Estimating employment longevity of nursing personnel.

Health-care managers require estimates of employment longevity (time from hire to termination) for use in assessing future pension obligations, costs that are functions of longevity and experience such as fringe benefits, and workforce stability. Existing approaches for obtaining these estimates are problematic, complicated by time-varying data or infeasible data requirements, and may result in biased estimators. Renewal theory is presented as a model for estimating the complete distribution of the longevity of a population when the only data available is censored (i.e., time on the job of current employees). The model's ability to estimate nursing personnel longevity is demonstrated.

Strengthening hospital nursing: innovations for the improvement of patient care.

Hospital services account for over 40% of the total U.S. expenditure on health care. A key to the provision of effective and efficient hospital services rests on an adequate number of qualified nursing staff. The demand for qualified nurses continues to outstrip the existing and anticipated supply. In addition, hospitals face a growing public accountability for quality patient care. To meet these challenges, hospitals must develop innovative strategies to strengthen hospital nursing and improve patient care. A major challenge to the industrial engineer/health systems engineer is how to best design the hospital system of the future. The purpose of this paper is to enumerate common problems experienced in hospitals and to describe the innovative solutions to these problems proposed by hospitals across the nation.

Dental practice management game: a new tool for teaching practice management.

This paper describes the structure of a dental practice management game. The game covers a wide spectrum of management decisions in setting up and in running a dental practice. The decisions considered include practice location, the form of practice, staffing mix, salaries, and the purchasing or leasing of equipment. The game also covers major daily, monthly, and yearly decisions including forecasting patient load, economic analysis, and scheduling of patients, workers, and operatories. The paper also discusses the cost and time required to run the game. Many sample printouts are included.

Off-day scheduling with hierarchical worker categories.

A work force includes workers of m types. The worker categories are ordered, with type-1 workers the most highly qualified, type-2 the next, and so on. If the need arises, a type-k worker is able to substitute for a worker of any type j greater than k (k = 1, ..., m - 1). For 7-day-a-week operation, daily requirements are for at least Dk workers of type-k or better, of which at least dk must be precisely type-k. Formulas are given to find the smallest number and most economical mix of workers, assuming that each worker must have 2 off-days per week and a given fraction of weekends off. Algorithms are presented which generate a feasible schedule, and provide work stretches between 2 and 5 days, and consecutive weekdays off when on duty for 2 weekends in a row, without additional staff.
