[Disability and damage of integrity].

A Swiss ophthalmologist should at minimum be aware, that, according to insurance law, these two technical terms are not identical. Some details of legal principles have to be known if being an expert for insurance or court of justice. Damage of integrity is related to the immaterial, permanent and significant reduction in physical or mental integrity, whereas invalidity relates exclusively to economic damage as a consequence of a permanent or temporary reduction of income due to accident or occupational disease; as a consequence the level of any invalidity pension is not directly related to the medically defined damage of integrity.

Disability Evaluation↗

[Assessing accidental damage to the integrity of the eye with preexisting damage].

According to the insurance law, compensation for damage of integrity has to be realized only in cases of accident caused damage of integrity. In cases of preexisting damage on the affected eye the evaluation is simple: Total damage minus non accidental cause integrity damage is equal to accident caused damage of integrity. More difficult is the evaluation in cases of preexisting damage on the partner eye. According to the visual function of the non affected eye (first eye) the basic value of the accident eye (second eye) amounts in case of total loss between 30 and 70%. Values in between in cases of partial damages are explained by means of a three-dimensional graphic.

Diagnosis, Differential↗

[What should the Swiss ophthalmologist know about integrity damage?].

The purpose of this new instrument in Swiss social insurance (applicable since January 1, 1984) is to compensate for an immaterial, permanent, and significant reduction in physical or mental integrity due to an accident or any legally defined "occupational disease". The indemnity is paid once, as a capital sum, on an abstract and egalitarian basis, irrespective of the patient's age, sex, occupation, or income. In contrast, the invalidity pension provided under the social insurance scheme is paid on a monthly basis and compensates exclusively for economic damage as a consequence of a permanent or temporary reduction in income in cases of accident or disease.

[The terminology of blindness: remarks on the behavior of the ophthalmologist in cases of tapetoretinal degeneration (author's transl)].

The word "blind" should never be applied to nonamaurotic patients. The additional linguistic effort involved in accurately defining the various forms of visual impairment is a psychological service to which patients are entitled. The author outlines the immense task of coping with hereditary tapetoretinal degenerations which it was hitherto impossible to treat. Even today we are all still clearly - and unformtunately - far from being able to raise our patients' hope with the prospect of cure or definitive stabilization of their conditions.

Blindness↗

[Intraocular lenses - aphorisms of an operating practitioner (author's transl)].

The implantation of an intraocular lens is an almost ideal solution to manage monocular cataracts. We recommend this operation if medically indicated; it is technically more difficult, needs microsurgical skill and general anesthesia and therefore should be limited to a reduced number of highly qualified operation-teams. Uncontrolled enlargement of indications will result in higher complication-rates. As we do not yet know on complications due to alteration of foreign intraocular material after many years, we rather prefer to operate double-sided cataracts in the conventional way, specially in younger patients. Regarding to the global problems of curable (operable) blindness, we think that young eye-surgeons should be trained for conventional cataract operations without using microscope.
