in vitro mitotic activity of the tumor cells and the limited resolution of chromosome banding. Molecular genetic analyses are hampered by nonclonal cells contained in the specimens and by the limited knowledge of candidate genes involved. As a complement to cytogenetic and molecular genetic techniques, fluorescence in situ hybridization (FISH) has proven powerful in the molecular cytogenetic analysis of B-CLL. FISH allows the detection of aberrations such as trisomies, deletions, and translocation breakpoints on the single cell level in dividing as well as non-dividing cells without the prerequisite of detailed physical maps or knowledge of involved genes. As detected by the interphase cytogenetic FISH approach, the most common chromosome abnormalities of B-CLL are deletions in band 13q14, followed by deletions in 11q22-q23, trisomy 12, deletions in 17p13, and deletions in 6q21. Abnormalities in 17p13 seem to involve the TP53 tumor suppressor gene, but as yet no candidate genes have been identified for the other frequent aberrations. Toward the identification of such genes by positional cloning, FISH can be applied for detailed aberration mapping at the molecular level. Furthermore, the accurate detection of chromosome aberrations in B-CLL by FISH provides a valid basis for the evaluation of their prognostic significance. Inactivation of TP53 in 17p13 and deletions in 11q22-q23 have already been shown to be among the most important independent prognostic factors. Genetic abnormalities may eventually provide biological parameters, allowing a risk assessment for individual patients at the time of diagnosis of this clinically heterogeneous disease.

Comparison of comparative genomic hybridization, fluorescence in situ hybridization and flow cytometry in urinary bladder cancer.

Comparative genomic hybridization (CGH) was applied to screen the genetic events in six invasive urinary bladder cancers. These cases were also studied by flow cytometry (FCM) and fluorescence in situ hybridization (FISH). Four samples showed partial gain on chromosome 8, with the common region involved was on 8q23-qter. Full or partial deletion on chromosome 2 and 17p in addition to gain on 20q was found in two cases. Interestingly one diploid tumor with low mitotic index, stage and grade showed more genetic aberrations (8 gains and 7 losses) by CGH than other aneuploid tumors with high mitotic index, stage and grade. The numerical chromosomal aberration detected by FISH for chromosomes 7, 8, 9, 10, 11 and 17 were 50% in T1 cases and 100% in T2-T4 cases. FISH was performed on chromosome 8q and 17p to compare and validate the sensitivity of CGH. The agreement was 100% for 8q24 locus and 50% for p53 locus. This indicates that different molecular genetic techniques showed relatively different aspect of genomic aberrations.
