[Diverticular disease of the colon].

The great significance of diverticular disease, clinical symptoms, and pathologic-anatomical changes are demonstrated. In our opinion the diagnostic possibilities of a carefully performed hypotonic enema with double contrast are not yet fully used; mostly the patients have to suffer from an endoscopic investigation of the colon. Radiologic double-contrast examination is the method of choice, resulting in reliable and clear pictures of the extent of diverticulosis and in most cases also of its complications. Some typical examples are given to point out radiological criteria of the diverticular disease.

[The significance of x-ray-examination in early diagnosis of colon carcinoma compared with colonoscopy (author's transl)].

The medical literature and our own observations show the importance of x-ray examinations to diagnose colon carcinoma. The conventional barium enema examination without double contrast is not suitable for a general medical check-up. In contrast to the diagnostic value of the conventional procedure, the diagnostic accuracy of double contrast examination is comparable to that of the colonoscopy. In 1108 patients, endoscopically proven polyps smaller than 1 cm were detected in 61%, whereas the percentage for polyps bigger than 1 cm was 87. These results are achieved only in patients with a sufficient preparation. Two of 25 colorectal carcinomas were detected neither by endoscopy nor by the double contrast method.

Barium Sulfate↗

[Specific oesophagitis (author's transl)].

Tuberculous oesophagitis is a very rare finding; of all organs, involvement of the oesophagus is the least likely. Autopsies on tuberculous patients show an incidence of 0.14%. According to Wexels, only 125 cases have been described in the world literature up to 1954. In general, tuberculous oesophagitis can be included in the differential diagnosis of dysphagia only if this symptom is associated with tuberculosis of an organ, or miliary tuberculosis.

Deglutition Disorders↗

[Syndrome of the unilateral or partially "hyperlucent lung"].

The paper presents the picture of the unilateral or partial "hyperlucent lung" syndrome. The most common causes of this anomaly are described according to pathological and anatomical criteria. The difficult diagnosis of its X-ray-appearance is discussed. The value of differential diagnostic criteria is demonstrated in suitable cases. Differentiation from the pseudo-syndrome is also outlined. Various methods of radiological examination are described and evaluated.

Angiocardiography↗

[X-ray symptoms in ulcerative colitis].

Barium enema studies in 200 patients with ulcerative colitis were analyzed according to radiological symptoms. The results of the radiological examination were compared with those of rectoscopy and biopsy. In about 75 per cent of patients good agreement was achieved.

[Pharmacoradiography of the gastro intestinal tract (author's transl)].

Pharmacoradiography of the digestive system improves often accuracy of diagnostic information compared to conventional X-ray examinations. Pharmacoradiography should be used more often; the examiner has to be familiar with the principal effects and side effects of the pharmaceutica. The present paper gives a short summary about drugs and examination techniques most often used.

Atropine↗

[The diagnosis of gall-stones by ultrasonics (author's transl)].

Ultrasonic and radiological tests were compared in 267 patients suspected of having gall-stones. Radiological diagnosis was not possible in 20, because of lack of opacification, while in nine a gall-bladder was not demonstrable by ultrasound. In 226 the two tests coincided (positively or negatively), with 21 negative cholecystograms being interpreted as proof of stone. Seven of twelve discordant results were proven at operation: in three the ultrasonic, in four the radiological diagnosis was confirmed. Because of its accuracy, simplicity and absence of any risk ultrasonics is recommended as the first test to be performed if cholelithiasis is suspected.

Cholecystography↗

[Multilocular tumors of small intestine (author's transl)].

Tumors of small intestine can be recognized in time and evaluated histologically by means of roentgenographic and endoscopical-bioptic methods, respectively. Among radiologic techniques, the double contrast examination of the small intestine has especially high diagnostic value. On the other hand, selective angiography reveals bleeding of accompanying small intestinal tumors. Out of 23 patients with these tumors -- diagnosed in the Medical Department of the Erlangen University from 1968 to July 1974 -- there was one case with multilocular occurrence; the diagnostic efficiency of double contrast technique proved to be particularly suitable in this patient.

Adenocarcinoma↗

[Unusual clinical course of malignant lymphoma(author's transl)].

Primary malignant lymphoma of the gastrointestinal tract and intestinal involvement in diffuse lymphoproliferative diseases are far more common than generally believed. A typical clinical course of these disorders may rose considerable diagnostic problems. Scirrhous growth patterns, diffuse gastrointestinal polyposis and multiple colonic ulcers are presented as rare variants of malignant lymphoma of the digestive tract.
