[Treatment of metastatic kidney cancer with recombinant alpha-2 or gamma interferon. Results of 2 clinical phase II and III studies].

In a phase-II and a phase-III study patients with histopathologically documented metastatic renal cell carcinoma were treated either with gamma-interferon in two different doses (100 micrograms/m2 3x/week for 4 h i.v. every other week or 500 micrograms/m2 5x/week for 24 h i.v. every other week) or with alpha-2-interferon alone (18 x 10(6) U 3x/week weekly i.m.) or in combination with vinblastine (0.1 mg/kg every third week i.v.). The purpose of these studies was to evaluate the response rate, the duration of response, the survival, the efficacy and the toxicity of the different forms of treatment. The overall response rate to gamma-interferon was 30% in both regimens. The response rate of treatment with alpha-2-interferon was found to be 31%. The duration of response ranged between 2 and 34+ months in patients treated with gamma-interferon and between 2 and 24+ months in those receiving alpha-2-interferon. Patients with objective tumor response showed a significantly longer survival than those not responding (p = 0.0056). Low-dose-gamma-interferon and alpha-2-interferon treatment could be easily done on an outpatient basis. In conclusion, interferon treatment seems to be of value in the therapy of patients with well documented progressive disease in metastatic renal cell cancer.

[Computed tomography of soft tissue tumors of the extremities].

The results of 80 CT examinations in 57 patients with malignant (32) and benign (25) soft tissue tumours of the extremities are reported. Thirty-five patients underwent surgery and the CT features (size, structure, density, necrosis, calcification, bone involvement) were compared with the pathological and histological findings. Three quarters of the tumours which were non-homogeneous on CT were malignant, one quarter benign. Only half the cases of malignant tumours showed a definite margin, but 70% of benign tumours did so. Indefinite margins were found in 50% malignant and 30% benign tumours. Soft tissue tumours of the hands and feet were difficult to differentiate. Difficulties were also found during follow-up (37 patients) because of anatomical changes following surgery. Although some histological types tend to be associated with particular CT findings, a definite histological diagnosis can only be made in the case of lipomas.

[Computed tomographic follow-up controls after the radiotherapy of brain metastases].

The results of CT control (n = 154) of 69 patients with cerebral metastases (n = 106) are presented. 89 metastases showed a reduction of volume after radiotherapy. 19 metastases increased in volume, and five patients developed new metastases. In the contrast representation 49 out of 66 patients showed a reduced enhancement after radiotherapy, an increase was observed in twelve patients. In 15 out of 22 cases, the irradiation was followed by a decrease of ventricle compression. In case of ventricular dilatation (n = 5), three decreases and two increases were observed. In case of displacement of the mid line (n = 15), eight decreases, five increases, and two identical results were found. Twelve patients presented radiogenic side effects of the central nervous system or the cerebral metastases.

[Intraoperative sonography to localize a kidney tumor].

The authors present the case of a 61-year old man with microhematuria, in whom the preoperative diagnostic work-up (urography, percutaneous sonography, CT, retrograde pyelogram) had led to the diagnosis of a right sided renal tumour. Intraoperatively a kidney tumour was neither seen nor palpated. However, the mass was clearly outlined by intraoperative sonography. The following nephrectomy revealed an intrarenal adenocarcinoma of the kidney of 2.5 cm diameter.

Noninvasive ultrasound in detecting and staging bladder carcinoma.

Patients with suspected bladder abnormalities were examined by noninvasive suprapubic sonography to define the accuracy of ultrasound for detecting and staging bladder carcinomas. In 103 patients, 65 tumors were found by cystoscopy, of which sonography detected 61 (94%). Four lesions less than 2-3 mm were missed at the bladder dome, the ventral wall, and side wall. The sonographic staging was correct in 83% of all tumor stages with the lowest value of 69% for T2/T3a tumors; excluding recurrent tumors, the overall accuracy increased. From these results, suprapubic sonography is considered to be a reliable noninvasive technique for detecting bladder tumors and for preoperative local staging. The staging results are comparable with reports in the literature on the accuracy of intravesical sonography.

[Computed tomographic control of the course of primary brain tumors following radiotherapy].

Two hundred and seventy-eight CT scans are presented which were carried out on 105 patients following radiotherapy for primary cerebral tumours. Of these, 90 had surgery and 15 did not. Amongst 63 patients who showed abnormal enhancement before irradiation, enhancement was reduced in 32, increased in ten and remained the same in 17 following treatment. Of the 42 patients who did not show enhancement before irradiation, this was observed in 24 patients after treatment, but not in the remaining 18. Changes in the pattern of contrast enhancement was very variable in both groups. Other signs of tumour (compression of ventricles, mid-line displacement, ventricular dilatation) showed no differences in the groups. Changes in the central nervous system due to irradiation were observed in 25% of cases.

Computertomographic diagnosis and follow-up studies of bone and soft tissue tumours of the thoracic wall.

The results of computertomographic investigations (primary diagnosis n = 103 and follow-ups n = 176) of inflammatory (n = 7), benign (n = 4) and malignant bone (n = 65) and soft tissue tumours (n = 27) of the thoracic wall were analysed. The precise differential diagnosis of inflammatory destructive bone lesions from malignant bone tumours was not possible. Two thirds of the malignant bone tumours had an infiltrative growth into the adjacent tissues. Only computed tomography (CT) revealed their full extent. In addition, CT was very helpful for planning surgical treatment and radiotherapy. Comparable indications for CT were also obtained for soft tissue tumours.

[Sonography of the efferent urinary tract. Suprapubic and intravesical methods].

The ability of sonographic methods to demonstrate the urinary passages and their abnormalities were studied systematically. The ureters can only be demonstrated if they are dilated. Sonography of the bladder can be used as a screening method for tumours, diverticula and stones. Intravesical sonography is useful for staging of bladder tumours. The value of these methods is discussed.

Comparative study of suprapubic sonography and computed tomography for staging of prostatic carcinoma.

In addition to the digital rectal examination, suprapubic transvesical ultrasonography and computed tomography were used to stage local tumor extension in 41 patients with histologically proven prostatic carcinoma. Although 22% of the cases revealed stage A/B disease on rectal examination, these numbers were 7% for ultrasonography and 37% for CT. For stage C/D disease the percentages were 73% for digital rectal staging, 81% for sonography, and 30% for CT. Compared with the digital examination, 22% versus 10% of the cases had to be upstaged by the results of ultrasound versus CT. A downstaging became necessary by sonography in 7% and by CT in 44%. The results are compared with the findings after prostatectomy, autopsy, and cystoscopy as well as with pathohistological data from the literature. From these results, suprapubic transvesical sonography is considered to be more reliable than CT for local tumor staging.

Quantification of tracer kinetics in intra- and extrahepatic bile ducts and of gallbladder filling rates in hepatobiliary dynamic scintigraphy using 99mTc-IDA derivates.

A method is presented of calculating the flow of 99mTc-labelled IDA derivates in intra- and extrahepatic bile ducts. By computer means the superimposed liver parenchymal activity is subtracted from the original sequence data in each pixel and at each time. In addition to known evaluation methods, the described procedure seems to be helpful for clinical application in order to better display anatomical details of intrahepatic structures like bile ducts and to calculate quantitative parameters like appearance times in intra- and extrahepatic regions. Examples are given and the possibility of calculating a gallbladder filling rate for separating normal gallbladders from stone-filled ones is statistically proven.

[Suprapubic prostatic sonography for staging prostatic carcinoma].

Sixty-one patients with histologically confirmed carcinomas of the prostate were examined by suprapubic, transvesical prostatic sonography in order to stage local tumor spread. The results were compared with rectal palpation. Compared with rectal palpation, sonography led to tumor up-staging in 16% and down-staging in 7%. The results showed good agreement with findings at prostatectomy, autopsy and cystoscopy. Additionally, the sonographic up-staging/down-staging figures were compared with "tumor staging after prostatectomy, as compared with clinical staging" quoted in the literature. It is concluded that suprapubic prostatic sonography is reliable supplementary method of examination for local tumor staging and that it improves the classification of local tumor extent.

[Reaction of hemodynamic parameters to coronary angiography with two different age groups (author's transl)].

Two groups of patients (A = 19 less than 50 a; B = 21 greater than 50 a) were supervised during coronary injection as to cardiac and pulmonary pressure. In both groups there is an equivalent increase of the cardiac index by 20%, the initial value being lower in groups B. The increase lasts for more than 2 Min. During 5 Min the pulmonary pressure is increased by 25%. The reaction is based on an intracellular calcium flow induced by contrast medium.

[Suprapubic prostatic sonography in the diagnosis of carcinomas and adenomas of the prostate (author's transl)].

Prostatic adenomas and carcinomas were examined by suprapubic transvesical prostatic sonography. The value of the method is comparable with transrectal prostatic sonography. Adenomas and carcinomas can be differentiated on the basis of their echo patterns and marginal contours. In the presence of carcinomas, sonography is a valuable addition to clinical staging and staging by CT. Compared with CT, the ability to perform longitudinal sections permits good evaluation of the bladder base. Sonography is also valuable for serial observations after treatment and for diagnosing recurrences.

[Variant of the subclavian steal syndrome associated with an anomalous origin of the left vertebral artery].

A case with a variant of the subclavian steal syndrome associated with an anomalous origin of the left vertebral artery from the aortic arch is presented. Prior to the examination a carotid-subclavian bypass had been performed. The main angiographic finding is the blood supply of the distal subclavian artery through an ipsilateral cervical collateral system. Angiographic procedure and therapeutic consequences are discussed.

[Influence and importance of dacarbazine and BCG in sonography of the liver (author's transl)].

Basing on results obtained in a group of 30 patients with malignant melanoma, the authors investigated the influence of therapy with cytostatics and of immunotherapy with dacarbazine or BCG, respectively, on the sonographic findings in examination of the liver. In 6 patients, we recorded a larger echo reflex pattern subsequent to therapy, similar to the observations made in diffuse changes in the liver and in the formation of multiple metastases in the liver. This complicated the differential sonographic assessment in respect of excluding multiple liver metastases. Diffuse growth occurred in three patients; we did not observe any extended branches of the protal vein, nor did we see any changes in the biliary tract pattern.
