Orbital lymphangioma and its association with intracranial venous angioma.

Orbital lymphangioma is an uncommon, benign cystic lesion, generally manifest in childhood. It usually presents with a slowly progressive proptosis, displacement of the globe, ptosis and restriction of eye movements. Occasionally, focal lesions may remain asymptomatic. Spontaneous intraorbital haemorrhage may cause acute proptosis, compressive optic neuropathy and loss of vision. An 8-year-old girl presented with progressive left proptosis and a unilateral soft tissue mass at the supraorbital frontal region. Multiple intracranial venous angiomas were seen on MRI. Surgical decompression of the optic nerve was performed because of progressive decrease in visual acuity. We review the relevant literature; rarity of the orbital lymphangioma and its association with non-contiguous intracranial vascular anomalies were emphasized.

A short term study of the additive effect of timolol and brimonidine on intraocular pressure.

PURPOSE: To evaluate the additive ocular hypotensive effect of the combination of brimonidine and timolol on intraocular pressure (IOP) reduction in patients with glaucoma. METHODS: This was a prospective, randomized, double-masked, crossover study in 20 patients with primary open angle glaucoma (POAG) on therapy receiving timolol maleate 0.5% twice daily, with IOP greater than or equal to 22 mmHg in one eye. The treatment period was 3 weeks and during this period timolol + brimonidine or timolol + placebo were applied topically twice daily and IOP, blood pressure, heart rate and pupil size were measured. RESULTS: Combined therapy (timolol + brimonidine) had clinically significant IOP-lowering effect during the treatment period P << 0.01). The mean diurnal IOP was significantly reduced by an average of 5.1-5.9 mmHg (21.2-24.5%) compared with baseline value. The timolol + placebo combination had no clinically significant IOP-lowering effect (P > 0.05). No clinically significant side effects were observed during the treatment of both groups. CONCLUSIONS: This study showed that the combination of topically applied brimonidine and timolol cause a marked and sustained IOP reduction.

Synthesis of some novel 3-methyl-6-(2-substituted propanoyl/propyl)-2-benzoxazolinone derivatives and anti-nociceptive activity.

In this study, 12 new 3-methyl-6-(2-substituted aminopropanoyl)-2-benzoxazolinone and 3-methyl-6-(1-hydroxy-2-substituted aminopropyl)-2-benzoxazolinone derivatives have been prepared. Their structures have been elucidated by IR, 1H NMR spectra and by elementary analysis. The anti-nociceptive activity of these compounds has been investigated by using a modified Koster test. It was found that most compounds are capable of inducing anti-nociception in animals.

Extracapsular cataract extraction with posterior chamber lens implantation in capsular glaucoma.

PURPOSE: To evaluate intraocular pressure (IOP) control after extracapsular cataract extraction (ECCE) with posterior chamber intraocular lens (PCIOL) implantation in patients with capsular glaucoma (CG) and coexisting cataract. METHODS: This prospective study included 20 patients (20 eyes) having CG and cataract whose IOPs were under 22 mm Hg and controlled with antiglaucoma medication. All patients had ECCE with PCIOL implantation and the follow-up period was at least 18 months. IOP was measured postoperatively at 3, 6, 9, 12 and 18 months and compared with preoperative IOP. RESULTS: Following the cataract extraction, PCIOL implantation produced a statistically significant reduction in IOP at all time points compared with the preoperative IOP during the study period (p < 0. 001). The mean preoperative IOP was 18.25 +/- 1.83 mm Hg; postoperatively at 1 month, it was 13.45 +/- 2.06 mm Hg; at 3 months 14.80 +/- 2.50 mm Hg; at 6 months 15.35 +/- 1.27 mm Hg, at 12 months 14.85 +/- 1.87 mm Hg and at 18 months 15.15 +/- 1.42 mm Hg (p < 0. 0008). The mean reduction in IOP was 16.98% from baseline at 18 months postoperatively. The mean number of antiglaucoma medication was reduced from 1.35/eye preoperatively to 0.60/eye postoperatively at 18 months (p < 0.0007). CONCLUSION: The result of our study revealed that ECCE with PCIOL implantation may be a reliable choice in controlling IOP in patients with CG.

Synthesis of some new pyridyl-ethylated benzoxazolinones with analgesic and anti-inflammatory activities.

Twelve new 3-[2-(2- and/or 4-pyridyl)ethyl]benzoxazolinone derivatives have been synthesized by reacting 2- and/or 4-vinylpyridine and appropriate benzoxazolinones. Their chemical structures have been proven by IR, 1H-NMR and elemental analysis. Analgesic activities of these compounds were investigated by a modified Koster and constant temperature hot-plate test. Test results revealed that most of the compounds at 100 mg/kg dose level showed significant analgesic activities when compared to acetylsalicylic acid and morphine. Therefore the compounds were screened for their anti-inflammatory activities using the carrageenan hind paw edema test. Compounds 3-8 were found more active than indometacin. In gastric ulceration studies, any of the compounds showed no gastrointestinal bleeding at 100 mg/kg dose level.

Monotonic algorithms for transmission tomography.

We present a framework for designing fast and monotonic algorithms for transmission tomography penalized-likelihood image reconstruction. The new algorithms are based on paraboloidal surrogate functions for the log likelihood. Due to the form of the log-likelihood function it is possible to find low curvature surrogate functions that guarantee monotonicity. Unlike previous methods, the proposed surrogate functions lead to monotonic algorithms even for the nonconvex log likelihood that arises due to background events, such as scatter and random coincidences. The gradient and the curvature of the likelihood terms are evaluated only once per iteration. Since the problem is simplified at each iteration, the CPU time is less than that of current algorithms which directly minimize the objective, yet the convergence rate is comparable. The simplicity, monotonicity, and speed of the new algorithms are quite attractive. The convergence rates of the algorithms are demonstrated using real and simulated PET transmission scans.

3-substituted piperazinomethyl benzoxazolinones. Analgesic and anti-inflammatory compounds inhibiting prostaglandin E2.

Fourty-three new benzoxazolinone derivatives having a piperazinomethyl group at the third position of the ring were synthesized by using appropriate benzoxazolinones and 4-substituted piperazines via a Mannich reaction. The structures of the compounds were elucidated by spectral data and microanalyses. Analgesic activities were evaluated by a modified Koster test. All compounds, except 7, 14, 21, 32, and 41, showed analgesic activity higher than that of acetylsalicylic acid. The compounds were also screened for their anti-inflammatory activity using a carrageenan paw edema test, and those exhibiting high anti-inflammatory activity were investigated for their ability to inhibit prostaglandin E2 induced paw edema. The results of anti-inflammatory testing indicated that most of the compounds were more active than indometacin. Ulcerogenic activities of the compounds were also studied and no gastrointestinal bleeding was observed at the 100 mg/kg dose level.

Some new benzoxazolinone derivatives with analgesic and anti-inflammatory activities.

Fourteen new 6-acyl-2-benzoxazolinone, ethyl-(6-acyl-2-benzoxazolinone) acetate and (6-acyl-2-benzoxazolinone-3-yl) acetic acid derivatives were synthesized, and their physical properties and UV absorption data were determined. Their chemical structures were elucidated by IR, 1H-NMR and elemental analysis. The analgesic activities of these compounds were investigated by a modified Koster's test. It was shown that the respective activities of compounds 1-4 and 10-14 were higher than that of acetylsalicylic acid. Therefore, they were screened for additionally anti-inflammatory activities using the carrageenan mouse paw edema test. The compounds were further screened for their ability to inhibit prostaglandin E2 (PGE2) induced paw edema and compared to indometacin. Compounds 12 and 13 were very potent in inhibiting PGE2 induced edema.

[The antigenic relationship between Syringospora clausseni and the Salmonella C(1) group O antigen].

The antiserum against Salmonella (0 6, 7) antigen aggultinated Syringospora clausseni, and the serum against this fungus cross-agglutinated Salmonella (0 6, 7) antigen. Cross aglutination and absorption experiments showed that there was a common antigenic determinant between Syringospora clausseni and S. cholerae suis (6,7). This antigenic determinant may resemble the Minor antigen which is not used in usual sero-typing of Salmonella.

Effect of ketanserin administration on intraocular pressure.

PURPOSE: We evaluated the effect of oral single-dose (20 mg) ketanserin on intraocular pressure (IOP) in normotensive and hypertensive eyes. METHODS: This study included 15 healthy volunteers and 16 patients with ocular hypertension. Systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate, IOP and pupil diameter were recorded at baseline and at 1-hour intervals for 6 h, in addition, tonographic outflow facility was studied at the third hour after the administration of placebo or an oral single dose of 20 mg ketanserin given in a randomized double-blind crossover fashion. The alternative treatment was applied a week later. RESULTS: In both groups, oral single-dose (20 mg) ketanserin significantly lowered IOP and SBP (p < 0.01). No variation was observed in DBP, heart rate and pupil diameter (p >0.01). Moreover, after drug administration, the total outflow facility measured by conventional tonography increased in a statistically significant way (p < 0.01). Placebo did not induce any significant reduction in IOP and SBP in either group. CONCLUSION: The results showed that systemic ketanserin can be used in the treatment of glaucoma patients to reduce IOP.

Cyclosporin A plus prednisone treatment of steroid-sensitive frequently relapsing nephrotic syndrome in children.

Recently, there have been numerous reports on the use of cyclosporin A (CyA) in children with nephrotic syndrome (NS). In this prospective study, we wanted to evaluate the efficacy of CyA together with prednisone therapy in children with steroid-sensitive frequently relapsing NS. A total of 11 children (7 boys, 4 girls) with steroid-sensitive NS were included in this study. The patients ranged in age from 3.5 to 15 years (average 8.45 +/- 4.26 years). Renal biopsy showed minimal change disease in five, mesangial proliferation in four, focal glomerulosclerosis in one and membranous glomerulonephritis in one. The NS had lasted from 13 to 113 months (average 50.27 +/- 38.60 months). The number of relapses varied from three to 10 episodes with an average of 5.9 +/- 3.3 episodes. Patients received 5 mg/kg CyA daily in two divided doses for five months and prednisone for a total of eight weeks (30 mg/m2 daily for 4 weeks followed by 30 mg/m2 on alternate days for 4 weeks). After the completion of the treatment protocol, no therapy was given unless a relapse was observed. Mean follow-up period was 14.9 +/- 5.99 months with a range from six to 26 months. Before this combined treatment, there was a mean relapse rate of 0.144 +/- 0.05 relapses month with a range from 0.088 to 0.238. After discontinuation of therapy, the relapse rate dropped to a mean of 0.0179 +/- 0.031 with a range of 0 to 0.083. In conclusion, it would appear that a combination of CyA and prednisone is effective, sustaining the remission in steroid-sensitive NS. Corticosteroids in combination with CyA may be a better approach than conventional steroid treatment in such patients.
