Type I allergy induced by Limulus Amoebocyte Lysate (LAL).

During the last few years Limulus Amoebocyte Lysate (LAL) has been extensively used to detect minimal amounts of endotoxins of Gram-negative bacteria in products of the pharmaceutical industry, in food stuff, body fluids, house dust and room air. LAL is produced from cells of the haemolymph (amoebocytes) of the horseshoe crab (Limulus polyphemus), which respond with an extremely sensitive clotting system upon contact with endotoxins. In this study we demonstrate by typical case history, positive skin test and ELISA the occurrence of Type I allergy to LAL in a patient suffering from conjunctivitis and rhinitis at work.

[Quality control in the allergy laboratory--4 years "Allergy Ring Trial" in Austria].

In 1988 the Austrian Society for Allergology and Immunology initiated an external quality control program for the in vitro allergy diagnosis. In 12 mailings, 62 sera from allergic patients were sent to selected laboratories in order to determine total and antigen specific IgE according to the laboratory-specific methods. The values for total IgE varied considerably (73.9% were within +/- 1 SD, 94.3% within +/- 2 SD and 99.1% within +/- 2 SD, but only 0.9% beyond that). Sources of error were mainly attributable to inappropriate equipment and low quality reagents, but also bad test performance, with respect to personnel and the routines. In contrast, antigen specific IgE against pollen, mites, moulds, insect venoms, animal danders, drugs, parasites, environmental and food antigens revealed quite homogeneous results. Out of 1492 data, only 46 (3.1%) had to be declared as "wrong", and the variability of the RAST-classes was low. Whereas the quality of the reagents from all the different suppliers was not absolutely reliable at the beginning of the study, it improved considerably with time, as consequence of our complaints. The comparability of the methods for detecting total-IgE were non-satisfactory, whereas those for antigen specific IgE were generally good. The variety of methods employed (radio- and enzyme immunologic, fluoro- and nephelometric methods, etc.) should entail appropriate consequences, especially critical comparisons within one and the same laboratory; in addition, international standardization of the "normal" values should replace company-standards, and quality control programs for each test system before it is marketed should be mandatory. [Results of systemic treatment of lichen planus with a new aromatic retinoid (RO 10-9359) (author's transl)].

24 patients suffering from generalized lichen planus were treated systemically with a aromatic retinoid (RO 10-9359). Initially a dose of 100 mg retinoid per day was administered for 2 to 3 weeks, then the daily intake was reduced slowly. The average duration of treatment was 72 days. This therapy produced a significant improvement in all patients. Lesions recurred during reduction of administered retinoid in 3 patients and after stopping treatment in 5 cases. Initially side effects consisted of cheilitis and exfoliation of the skin over the palmar and plantar regions; later 50% of the patients developed an effluvium. The results of the present study indicate that the aromatic retinoid can be considered as an alternative drug to systemic steroids in the treatment of lichen planus.

[Vitiligo from p-tert. butylphenol; a contribution to the problem of the internal manifestations of this occupational disease].

10 worker suffering from occupational vitiligo due to p-tert. butyl phenol have been observed in an Austrian resin factory. The dust concentration and its content of p-tert. butyl phenol in the working area were measured. On the basis of these results preventive measures were taken to reduce the exposure of the workers to p-test butyl phenol. In the presented cases an involvement of internal organs--as described by Rodermund et al. (1975)--could not be observed.

[The implication of trace metals in household products in the causation of contact dermatitis (author's transl)].

The 6 most common fluid and powder detergent brands in use in Austria were analyzed as to their nickel and chromium content. The analysis was carried out by means of atomic absorption spectroscopy. 1. The above-mentioned metals were found to be present in each of the products analysed. On average, the fluid detergents contained ten times less nickel and chromium than the powders. 2. Repeated investigations revealed a considerable variation in metal ion content in different batches of the same detergent. Thus, presumably the raw products already contain nickel and chromium and it is unlikely that these ions get into the products during the manufacturing process. 3. In the appropriately diluted detergent powder solutions, as recommended by the producer, the content of nickel was found to lie between 0.4 ppm and 0.717 ppm and the chromium content ranged from 0.733 to 0.917 ppm. Since powder detergents also contain complex-producing agents, it can be assumed that these concentrations of metal ions do not suffice for sensitization. In the fluid detergents the nickel and chromium contents were found to be below the safety limit for the elicitation of an excematous reaction.

[The neural periphery in lichen planus lesions (author's transl)].

In 12 patients suffering from generalized Lichen planus (L.p.) the neural periphery of lesions was studied by modern histochemical and electron microscopical methods. In contrast to the investigations published until now on this subject no signs of neural hyperplasia in L.p. could be found. Only some nerves localized in the Lichen planus infiltrate exhibited morphological changes indicating increased neural de- and regeneration in this region. According to the results obtained the nerve changes in L.p. are caused by the typical inflammatory reaction found in this dermatosis. Therefore the neural theory of L.p. cannot be sustained on a histological basis.

[Leg ulcer and allergic eczematous contact dermatitis incidence of contact allergies induced by topical therapy (author's transl)].

In the IInd Department of Dermatology, Vienna University, 200 patients (143 female, 57 male) suffering from leg ulcers due to chronic venous insufficiency underwent patch testing with routine allergens, vehicles and their ingredients, as well as topical antibiotics. 114 patients (57%) had contact allergy. Balsam of Peru heads the list of allergens with postive reactions in 22%, closely followed by oil/water emulsions such as Drointin (21%), Ultrasicc (20%) and Ung. emulsificans aqu. (15,5%). The causative agents in these preparations were emulsifiers on the one hand (Lanette O, E, N) and preservatives (parabens) on the other hand. In comparison, allergic reactions to water/oil emulsions were less common. The low index of sensitivity to local antibiotics was also surprising. The results of the present study, in accordance with other reports in the literature, underline the need (1) for full label diclosure of cosmetics and topically-applied drugs and (2) for patch-testing every patient suffering from leg ulcers of long duration.

Specific and total serum IgE measurements in the diagnosis of penicillin allergy. A long term follow-up study.

Sera from 204 patients with acute or former reactions following penicillin treatment were investigated by RAST with specificity for benzylpenicilloyl and phenoxymethyl-penicilloyl. The IgE antibody levels were followed up for of more than 2 years. Positive results could be observed at the time of the acute reactions and the weeks following; later months or years in many cases. Occasionally negative RAST results occurred within 30 days; on the other hand high levels of specific IgE persisted for years in some patients. Total serum IgE measured by RIST and the RAST results showed similar patterns, with mean levels of 568 u/ml between 0 and 30 days after the penicillin allergic reaction, 286 u/ml between 30 days and 1 year, and 195 u/ml in cases after 1 year corresponded well with the observed decline in specific IgE. Skin tests performed with penicilloyl-polylysine in 118 patients showed an overall correlation of 91-5% with the RAST results. Differences between the two test systems were observed in patients with a long interval between the test and the last allergic attack. None of the patients with proved penicillin allergy had atopic diseases or a history of atopy. On the basis of these findings it is suggested that the determination of both circulating specific IgE and total IgE should be of great value for diagnostic purposes during acute drug reactions and in the months immediately afterwards.

Cell proliferation in the Lichen planus infiltrate. Investigations by light- and electron-microscopic autoradiography.

In seven patients suffering from lichen planus (LP) light- and electron-microscopic 3H-thymidine (3HT) labeling studies were performed and the following results were obtained. 1. Using the light-microscope, a large number of 3HT labeled cells can be observed in the dermal infiltrate of LP. Most of the labeled nuclei belong to mononuclear, 'round cells' while isolated endothelial cells, fibroblasts, histiocytes and mast cells could be found labeled. 2. The mean labeling index of 'round cells' in our patients was 4.27 +/- 1.57%. 3. The evaluation of the electron microscopic autoradiographs showed that, as far as 'round' cells were concerned, exclusively nuclei of cells showing the morphological criteria of stimulated lymphocytes were labeled. These findings support the view that in LP, as in allergic contact dermatitis, a high proliferative activity of lymphocytes takes place.

The epidermal cell proliferation in lichen planus.

The proliferative activity of epidermal structures was determined in 6 patients suffering from lichen planus (i.p.). By using the in vitro 3H-thymidine labelling technique, semithin Epon sections of lesional and perilesional uninvolved skin were compared autoradiographically with normal controls. The following results were obtained. The labelling index of epidermal cells was significantly increased within the i.p. papules. The mean values ranged from 41% in acanthotic i.p. to 23% in atrophic i.p. whereas in adjacent unaffected skin (15% and 11% respectively) as well as in normal controls (6%) a lower 3H-index was observed. As regards damage to basal cells within i.p. lesions, a factor--the relation between the number of basal cells in involved and uninvolved skin in reference to the epidermal length--was calculated in order to correct the autoradiographic results in involved skin. The differences between i.p. papules, adjacent skin and controls remained statistically significant even after correction. The intradermal eccrine ducts revealed a significantly increased labelling index in i.p. lesions as compared with perilesional and normal skin. In this respect no difference could be observed between the acanthotic and the atrophic variant of i.p. These findings indicate that replacement of damaged basal cells in i.p. is achieved by an increase in actively dividing keratinocytes in both epidermis and skin appendages within the lesion.

[Septic complications of gonorrhea].

Septic gonococcal complications consist in intermittent fever, arthralgia and skin lesions. In recent years predominantly females suffering from this disease were observed. This diagnosis is made by the demonstration of gonococcal infection combined with the above mentioned clinical symptoms. A further confirmation is possible by blood culture and the demonstration of gonococci in skin lesions or joint fluid.

[Immunological investigations in lichen planus patients (author's transl)].

30 patients suffering from generalized lichen planus (= L.p.) have been examined by the following methods in order to more precise information regarding their immune-status: 1. Quantitative determination of immuno-globulins including complement fraction (beta 1C/A) 2. Intracutaneous tests with recall-antigens 3. Investigation of cutaneous sensitivity to DNCB 4. Lymphocytemigrationinhibition- and Lymphocytetransformationstests. The results obtained were not statistically significant when comparing the results of this investigation in patients suffering from L.p. with the control group. Therefore, the theory that immunological processes are the basis of these occurences is still under discussion. Further studies applying more specific methods should be conducted in order to clarify this problem.

[Cytoid bodies in basal cell epithelioma].

100 basal cell epitheliomas were surveyed for the presence of cytoid bodies by histological and histochemical techniques. By this means a differentiation of the following structures was possible: 1. Elastic globes were present in 32 specimens. Their localisation corresponded in part to the typical subepidermal situation, but invasive tumor growth resulted in their translocation into the deeper corium. 2. Civatte bodies were found in the dermoepidermal junction area in 16 cases. Similar structures were present within the tumor cell islands of 37 patients. 3. Russel bodies could be observed within the inflammatory peritumoral infiltrate containing numerous plasma cells in 30 specimens. 4. Clumpy or ovoid deposits of amyloid were present within the stroma of 41 epitheliomas, whereas no amyloid was found in the tumor cell islands.
