Brain abscess caused by Salmonella enteritidis in an immunocompetent adult patient: successful treatment with cefotaxime and ciprofloxacin.

A previously healthy 43-y-old man, who had spent 2 weeks in northern India, was admitted to hospital after a 2-day history of pyrexia, confusion and frontal headache. Cranial computerized tomography (CT) showed an abscess in the right parietal lobe. Spinal fluid and blood cultures gave growth of Salmonella enteritidis within 24 h. Treatment with cefotaxime was initiated, but ceased after 3 weeks due to drug fever, and ciprofloxacin was then given orally for 4 months. After 6 months, the patient was considered cured. Cases of salmonella brain abscesses are reviewed.

[Hysterosalpingography using iotrolan--a new contrast medium].

Iotrolan was investigated as a new contrast medium for hysterosalpingography. Fertility rates and side-effects were registered in a series of 108 consecutive patients. The results indicate a higher fertility rate after hysterosalpingography with iotrolan than after the use of other water-soluble contrast media but a lower fertility rate when compared to lipiodol. As for side-effects, the study indicates that the frequencies of low abdominal pain and vaginal bleeding after hysterosalpingography are equal for iotrolan and lipiodol but higher for other, water-soluble contrast media. A prospective, randomized study is recommended for the clarification of these aspects.

[Periventricular cerebral hemorrhage in children with very low birth weights in the county of North Jutland. Retrospective study].

The object of this investigation was to determine the frequency of intracerebral periventricular haemorrhage in neonates with very low birthweights in an unselected material from the County of North Jutland. During the years 1988-1989 a total of 86 infants with low birthweights were born in the county. Intracerebral periventricular haemorrhage was diagnosed by means of ultrasonic scanning of the cerebrum and/or postmortem examination. Seven infants had to be excluded as neither of these examinations had been performed. The material thus consisted of 79 infants with a median weight of 1,196 gram (range 565-1,500 g) and a median gestational age of 30 weeks (range 25-37 weeks). Periventricular haemorrhage was found in a total of 13 infants, corresponding to 16% and the haemorrhages were found mainly in infants with gestational ages less than or equal to 29 weeks (32%). Haemorrhages with involvement of the parenchyma and/or dilation of the ventricular system comprised more than half of the cases of haemorrhage (8 out 13). The frequency of periventricular haemorrhage fell with gestational ages less than or equal to 30 weeks and the haemorrhages were less extensive. The respiratory distress syndrome and asphyxia were found to important risk factors for the development of periventricular haemorrhage.

Reinforced anatomical reconstruction of the anterior talofibular ligament in chronic anterolateral instability using a periosteal flap.

Twenty-eight consecutive ankles with chronic severe lateral instability were treated by reinforced anatomic reconstruction using a periosteal flap from the lateral aspect of the fibula. At the followup after a median of 24 months, 86% had excellent or good functional results. This result compares favorably with other techniques. As the results after tenodesis appear to deteriorate with time, anatomic reconstruction is recommended. No complications were encountered. No statistical correlation was found between clinical and radiological versus functional instability at followup.

[Ultrasonically guided intraperitoneal catheterization for ascites drainage or intraperitoneal chemotherapy].

A new method for drainage of ascites is presented: ultrasound guided intraperitoneal catheterization. Forty-six drainages were performed in 17 patients. The duration of drainage was from one to 12 days. Two patients had slight complications during drainage. Puncture was easily done even in cases of difficult or impossible conventional puncture. The method provided very effective drainage. Further, we present ultrasound guided intraperitoneal catheterization for chemotherapy in ovarian cancer. In eight patients 51 catheterizations were performed. Three punctures were primarily unsuccessful, but successful on the second attempt. There were no complications in these or in other cases.

Lauge-Hansen classification of malleolar fractures. An assessment of the reproducibility in 118 cases.

The radiographs of malleolar fractures in 118 consecutive patients were assessed by 4 observers using the Lauge-Hansen classification. The interobserver variation was large; only 51 of the 118 radiographs were classified identically by all 4 observers. With the purpose of assessing intraobserver variation, the same observers classified the same radiographs 6 weeks later. The first and the second classification were identical in 75-97 of 118 radiographs (64-82 percent). These results show that the Lauge-Hansen system is difficult to apply in a reproducible way.
