Evaluation of Helicobacter pylori in gastric MALT-type lymphoma: differences between histologic and serologic diagnosis.

Helicobacter pylori provides the pathogenic background for the development of gastric MALT-type lymphoma. The assessment of H. pylori is limited by the accuracy of the detection method used. Especially in patients with low-grade MALT-type lymphoma, the H. pylori status is the crucial question for therapeutic management. In this study, 60 patients with gastric MALT-type lymphoma. (lowgrade, 22; high-grade, 38) were investigated for the presence of H. pylori by histologic and serologic means. In 98% of the patients with MALT-type lymphoma, H. pylori-specific IgG serum antibodies were detected. In contrast, on histologic examination, H. pylori was found only in 78% of the patients (low-grade, 77%; high-grade, 79%). In this study, a discrepancy between serologic and histologic evaluation of the H. pylori status in gastric MALT-type lymphoma was found. Therefore, a H. pylori eradication therapy in low-grade MALT-type lymphoma, which often leads to a complete tumor regression, should not be excluded as a first line therapy because of a negative H. pylori status on histologic examination.

Antimicrobial peptides in the stomach of Xenopus laevis.

Antimicrobial peptides are widely distributed in nature and appear to play a role in the host defense of plants and animals. In this study we report the existence of antimicrobial peptides in the stomach of the vertebrate Xenopus laevis, an animal previously shown to store high concentrations of antimicrobial peptides in its skin. Antimicrobial activity was detected in extracts of X. laevis stomach tissue and nine antimicrobial peptides were then purified. A novel 24-amino acid peptide, designated PGQ, was isolated from these extracts, and has the following amino acid sequence: GVLSNVIGYLKKLGTGALNAVLKQ. PGQ is relatively basic and has the potential to form an amphipathic alpha-helix. The other peptides isolated are members of the magainin family of antimicrobial peptides, and include magainins I and II, PGLa, xenopsin precursor fragment, and four caerulein precursor fragments. None of these peptides had been previously identified in tissues other than the skin. The purification of the peptides from stomach extracts and subsequent protein sequence analysis reveals that the peptides have undergone the same processing as their dermal counterparts, and that they are stored in their processed forms. Northern blot analysis indicates that the magainin family of peptides are synthesized in the stomach, and immunohistochemical studies demonstrate that magainin is stored in a novel granular multinucleated cell in the gastric mucosa of Xenopus. This study demonstrates that the magainin family of antimicrobial peptides is found in the gastrointestinal system of X. laevis and offers an opportunity to further define the physiological role of these defense peptides.

Tracheal antimicrobial peptide, a cysteine-rich peptide from mammalian tracheal mucosa: peptide isolation and cloning of a cDNA.

Extracts of the bovine tracheal mucosa have an abundant peptide with potent antimicrobial activity. The 38-amino acid peptide, which we have named tracheal antimicrobial peptide (TAP), was isolated by a sequential use of size-exclusion, ion-exchange, and reverse-phase chromatographic fractionations using antimicrobial activity as a functional assay. The yield was approximately 2 micrograms/g of wet mucosa. The complete peptide sequence was determined by a combination of peptide and cDNA analysis. The amino acid sequence of TAP is H-Asn-Pro-Val-Ser-Cys-Val-Arg-Asn-Lys-Gly-Ile-Cys-Val-Pro-Ile-Arg-Cys-Pr o- Gly-Ser-Met-Lys-Gln-Ile-Gly-Thr-Cys-Val-Gly-Arg-Ala-Val-Lys-Cys-Cys-Arg- Lys-Lys - OH. Mass spectral analysis of the isolated peptide was consistent with this sequence and indicated the participation of six cysteine residues in the formation of intramolecular disulfide bonds. The size, basic charge, and presence of three intramolecular disulfide bonds is similar to, but clearly distinct from, the defensins, a well-characterized class of antimicrobial peptides from mammalian circulating phagocytic cells. The putative TAP precursor is predicted to be relatively small (64 amino acids), and the mature peptide resides at the extreme carboxyl terminus and is bracketed by a short putative propeptide region and an inframe stop codon. The mRNA encoding this peptide is more abundant in the respiratory mucosa than in whole lung tissue. The purified peptide had antibacterial activity in vitro against Escherichia coli, Staphylococcus aureus, Klebsiella pneumonia, and Pseudomonas aeruginosa. In addition, the peptide was active against Candida albicans, indicating a broad spectrum of activity. This peptide appears to be, based on structure and activity, a member of a group of cysteine-rich, cationic, antimicrobial peptides found in animals, insects, and plants. The isolation of TAP from the mammalian respiratory mucosa may provide insight into our understanding of host defense of this vital tissue.

Biologic and biochemic properties of recombinant platelet factor 4 demonstrate identity with the native protein.

Platelet factor 4 (PF4) is a 70 amino acid protein released from the alpha-granules of platelets after activation. The exact biologic function of this protein is unknown. We have constructed an expression vector for recombinant PF4 (rPF4) in the T7-based promoter vector pT7-7 to better study the relationship between PF4 structure and function. The protein was expressed in Escherichia coli and purified to homogeneity by heparin-agarose affinity chromatography and reverse-phase high-performance liquid chromatography. Purity of protein was confirmed by immunoblot analysis and sodium dodecyl sulfate-polyacrylamide gel electrophoresis, which resulted in a single component with a molecular weight of 8,000 daltons. The amino acid composition and sequence of the N-terminal 20 residues showed that rPF4 is identical to PF4 prepared from human platelets (hPF4), except for an N-terminal initiating methionine residue. Immunoblots revealed that rPF4 and hPF4 bound polyclonal anti-hPF4 equally well, while chemotaxis experiments demonstrated similar potencies as neutrophil attractants. Dose-dependent neutrophil chemotactic responses and competitive studies with polyclonal anti-hPF4 antiserum further demonstrate similar chemotactic properties of the two PF4 species. In conclusion, our data show that this recombinant protein and the native protein appears to have similar immunologic, heparin-binding, and chemotactic properties. The chemotactic properties of hPF4 appear to be entirely intrinsic to the protein and not due, in part, to any contaminating protein. Furthermore, our expression vector should prove useful for the construction of recombinant forms of PF4 to investigate structure/function relationships of this biologically important protein.

[On the aetiology of malignant pericardial mesothelioma (author's transl)].

Two cases of malignant pericardial mesothelioma are described. Morphologically they corresponded to the epithelial or fibrous type, respectively. In one patient, an old-metal dealer, an increased number of asbestos bodies were found in the lung parenchyma and phase-microscopy demonstrated increased asbestos needles in the lung parenchyma. Since, beyond a certain fibre size, asbestos can induce bronchial carcinoma as well as pleural or peritoneal mesothelioma, it is likely that the pericardial mesothelioma had a similar aetiology.

[Serial observations of a pulmonary echinococcus, with rupture into the bronchial system (author's transl)].

The radiological course of an echinococcus cyst of the lung before and after rupture is described, together with the cytological and histological findings. The resultant tissue changes and appropriate treatment (lobectomy) are discussed. In the presence of the appropriate radiological changes, eosinophil granulocytes in the sputum suggest cyst rupture, even in the absence of echinococcus tissue. However, bronchial asthma or Löffler's infiltrates should be excluded clinically. They delayed appearance of peripheral blood eosinophilia after cyst rupture and the development of an eosinophilic broncho-pneumonia is noteworthy.

[Pathological anatomy of the vinylchloride-disease (author's transl)].

A letal case of angiosarcoma of the liver is reported. The 38 years old chemical laboratory assistant was exposed in vinylchloride medium about 3 1/2 years. The sarcoma metastasised into many other organs. This case is the fifth one reported in the German Federal Republic, in four of the cases the patient died. VC concentration at the work place was not measured. Thrombocytopenia observed in about 80% of the VC cases was not seen. There will be possibly observed further VC-caused tumors in the population. Nevertheless there might be other even extrahepatic VC-caused tumors in man, which could be concluded by animal experimental investigations.
