Bladder carcinoma treated by partial cystectomy: a review of 54 cases.

A retrospective study of 54 partial cystectomies done between 1958 and 1973 was undertaken to evaluate the proper role of the procedure in the treatment of bladder carcinoma. The operative mortality rate was 1.8% and the postoperative morbidity was low in the series. The 5-year survival rate for stages B1 (21 cases) and B2 (16 cases) lesions was 57%, with an over-all 5-year survival rate for all stages of 48%. Indications for the operation are presented and factors influencing survival are discussed.

Urine cytology findings in analgesic nephropathy.

Urine cytology screening for neoplasm led to the detection of 3 urothelial carcinomas and 1 severe urothelial dysplasia in 98 patients with analgesic-induced papillary necrosis. A further 18 patients had changes suggesting that they were at risk of incurring malignancy in the near future. Routine urine cytology is recommended in all cases of analgesic nephropathy.

Acute myocardial infarction following gold sodium thiomalate induced vasomotor (nitritoid) reaction.

A vasomotor (nitritoid) reaction occurred following an initial injection of gold sodium thiomalate (GST; Myochrysine) in a 69-year-old man with rheumatoid arthritis (RA). An acute anterior wall myocardial infarction, documented by serial electrocardiographic and serum enzyme changes, developed immediately thereafter. A second patient, a 49-year-old man with RA and a history of GST-associated vasomotor reactions, was monitored clinically and electrocardiographically after GST administration. Sinus tachycardia developed and peripheral blood pressure fell within 2 minutes of injection, simultaneous with the onset of vasomotor symptoms. Vasomotor reactions from GST may compromise myocardial perfusion by their action on arteriolar smooth muscle, and thus result in peripheral vasodilatation, or they may act by adrenergic discharge initiated by such a reaction, and thus increase myocardial work and oxygen demand. Aurothioglucose (Solganal), rarely produces vasomotor reactions, and may be preferred to GST in elderly RA patients with concomitant cardiovascular disease or atherosclerosis.

ABO blood group in relation to hypernephroma.

The correlation of blood groups in adenocarcinoma of the kidney is attempted in 125 cases. It appears that there is an inherited element in the susceptibility or protection against the hypernephroma according to blood groups.
